A RESOLUTION OF THE CITY COMMISSION OF
THE CITY OF BOYNTON BEACH, FLORIDA,
AUTHORIZING AND DIRECTING THE MAYOR AND
CITY CLERK TO EXECUTE AN AGREEMENT
BETWEEN THE CITY OF BOYNTON BEACH AND
THE SCHOOL BOARD DISTRICT OF PALM BEACH
COUNTY FOR THE EDUCATION AND TRAINING OF
STUDENTS IN THE PARAMEDIC PROGRAM; AND
PROVIDING AN EFFECTIVE DATE.
WHEREAS, the City Commission of the City of Boynton
Beach, Florida, upon recommendation of staff, has deemed it
to be in the best interests of the citizens and residents of
the City of Boynton Beach to enter into an Agreement between
the City of Boynton Beach and The School Board District
of Palm Beach County for the education and training of
students in the Paramedic Program, said Agreement being
attached hereto as Exhibit "A"
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF
THE CITY OF BOYNTON BEACH, FLORIDA, THAT:

AND
CITY OF BOYNTON BEACH
II.
PARTICIPATING AGENCIES:
The participating agencies in this agreement are The School District of Palm Beach County, hereinafter
called the "SCHOOL DISTRICT," and the City of Boynton Beach , hereinafter called the EMS
"PROVIDER."
STATEMENT OF AGREEMENT:
This is a mutual agreement between the administration of the Provider and The School-District of Palm
Beach County, Florida, that the EMS Provider will accept emergency medical tectmology/EMT students
from the vocational-technical education programs for supervised learning experiences in the care of patients
in accordance with the provisions set forth in this agreement.
Whereas, the parties hereto recognize that the School District is in need of health care facilities to train
vocational education students in the area of emergency medical technology education pursuant to the State
Plan for Vocational Education of the State Department of Education and The School District of Palm Beach
County policies and procedures; and
Whereas, the parties hereto recognize that interest in promoting those educational goals and objectives of
the State Department of Education and The School District of Palm Beach County which further excellence
in emergency medical services; and
Whereas, the Provider desires to provide the necessary Emergency Medical Services facility for observation
and selected clinical experiences to assist this program both parties therefore agree, in consideration of the
mutual covenants and agreements expressed herein, as follows:
GENERAL PROVISIONS OF THE AGREEMENT:
The education of the student shall be the primary purpose of the training program.
The School District shall be responsible for the education of the student.
The School District shall be responsible for selecting learning experiences for the students
with the assistance and cooperation of the EMS Provider personnel.

The Emergency Medical Services Provider agrees to share in the education of the student
through the cooperation and assistance of its staff with the faculty of the School District
and in the guidance and supervision of the students.
The number of students scheduled to a Provider will be planned by the School District
and the Provider.
Students will be. involved chiefly in learning experiences through observation; however,
in order to conform to the requirements of the Florida Department of Education Program
Course Standards and the Department of. Health and Rehabilitative Services Rules,
didactic instruction reinforced in the clinical area will be offered by the Provider. Any

THE SCHOOL DISTRICT,S RESPONSIBILITY:

The School District will maintain stalldarcks which include, provision for physical
examination, T.B. screening, RUbella vaccination, serology, hepatitis B vaccine or signed
waiver and education on universal precautions and blood borne pathogen standards; said
standards to be provided priOr to ambulance riding experience.

medical technology practice, and evaluation of student competency and health.
Preparing a clinical rotation plan for s~rvices to be used for experience and
securing the written approval of the plan from the Director of the Emergency
Medical Services facility prior to the beginning of the .affiliation: Before any
material changes are made in the plan, they will be discussed and approved in
writing by the Director of the Emergency Medical Services facility.
THE EMERGENCY MEDICAL SERVICES FACILITY'S RE3PONSIBILITY:
To make available to faculty and students facilities and clinical services for planned
learning experiences through observation and seleCted clinical experiences in the
emergency medical technology curriculum.
To share and assist in the responsibility for the guidance and supervision of the students,
particularly in restricted areas.
The Provider may include members of the faculty of the School District in staff meetings
when policies to be discussed will affect or are related to the emergency medical
teChnology program. The Provider shall, at all times, provide The School District with
written notice of the changes in policy and procedures in a timely manner,especially as
such policies and procedures relate to or affect the emergency medical teChnology
program.

