The City renewed the construction and testing permit for Aquifer Storage and Recovery (ASR) Well-2
in 2012, with a 5 year duration (FDEP File # 0196693-004-UC). However, there is no “timely
submittal” when applying for an operation permit. The City also operates a second Class V, Group 7,
ASR well at the East Water Treatment Plant, ASR-1, which has been operating under a letter of
authorization since 1992. The renewed construction and testing permit for ASR-1 presents the
construction and operating characteristics for both ASR wells, and in consideration of the wells’
shared storage zone, one well is acknowledged as the defacto monitoring point for the other.
Furthermore, the FDEP has previously recommended that ASR-1 operate under an operation permit,
consistent with the current regulatory framework. Therefore, in the interest of obtaining an
operation permit for both ASR-2 and for ASR-1, with enough time to address any regulatory
concerns, the City wishes to begin those discussions with FDEP now.
Additionally, the City’s Class I Injection Well Operation Permit (FDEP File # 0178213-003-UO) expires
February 13, 2016. The City must submit a permit renewal application no later than 60 days prior to
the expiration date of the current permit. The City therefore wishes to prepare a permit renewal
application for submittal to FDEP no later than November 2015. Provided the application is
submitted timely, in accordance with Rule 62-528, F.A.C., the injection well system will remain in
compliance.
CH2M HILL Engineers, Inc. (CH2M) has successfully worked with Utility staff to provide professional
services on an as-needed basis to support the City’s permitting and testing requirements. CH2M
proposes to provide these services on an as-needed basis to provide technical support for the
described permitting efforts. CH2M’s scope of work under this Task Order will consist of two
separate tasks:
1. Pre-application services for the ASR system in preparation for operational permitting in
2016; and
2. Technical services supporting the Injection Well Operation Permit renewal.

The scope of services included in this authorization for each of the two tasks identified above are as
follows:
1.ASR Pre-Application Services
a.Data and Operations Review
i.Review of operational flows and water quality from both ASR-1 and ASR-2,
including informal correspondence with Utilities Management and staff.
ii.A brief Technical Memorandum (TM) summarizing findings from the data
review. This TM is intended as an internal reference for the City in
preparation for both pre-application regulatory discussion and application
requirements.
b.Meetings:
i.One meeting at the plant to review operational procedures and to review
operational and monitoring data with Utility staff;
ii.One follow-up meeting with Utilities management to discuss permitting
strategy;
iii.One meeting with FDEP in Tallahassee to discuss permitting approach for
both ASR-1 and ASR-2.
iv.One follow-up meeting with the SFWMD and/or FDEP in preparation for
permitting in 2016.
The intent of the data review and meetings is to prepare for the 2016 operational permitting
process in alignment with FDEP. The permit should allow the City to continuing to maintain
operations at both wells in accordance with the City’s Water Use Permit, balance water demand,
preserve public health, and mitigate potentially conflicting monitoring or operational conditions.
The City may be requested by FDEP to conduct additional cycle testing or monitoring in 2016 in
preparation for permitting. CH2M can assist with additional testing not included in this scope if
authorized by the City.
2.Injection Well Operation Permit Renewal
CH2M will prepare the application and supporting material, which will consist of an application
document that includes the following supporting material, as required by FDEP:
1. An updated map showing the location of the injection wells or well field area for which a
permit is sought and the applicable area of review. Within the area of review, the map must
show the number or name, and location of all producing wells, injection wells, abandoned
wells, dry holes, surface bodies of water, springs, public water systems, mines (surface and
subsurface), quarries, water wells and other pertinent surface features including residences
and roads. The map should also show faults, if known or suspected. Only information of
pubic record and pertinent information known to the applicant is required to be included on
this map.

