Report for Vehicles/Equipment PURPOSE To establish a standard Pre-Trip/Safety Check and Inspection for all City owned and operated equipment in the medium-to-heavy class (attached list).
OBJECTIVE To enhance the operator’s ability to monitor excessive fluid uses, and other potential problem areas and report to Fleet Maintenance any concerns before they can develop into
larger problems. RESPONSIBILITY Vehicle operators are responsible to perform daily inspections and to complete the Pre-Trip Inspection Report form. PROCEDURE Use JJ Keller Driver’s Vehicle
Inspection Report (116-B) a 3 part form available through the Warehouse. Vehicle operators shall complete the Pre-Trip Inspection Report in all areas that are applicable for the type
of vehicle being operated. White copy will be given to the Supervisor, Yellow copy given to Fleet Maintenance followed up with a Vehicle/Equipment Repair Request if any discrepancy is