THE SCHOOL DISTRICT'S POLICIES:
The educational program shall consist of no less then six hours per week of classroom
instruction, laboratory practice and experiences in seleCted learning situations in hospitals
and other health agencies. The division and arrangement of tifiae to include the
theoretical and clinical learning experiences shall be determined by the faculty, and be
based upon thc needs of the students for specific learning experiences to meet the
objeCtives of the program.
Students will be assigned for emergency medical services, clinical and pre-hospital
experience Monday through Sunday during the hours of 8 a.m. to 11:00 p.m. for 6 to
8 hour shifts.
3. Students will be given holidays as provided for in the regular school calendar.

REQUEST FOR WITHDRAWAL OF STUDENT:
The participating Provider has the right to request the School District to withdraw any student from
its facilities whose conduct or work is not, in the reasonable.opinion of the administrator of the
EMS facility, in accordance with acceptable standards of performance. The School District may

TERMINATION OF AGREEMENT:

MODIFICATION OF AGREEMENT:
Modification of the agreement shall be made only upon mutual consent of both parties and a
contract addendum shall be executed by both parties hereto.
TITLE IX AGREEMENT:
The participating agency agrees not to discriminate 'against ally traillcc in any manner whatsoever
on account of race, creed, color, age, sex, handicap or national origin.
J. LENGTH OF AGREEMENT:
This agreement will be effective for the fiscal year July 1, 1994, tln~ugh June 30, 1995.
K. RENEWAL OF AGREEMENT:
It is the intent of both parties to review this agreement each year and renew if approved by both

COPIES OF THIS AGREEMENT:
This agreement shall be governed by the laws of the State of Florida and if any dispute arises, then
venue shall be in Palm Beach Count,.
Copies of this contractual agreement shall be submitted to the office of the State of Florida
Department of Health and Rehabilitative Services.
Copies of the contractual agreement shall be placed.on file in the Vocational-Techaical Education
Department.

ACTION OF SEPTEMBER 22, 1994
WHEREAS, the SoUth Central Regional Wastewater Treatment and Disposal Board did
on September 22, 1994, by a vote of 7-3, approve Proposal A Organization with
modification that Legal Council be shown directly responsible to the Board and
Consultants, Engineers, and Auditors be responsible to the Executive Director. The
Position shown as Secretary be changed to Administrative Assistant I and Bookkeeper

Purpose for the Position:
This position is responsible for the direction and coordination of the financial affairs of the
Board, including investing, billing, accounts payable/receivable and weekly payroll activities.
This position is also responsible for maintaining the general ledger and assists in preparation of
the financial statements to provide management with accurate information regarding the
financial position of the Board and to satisfy the requirements of the appropriate regulatory
agencies.
Maintains personnel records.
In conjunction with the above to be responsible for the coordination of the purchasing activities
of the Board.

Essential Responsibilities:
1. Provides Executive'Director with reliable reports on the current financial position
by maintaining appropriate accounting systems enabling timely and accurate
preparation of financial statements.
2. Participates in controlling capital and operating costs by assisting in the preparation of
budgets and reporting budget compliance information and cumulative project cost data
to the appropriate department personnel. :
3. Oversees and maintains all the General Ledger Systems and Schedules, including
Accounts Receivable, Accounts Payable, Balance Sheet and Chart of Accounts.
Reconciles bank statements.
4. Responsible for account transfers and investments.
5. Types vouchers, invoices, account statements, reports, etc. using the computer.
6. Verifies and posts details of cash receipts and disbursements, using a computer.
7. Updates the inventory schedules of the Repair and Replacement Fund based on original
purchase price, life expectancy, changes concerning the assets, and other significant
information.
8. Must collect and verify accuracy of all employees' time sheets, and submit the same in a
timely manner.
9. Enters approved budget changes or transfers into the computer.
10. Prepares all payroll related reports/tax returns and deposits.
11. Maintains all schedules pertaining to employee compensated absences.
12. Responsible for completing the standard accounting procedures checklist by the 21 st of
each succeeding month.
13. Provides all information for the auditors quarterly and annual review in a complete and
timely manner.
14. Keeps record of insurance coverage and pension plan and assist in the coordination and
processing of insurance claims and respond to inquiries fa'om outside agencies and Board
personnel.
