I. Call to Order
Chair Bobich called the meeting to order at 11: 18 a.m.
II. Roll Call
The Recording Secretary called the roll and declared a quorum was present.
III. Acceptance of Minutes of 09/10/2007 regular meeting
Revisions to the attendance portion of the minutes were made as follows:
Mr. Landress' name is Mike Landress, not Frank. Glenda Hall and Tom Balfe were not
present. Ms. Scott explained Communications was part of Police and there was no
need to require both departments to be present.
Motion
Vice Chair Magazine moved to approve the minutes as corrected. Ms. Mack seconded
the motion that unanimously passed.

IV. Safety Committee Education and Training Program
Eric Falstad, Assistant Golf Course Superintendent, announced there was an accident
at the golf course involving two employees. He announced they have a tractor driven
mower with seven mowing blades behind it. When the tractor pulls it, there is a turning
reel that cuts the grass. There was a problem with the mower. The mechanics came
and fixed it and when they went to leave, one mechanic was too close. The mower reel
clipped his legs and knocked him over the reel to the other side of the mower fracturing
his arm. An investigation took place and an interpreter was brought in because there
was a language barrier. One issue was when the red light on the switch was off, he
thought the problem was corrected. Mr. Falstad thought it could have been avoided.
No machinery should be on when being serviced and the keys should be removed, the
vehicle parked in neutral and the emergency brake on. A simple issue turned into
multiple issues.
Mr. Altman asked who received a copy of the accident report and advised this could go
on the Near Miss Reporting System.
Vice Chair Magazine announced employees should not be filling out the Supervisors
Accident Report. He commended the Golf Course for the way they investigated the
incident. Mr. Falstad took minutes of the meeting and Vice Chair Magazine advised
they will review the minutes at the next Safety Meeting.
Mr. Altman will add the incident to the Near Miss Reporting System. This type of
incident should be discussed by the other safety coordinators at their Safety
Departmental Meetings.
Vice Chair Magazine distributed information promulgated by the Mayo Clinic on Office
Space Ergonomics. He invited the members to review the list of other slide shows
available.
V. Old Business
A. Fire Drills
Mr. Altman was not aware of any fire drill that took place. The process to schedule a
fire drill was to contact Fire and Life Safety. Mr. Macintyre handles them.
Ms. Rivers reported the Hester Center had a scrub fire recently and the Fire Department
was contacted. She advised City personnel reacted properly.

C. Departmental Inspections & Safety Meetings
No report given.
D. Defensive Driver Course
1. Solid Waste, special driving class
Vice Chair Magazine sent out the schedule for the Defensive Driving courses for the
year. Solid Waste Department employees would attend classes at a different time in
order not to interfere with their driving schedules. Vice Chair Magazine advised special
hours or days can be accommodated in order to obtain compliance.
E. Incident Review Board Referrals
None.
F. Broken curb and wheel stops - Development
Ms. Quinn reported Glenda Hall was working on the issue. Ms. Quinn took photos of all
repairs she could find that needed to be made and turned them over to Ms. Hall. Items
would be removed as opposed to repaired. Some wheel stops may be recycled plastic.
G. Smoking Cessation Classes - Human Resources
Ms. Mack reported Pat Sholos, the Benefits Coordinator, will send out an email polling
employees to determine if there is any interest in scheduling more classes. She
anticipated having more information next month.
H. Near Miss Accident Reporting System
More information will be forthcoming and distributed to all the departments
VI. New Business
A. Departmental Safety Issues
Glendon Morgan reported there was a slip and fall by the kitchen and an employee
bruised her side. Vice Chair Magazine reported they would install a handrail and
recommended the floors not be heavily waxed. A mat would not correct the situation
and he determined placing strips there would not improve the matter. The gradient is
within life safety standards. Chair Bobich explained they were looking for posters, or
making one to bring back to the committee, that would urge employees noticing
anything on the floor, to pick it up and not wait for anyone else to do so.

Ms. Welsh reported Facilities also installed a metal strip by the City Managers office to
address water issues and Mr. Bressner requested a plastic line be put on the end of it
because it was sharp. She was still waiting for Facilities to respond. Vice Chair
Magazine recommended purchasing an edge guard from the hardware store. The lobby
rug, when it rains gets very wet and at least one employee has slipped. Also the lobby
roof leaks. It was thought adding another mat to the lobby when it rains might help.
Vice Chair Magazine will look into the matter.
The safety footwear list was distributed and the form was the same as last year. This
was for blue and white collar workers and how it pertained to the Personal Protection
Equipment. There was discussion on firefighters shoes being included as part of their
uniform and being replaced as part of the uniform item or under the union bargaining
agreement. If it was part of the uniform they could remove the shoes and cover it under
the bargaining agreement. The committee will approve the list and turn the list over to
Finance. It was reported the City was holding up the checks. The Blue Collar
Bargaining Agreement had expired and negotiations continued.
Vice Chair Magazine reported the need for shoes was based on their job description.
New employees or employees unrepresented, would have reimbursement provided by
the 121 form.
Motion
Mr. Falstad moved to approve the list. Ms. Welsh seconded the motion that
unanimously passed.
A fast food calorie reference handout was reviewed.
VII. Announcements
Chair Bobich read an email pertaining to employee security at Intracoastal Park
Clubhouse on nights when there may be only one employee leaving when it is dark.
Ginny Shea had sent an email advising the janitor would walk employees out to their
cars. Employees were also encouraged to move their parked cars closer to the building
before dark. The Police could patrol the park during that time window. Ms. Rivers
anticipated that there would be many people at the park after dark.
VIII. Next Meeting Date
The next meeting will be November 19, 2007 due to the regular meeting falling on
Veterans Day.
