Fee Proposal
➢ The monthly flat fee that we propose is $2, 100

for Fire and $2,300 for Police, or $4,200 -
$4,400 combined , plus a one time charge of
$1 ,500 - $2,000 for data input.
➢ Benefits USA, Inc. will be responsible for
telephone expenses, routine copies and postage
(with the exception of mass mailings), and basic
computer programming. Benefits USA, Inc. will
not be responsible for the cost of printing,
postage for mass mailings and non-routine
major expenses for special tasks requested by
the Trustees.

Termination
➢ We agree to a thirty-day notice of termination by the Board. We
further agree to provide ninety days written notice to the Board in
the event of termination by Benefits USA, Inc. In the event of
termination, participant records will be turned over to the Trustees or
a new administrator in electronic and/or paper format.
Public Entity Crimes
➢ Neither the entity submitting this proposal, nor any officers,
directors, executives, shareholders, employees, members or agents
who are active in management of the entity, nor any affiliate of the
entity, have at any time been charged with and/or convicted of a
public entity crime.

reason. No claims have been filed that have been settled or that are

Although Benefits USA has only been in existence as a firm since 1997, our
principals have extensive backgrounds in the employee benefits field.
Benefits USA President, Peter Prior's experience as a public pension trustee
dates back to 1977. Althea Leigh-Lodge has been a benefits administrator
since 1996. Each of these individuals has continued his or her education in
the benefits and insurance fields and will be responsible for your pension
fund.
In addition to these two principals, other staff members include Livia Nixon,
Mei Fang, Leticia Perez, Liz Andrews, Laurel Patterson and Michael Lurch.
Ms. Nixon has been with our firm since 2005 and has worked in the
municipal pension field since 1990. Ms. Fang and Mr. Lurch have been with
Benefits USA providing technical and administrative support. Ms. Perez has
worked in various industries including the insurance industry and was
rehired after working with us for two years. Ms. Andrews has over 24 years
of municipal government experience, which includes 17 years specializing in

Errors & Omissions insurance with a limit of $1 ,000,000
and a Fidelity Bond in the amount of $250,000. In
addition, we carry General Liability with a limit of
$1 ,000,000.

The request asks us to provide the name of the contact person that will
be assigned to the Fund. The principal representative will be Pete
Prior.
As for internal control procedures for the processing of disbursements:
this is done through a warrant system. We will furnish the
appropriate trustee/s with a warrant containing the accounts and
amounts to be paid above their signatures. Any invoices paid
between meetings will be listed on the consent agenda at the next
meeting for ratification.
We recommend that the custodian process the pension checks for the
retirees and beneficiaries. As a Trust Company, they are fully
equipped to process the checks and provide alternative methods of
distributing pension checks during a time of crisis.
After reviewing your service providers, we do not have any relationship
with your vendors other than strictly a working relationship with other
clients.

re-constructed to the beginning of the fiscal year in order to be able to
provide the auditor the records they will need. Obtain past monthly reports
from custodian.
