(CONSULTANT) to provide engineering services for various general activities, approved by the Boynton Beach City Commission on July 1, 2014 This Task Order will be performed under that AGREEMENT SCOPE OF SERVICES The CITY has requested a proposal under Scope A of the AGREEMENT to provide Construction Management Services for the Raw Water Main — Section C (PROJECT) from the CONSULTANT to provide construction management services for the PROJECT. The PROJECT includes the installation of 36 -inch ductile iron and high density polyethylene (HDPE) piping by open cut, directional drill, and jack and bore construction methods. The route of the pipeline is along Woolbright Road, beginning west of Congress and ending east of Interstate 95 The PROJECT has 100% Design Documents (i e drawing and specifications) prepared by Brown and Caldwell (Engineer of Record) for bidding CONSULTANT shall provide general project administration for the anticipated twelve month duration of the PROJECT. CONSULTANT shall provide a Project Manager for the twelve month project duration and a Resident Project Representative (RPR) for an anticipated six month period of actual field construction Services to be supplied by CONSULTANT are described in the proposal request prepared by the CITY dated August 7, 2014 (Attachment A) No exceptions are taken to requested tasks; however, additional assumptions are listed below PROJECT PERSONNEL The CONSULTANT proposes Tyler Davis, P.E as the Project Manager and Bryant Facey, P.E as the RPR (Resumes attached). Mr. Davis has designed many pipeline projects and served as the engineer's Project Manager's representative and /or RPR on numerous occasions. Mr. Facey has served as a RPR in previous employment. Should changes in personnel be necessary, the CONSULTANT shall receive approval of the replacement by the CITY EXCEPTIONS TO SCOPE OF WORK 1. Preliminary as -built drawings will be submitted monthly for work completed and final as- built drawings will be submitted prior to substantial completion The as -built drawings will be certified as "current" prior to any approval of pay applications 2. As -built drawings will be marked for revision each month before approval of Contractor's Application for Payment (AFP). 3. Revisions will be addressed before approval of next AFP. 4. Consultant shall provide to the CITY copies of the RPR's daily inspection reports and digital photographs. Daily inspection reports and photographs shall be sent to the CITY electronically (FTP Site, Disc or Thumb - drive) on a weekly basis. 5. CONSULTANT shall provide updates and photographs for CITY's social media site(s). CITY will be responsible for site updates and maintenance. ASSUMPTIONS 1 If requested, CITY will provide temporary work space for CONSULTANT's Project Manager and RPR to work All meetings will be conducted on job site or at CITY's administrative offices 2. CITY or Engineer of Record shall provide bidding design documents, conformed designed documents, and all permits Permit preparation services are not included in Task Order CONSULTANT will verify monitor permit conditions and obtain permit close out 3 Certification of construction completion by CONSULTANT does not require CONSULTANT to prepare or sign and seal record drawings 4 Project schedule for construction is anticipated to be 12 months with 6 months of field construction. Work period beyond this time schedule shall be consider additional work for CONSULTANT to be negotiated with CITY 5. CONSULTANT is not responsible for job site safety. In the event that unsafe conditions are observed, CONSULTANT will advise CONTACTOR of observations and alert the CITY to the conditions observed 6 CONSULTANT is not responsible for preparing or approving Maintenance of Traffic Plans (MOTs) prepared by CONTRACTOR. Confirmation of MOT submittal and approval to the appropriate agencies (Florida Department of Transportation — District IV, Palm Beach County Roads and City of Boynton Beach Public Works) shall be conducted by CONSULTANT. The construction cost estimate for the project is approximately $4,200,000 and the construction duration is anticipated to be approximately twelve (12) months The Engineer of Record is Brown and Caldwell The selected consulting firm (Consultant) will be responsible for construction management services for the project Specific tasks will include Task 1 — Services During Construction. The Consultant shall provide general project administration for the duration of the project 1 Conduct the Pre - Construction Meeting and prepare detailed meeting minutes 2 Receive, log and review shop drawings, procedures, and product submittals for general conformance with the design intent and provisions of the Contract Documents. It is anticipated that there will be a maximum of two submittals required per bid item 3 Establish and implement procedures for expediting the review of shop drawings, product data, samples, and other submittals with the Engineer of Record (EOR). 1 Scope A Raw Water Main — Section C Proposal Request — Construction Management Services 4 Monitor and ensure that the contractor(s) obtain all required governmental and regulatory permits, inspections, and approvals 5 Prepare agendas for monthly progress meetings and distribute prior to the meeting Provide written meeting minutes The monthly progress meetings will be conducted by the Consultant Meetings will be held at the City or at the construction trailer located at the project site as applicable. It is anticipated that this task will take four hours per month. 6 The Consultant shall be proactive and endeavor to obtain satisfactory performance from the contractor(s). The Consultant shall recommend courses of action to the City, if requirements of a contractor are not being met 7 Advise the City if It appears that the construction cost may exceed the approved budget and make recommendations for corrective action, 8. Conduct weekly construction meetings with the Contractor and the City on specific construction activities that are not in compliance with the approved contract, specifications or plans as needed If needed, this task is anticipated to take two hours per week This task will be a separate line item on the proposal and invoice to be used as needed. 9 Review monthly Applications for Payment (AFP) submitted in a format acceptable to the City. Consultant shall verify quantities as represented on the AFP and make recommendation to the City to proceed with the payment as requested, or as modified based on the Consultant's review 10 Review construction schedules, progress, photographs, and progress record drawings submitted with each AFP and report to the City, conditions which may cause delay in completion. 1 1 Respond in writing to the contractor's Request for Information (RFI) regarding the Contract Documents Preparation of response may require field time by the Consultant Prior to making their formal response, Consultant shall consult with the City. If required, Consultant will obtain answers to RFI's from the Engineer of Record (EOR) for those items that need design clarification or design resolution w 12 Receive and negotiate Change Order Requests (COR's) and provide recommendation to the City Preparation of response may require field time by the City's Project Engineer. 13 Assist the City in review, evaluation, and documentation of claims by the contractor(s) 14 Consultant shall certify construction completion to the various permitting agencies as applicable (South Florida Water Management District, Lake Worth Drainage District, Palm Beach County Health Department, Florida Department of Transportation, etc) based on visible project features. Transmit all required project closeout documentation to the City. Task 2 — Resident Protect Representative (RPR) The Consultant is to provide a Resident Project Representative (RPR) during the construction of the work for the duration of the project The Consultant shall provide the RPR for a five (5) day work week while actual construction activities are taking place It is anticipated that actual field construction will occur for six (6) months The RPR shall 1 Serve as the Consultant's liaison with the contractor(s) and City staff. 2. Conduct on -site observations of the work in progress to assist the City in determining if the work is proceeding in accordance with the Contract Documents and that completed work conforms to the Contract Documents, endeavoring to guard the City against defects and deficiencies in the work Report in writing to the City whenever Consultant believes that the work is unsatisfactory, faulty, defective, or does not conform to the Contract Documents, or does not meet the requirements of inspections, tests or approval required to be made, or has been damaged prior to final payment 3 Accompany visiting inspectors representing public and regulatory agencies having jurisdiction over the project Record in writing, the outcome of these inspections and report same to the City. 4 Consider and evaluate the Contractor's suggestions for modifications during construction and report them to the City, in writing Consultant shall make recommendations to the City for action 5 Witness all testing and /or procedures as required by the Contract Documents 6 Observe all pig insertion, flushing, disinfection, and pressure testing 7 Review as -built (Record Drawings) information from the contractor(s) to confirm conformance to the Contract Documents.
