A RESOLUTION OF THE CITY COMMISSION OF THE
CITY OF BOYNTON BEACH, FLORIDA, AUTHORIZING
AND DIRECTING THE MAYOR AND CITY CLERK TO
EXECUTE A CONTRACT BETWEEN THE CITY OF
BOYNTON BEACH AND CHILDREN'S SERVICES
COUNCIL IN THE AMOUNT OF $55,886.00 FOR THE
2000-2001 BOYNTON BEACH LATCHKEY
AFTERSCHOOL PROGRAM; AND PROVIDING AN
EFFECTIVE DATE.
WHEREAS, the City Commission of the City of Boynton Beach, upon
recommendation of staff, deems it to be in the best interests of the citizens of the
City of Boynton Beach to execute a Contract with the Children's Services Council
(CSC) in the amount of $55,886.00 for the funding of the 2000-2001 Boynton Beach
Latchkey After School Program; and
WHEREAS, the Latchkey After School Program is designed to assist those
in need of day care services at Rolling Green Elementary School, and is a
collaborative effort between the City, Children's Services Council (CSC) and the
School Board of Palm Beach County; and
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF
THE CITY OF BOYNTON BEACH, FLORIDA, THAT:
Section 1. The City Commission of the City of Boynton Beach hereby
authorizes and directs the Mayor and City Clerk to execute a contract with the
Children's Services Council (CSC), for grant funding in the amount of $55,886.00
to be utilized as primary funding for the Latchkey After School Program.
Section 2. This Resolution shall become effective immediately upon
passage.

MODIFICATION AND EXTENSION AGREEMENT
This Agreement, entered into as of this 1st day of October, 2000 by and between the
CHILDREN'S SERVICES COUNCIL OF PALM BEACH COUNTY, a political subdivision of
the State of Florida (herein. after refer[fid to as "Council") and City of Boynto,n
BeaclffDepartment of Leisure Serwees/Latchkey After School Program (,hereinafter referred
to as "Provider"), for the period from October 1, 2000 ("Effective Date") to September 30, 2001
WHEREAS, by Agreement dated as of October 1, 1998 (the "Original Agreement"),
Provider agreed to provide certain services and Council agreed to reimburse Provider for those
services,in program entitled Latchkey After School Program, and~ -
WHEREAS, the parties, pursuant to the terms of this Agreement, hav~decided to modify
and e/tend the Original Agreement.
WITNESSETH:
1N CONSIDERATION of mutual agreements hereinafter contained, the parties intending
to be legally bound hereby agree as follows:
1. The Original Agreement is hereby modified by amending paragraph 1 thereof to read
as follows:
The Council agrees to reimburse to the Provider sums up to $55,8~t6.00 for provision
of services described in Exhibit "A" (consisting of the Pr~)gram Ev~uation Model)
attached hereto and made a part hereof,, composite Exhibit B (coi~ststmg oft e
Council's Budget Forms A, A1 and B submitted by Provider) attac~ed hereto and
made a part hereof; and Exhibit "C" (consisting of the Program Da[a Form) attached
hereto and made a part hereof. It is expressly understood and agreed, by and between
the parties, that the services to be rendered hereunder by the Provider shall be
provided in compliance with all terms and conditions set forth in the Exhibits "A", "B",
and "C" and with the Conditions of Award attached hereto and made a part hereof as
Exhibit "D", as well as the terms and conditions of this document..T, he attached
Exhibits to this Agreement shall control over any inconsistent provisions in the
Exhibits attached to the Original Agreement.
2. The Original Agreement is hereby modified by amending paragraph 14 thereof to read
as follows:
14. Payments for services rendered pursuant to this Agreement shall be made to the
Provider monthly on a reimbursement basis. Provider shall sub~, it an invoice to the

amount budgeted, the Provider will notify the Council staff of the excess
revenues and plans to expend such fees. The Provider agrffes to prioritize the
use of excess fees in the following order:
a. Increase the number of clients attending the program within licensed
capacity.
b. Purchase those items which would be considered "enhancements" to
the service being provided in order to improve the quality of the
program.
c. Reduce the Council's funding by the amount of the excess fees received.
3. All fees generated by the Council funded program will be recorded in a
separate revenue account.
