A. Background
The'City of Boynton Beach owns and operates Injection Well IW-1 to di ~pose Of membrane
softerr fluid from the City of Boynton Beach West Water Treatment Plant (WTP).
XiteL~ ell was com, tructed in I992 and placed into operation ~n 1993. The injection
well is , and,pa~ker design, as ~required by Rule 62-528, Florida A~ative Code
(FAC), ,' diameter steel casing to a depth of 2,780 f~et below pad level
(bpl) anda ~3-$/8-i.~c!x outside diameter steel tubing to a depth of 2,720 feetbpl. The well is
completed v~ith open-hole construction to a depth of 3,312 feet bpl.

B. Scope of Services
Task 1 - Operating Permit Services
The following services will be provided by CH2M HILL:
· Prepare and submit to FDEP an operating permit application with s
information for the deep injection well f~cility, as required by FDEF
· Respond to up to two FDEP request for information (Ri=I) regardinl
applications.
· Attend up to two meeting with the City and FDEP to discuss
concerns/comments/issues the FDEP may have regarding the opel
application.
· Review draft permit and provide comments to the City and FDEP.
· Attend FDEP draft permit public notice meeting.
· Review Notice of Intent to Issue an operating permit and provide ~
and FDEP for the permit.

C. Assumptions
The following assumptions were made for the preparation of this Task Order:
· The City will provide a survey of the injection and monitor wells and have the surveyor
provide proof that the survey has been recorded at the County Courthouse if required
by the FDEP.
· The scope of services provided in this Task Order does not include the preparation of
addenda and updates to existing reports such as the injection well system engineering
report, mechanical integrity testing report, or operation and main nance manual.
· The C£i'Y will be responsible for all permitting fees.
D. Obligations of the CITY
· The CITY shall provide to CH2M HILL, as required for the performa of services
specified herein, record data and information related to the injection ~ tell system.
E. Compensation
Compensation by the CITY to CH2M HILL will be on a cost-reimbursabl,: basis in accordance
with Article 2.B of the Agreement. The estimated compensation for the s~rvices described in
this Task Order is $26,086 as described in the table below:

G. Schedule
The permit application will be submitted to FDEP within 45 days of the e.x~nZ~;n ;tion of this
Agreement. Responses to request for information will be prepared within.-.--§0 days of receipt of
the request for information.