2. A tabulation of data on all wells within the area of review which penetrate into the
injection zone, confining zone, or monitoring zone. Such data shall include a description of
each well’s type, construction, date drilled, location, depth, record of plugging and/or
completion, and any additional information the Department may require.
3. Maps and cross sections indicating the general vertical and lateral limits within the area of
review of all underground sources of drinking water, their position relative to the injection
formation and the direction of water movement, where known, in each underground source
of drinking water which may be affected by the injection.
4. Maps and cross sections detailing the hydrology and geologic structures of the local area.
5. Generalized maps and cross sections illustrating the regional geologic setting.
6. Contingency plans to cope with all shut-ins or well failures, so as to protect the quality of
the waters of the State as defined in Rule 62-3 and 62-520, F.A.C., including alternate or
emergency discharge provisions.
7. For wells within the area of review which penetrate the injection zone but are not properly
completed or plugged, the corrective action proposed to be taken under Rule 62-528.300(5),
F.A.C.
8. A certification that the applicant has ensured, through a performance bond or other
appropriate means, the resources necessary to close, plug or abandon the well as required
by Rule 62-528.435(9), F.A.C.
The injection well application package will include documents summarizing:
All available logging and testing program data and construction data on the well or well
field;
A satisfactory demonstration of mechanical integrity for all wells pursuant to Rule 62-
528.300(6), F.A.C.;
The actual operating data, including injection pressures versus pumping rates where
feasible, or the anticipated maximum pressure and flow rate at which the permittee will
operate, if approved by the Department;
The actual injection procedure;
The compatibility of injected waste with fluids in the injection zone and minerals in both the
injection zone and the confining zone;
The status of corrective action on defective wells in the area of review;
Record drawings, based upon inspections by the engineer or persons under his direct
supervision, with all deviations noted;
Certification of completion submitted by the engineer of record;
An updated operation manual including emergency procedures;
Proposed revisions to the monitoring program or data to be submitted; and,
Proposed plugging and abandonment plan pursuant to Rule 62-528.435(2), F.A.C.

This estimated cost is based on the following assumptions:
A Florida licensed professional engineer from the City Utilities Department will sign and seal
the permit application for the Class I injection well.
The operation permit renewal application for the Class I injection well will include some
components by reference, such as the certificate of completion, Completion Report and
data therein, since the City has renewed this permit previously on multiple occasions.
The operation permit renewal application for the Class I injection well does not include an
updated O&M manual. It is assumed that the City has maintained and will continue to
comply with the permit requirements for maintaining a current O&M manual at the West
Plant.
The City will provide a recent (less than 1 yr old) injectate water quality analysis meeting the
current FDEP list of parameters for re-permitting. Any additional analysis will be the
responsibility of the City.
The FDEP permit renewal application fee of $10,000 will be paid by the City.
Additional services not included in this proposal will require an amendment to this Scope of
Services.
The City will provide the electronic (Excel format) operational and monitoring data to CH2M
for both the ASR well and the Class I injection well.
This scope of work does not include the ASR well system operation permit application. ASR
permit application services will require a separate authorization, the scope for which will be
more clearly defined by the preliminary activities included in this Task Order.
The operation permit renewal application for the Class I injection well will be submitted
electronically, consistent with current FDEP guidance.
This Task Order includes one formal or informal response to a Request for Additional
Information (RAI) from FDEP for the Class I injection well permit application.

titled Hydrogeological Consulting Services, dated August 19, 2013, between the CITY of Boynton
Beach and CH2M Hill Engineers, Inc.

Compensation by the CITY to CH2M Hill Engineers, Inc. will be on a time and materials basis in
accordance with the above-mentioned Agreement. The estimated compensation for the services
described in this Task Order is $29,459.36 as shown in Table 1.

ASR Services: A review of ASR operations and data will be scheduled within one month of receiving
the data from the City. The meeting with FDEP will be scheduled at a time convenient for the City in
the 2015 calendar year.
The Class I injection well permit application must be submitted by the City no later than December
15, 2015. CH2M will provide supporting geologic/hydrogeologic documentation to the City no later
than November 30, 2015.
