FREYNEL R JEAN BAPTISTE
417 NORTH FEDERAL HWY
BOYNTON BEACH FL 33435
Dear Mr. Jean Baptiste:
This letter will serve as notice that I have reviewed your Campaign Treasurer's Report
that was submitted by the close of business at 5:00 p.m. on Monday, January 12, 2009.
The following corrections need to be made; therefore, you will need to file an
"Amendment" to the report you submitted:
1. You did not include an "Itemized Contributions" form which details how you
got the $750. You will need to include that form (copy attached).
2. On your "Itemized Expenditures" forms, you need to amend the sheets to
indicate the purpose of the expenditures in "Column 8" and use the description
code found on the back of the form in "Column 9". (copy attached)
3. Sequence #7 refers to DHSMV. Please write out what these letters stand for
and what is the purpose.
3. Sequence #9 shows that you had an expenditure of $100, but you did not
explain the purpose of the expenditure or provide the Description Code for that
item.
In accordance with FS 107.07(2)(b)1., "Any report which is deemed to be incomplete
by the officer with whom the candidate qualifies shall be accepted on a conditional
basis, and the campaign treasurer shall be notified by registered mail as to why the
report is incomplete and be given three (3) days from receiDt of such notice to
file an addendum to the reDort providing all information necessary to complete the
report in compliance with this section. Failure to file a complete report after such notice
constitutes a violation of this chapter."
America's Gateway to the Gulfstream
The City of Boynton Beach
City Clerk's Office
In addition, please be advised that in accordance with FS 106.07.8(b), the fee for late
filing is $50 per day for the first three (3) days late, and, thereafter, $500 per day for
each late day, not to exceed 25% of the total receipts for expenditures, whichever is
greater, for the period covered by the late report. The fine must be paid from the
candidate's Dersonal funds - not campaign funds.
I have enclosed blank "Itemized Contributions", "Itemized Expenditures" and Campaign
Treasurer's Report Summary. I have also included a copy of the report you submitted
so that you will have it to make your corrections. Please amend your report following
the directions found on the back of each of the forms and submit the report to
me within three (3) days of receipt of this letter.
Please note that I tried to contact you by telephone today in an effort to speed up this
process; however, the message I received was that the number was temporarily
disconnected. If you have any questions, please contact me.
Very truly yours,
CITY OF BOYNTON BEACH

(1) Type candidate's full name or name of the political committee (PC), committee of continuous existence (CCE) or
party executive committee (PTY).
(2) Type identification number assigned by the Division of Elections,
(3) Type cover period dates (07/01/03 through 09/30/03). (See Calendar and Election Dates for appropriate cover
periods.)
(4) Type page numbers (e.g., 1 of ~).
(5) Type date of expenditure (Month/DaylYear).
(6) Sequence Number - Each detail line shall have a sequence number assigned to it. Sequence numbers are to be
assigned within each reporting period and for each type of detail line. Thus the report type, detail line type, and
sequence number will combine to uniquely identify a specific contribution, expenditure, distribution or fund transfer.
This method of unique identification is required for responding to requests from the Division and for reporting
requirements.
For example, a Q 1 report having 40 expenditures would use sequence numbers 1 through 40. The next report
(Q2), comprised of 30 expenditures would use sequence numbers 1 through 30. Expenditures on amended Q1
reports would begin with sequence number 41 and on amended Q2 reports would begin with sequence number 31
See Amendment Type instructions below.
(7) Type full name and address of entity receiving payment (including city, state and zip code).
(8) Type purpose of expenditure (if expenditure is a contribution to a candidate, also type the office sought by the
candidate). PLEASE NOTE: This column does not apply to candidate expenditures, as candidates cannot
contribute to other candidates from campaign funds. However, PCs (supporting candidates), CCEs and party
executive committees contributing to candidates must report office sought (Section 106.07, FS.)
(9) Enter Expenditure Type using one of the following codes:

REF

(10) Amendment Type (required on amended reports) - To add a new (previously unreported) expenditure for the
reporting period being amended, enter "ADD" in amendment type on a line with ALL of the required data
The sequence number for expenditures with amendment type "ADD" will start at one plus the number of
expenditures in the original report. For example, amending an original Q 1 reports that had 75 expenditures, means
the sequence number of the first expenditure having amendment type "ADD" will be 76; the second "ADD"
expenditure would have sequence number 39.
To correct a previously submitted expenditure use the following drop/add procedure. Enter "DEL" in amendment
type on a line with the sequence number of the expenditure to be corrected. In combination with the report number
being amended, this sequence number will identify the expenditure to be dropped from your active records. On the
next line enter "ADD" in amendment type and ALL of the required data with the necessary corrections thus
replacing the dropped data. Assign the sequence number as described above.
(11) Type amount of expenditure.
