Dear Mr. Mandigo:
Please be advised: that on Tuesday, July 17, 1990, the City
Commission approved "the referenced site plan modification,
SUbject to staff comments, copies of which are attached.
These plans were approved subj ect to your compliance with the
attached stipulations and" must now be modified to incorporate
same. To initiate your sign-off procedure, please submit two (2)
sets of modified plans to the Office of the Si,te Development
Division of the Building Department, which will record the
required identification markings on each set of the modified
plans.
After each set of modified plans has the appropriate markings,
you should proceed with your sign-off on each set of plans from
each Technical Review Board member who made comments. The Zoning
& Site Development Administrator will provide the Building
Department sign-off and is the last Technical Review Board member
to review and sign-off the plans. If the Building Department did
not have comments that require sign-off, they are still the last
department to handle and process the plans. One (1) set of final
signed-off plans will remain with the Building Department and the
other set of plans will be returned to you to be retained at the
job site.
To help facilitate the sign-off process, you should make an
appointment to meet with each Technical Review Board member (only

to review and sign-off the modified plans. After securing all
the required Technical Review Board member signatures and
completing processing by the site Development Division, you may
apply for building permits by submitting the appropriate
documents and fee to the Plan Review and Permitting Division of
the Building Department. The Building Department will advise you
as to any additional permits which may be required, such as Palm
Beach Health Department (water and sewer), clearing/grubbing,
excavation/fill, drainage, paving, site lighting, landscaping and
irrigation.

improvements shown on the site plan. The site plan will . be
viewed procedurally as an as-built or record' drawing. If you

Upon review of the above mentioned project, the following list of
comments must be addressed in order to conform with Boynton Beach
City Codes:
1. Show the building setback dimensions from the property line
to the face of the building from two (2) adjacent property
lines.
2. Provide a detailed plan view and full section view drawing
of the six (6) foot high concrete block and stucco fence
shown on the plans. Identify the type, size and color of
material used for the block fence. Identify the size and
type of all required vertical and horizontal structural
material and components for the fence and its foundation.
3. The landscape plan must reflect the appropriate percentage
of native species landscape material.
4. Provide a detailed section view drawing of the site stair
and handrail. Specify the stair riser, tread and handrail
location, size and shape.
5. Show on the plans a handicapped accessibility "walkway"
(minimum width 5') leading to the building entrance from
the boundary limit of the site.
6.
Show on the plans handicapped accessibility "walkway"
(minimum width 5') leading to the entrance of the building
from the handicapped parking space. Provide detailed
drawings of the required curb cuts and/or curb ramps. Show
spot elevations along the handicapped "walkway" and provide
a detailed drawing of the ramp. The ramp and "walkway"
material and construction must comply with the State of
Florida Accessibility Requirements Manual.

7. Show on the plans typical regular and handicapped parking
space pavement markings (striping must comply with the
applicable codes).
In order to facilitate the permitting process, the following
information should be included with your documents submitted to
the Building Department for review and permitting:
1. State on the plans that the building and site will comply
with Palm Beach County Health Department regulations for
child care facilities.
2. All signage requires a separate permit and must comply with
the requirements of the Sign Code.

1. Reduced pressure backflow preventers will be required on the

sign to aid staff in determining whether the sign is a

7. Pursuant to Ordinance 84-11 and the master plan modification

8. A sidewalk system, linking the child care center with the
hospital, is recommended since the child care center is

Upon review of the above mentioned project, the following list of
comments have been provided as a courtesy with the applicants
full knowledge that there will be a full review made when the
project is placed on a future Technical Review Board agenda:
1. All drawings and/or documents submitted for public record
and prepared by a design professional shall show original
raised seal (legible) and signature of a Florida registered
design professional responsible for the drawings.
2. Provide a copy of the South Florida Water Management
District permit for modification of the existing permit for
the proj~ct.
3. Show the building setback dimensions from the property line
to the face of the building overhang" from two (2) adjacent
property lines.
4. Identify the height and type of all fences shown on the
plans.
5. Show and dimension on the site plan the location of the
dumpster enclosure. Provide a detailed plan view and full
section view drawing of the dumpster enclosure and pad.
Identify type, size and color of material used for the
sides of the enclosure. Identify the overall height, width
and length. Identify the size and type of all required
vertical and horizontal structural material and components
for the dumpster enclosure and pad.

Show on the site plan drawing the appropriate number of

parking space computations for all the required parking
spaces as specified by the zoning code parking requirements
(ratio 1 parking space for each 300 square feet of gross

made of hedge landscape material to screen on-site vehicles

· Dear Mr. Yaun:
'In follow up to our phone conversation of 5/22/90, I am submitting this request for
a modification to the above referenced general permit, As we discussed, no calculations
are being submitted as this is a reduction in impervious area for an existing general
permit, with no reduction in the stormwater retention system,
The area this phase occupies was originally permitted as 0,76 acres of impervious
area in the form of a parking lot, The proposed modification calls for 0.11 acres of
paving and 0.19 acres of building for use as a day care center. The difference of 0.46
acres will be in landscaping and a play yard,
As this portion of the site is on the crest of a hill, none of the stormwater
retention facilities in the original permit are in this area, Therefore, there is no
reduction of the retention capacity of the originally permitted system. In addition, due
to changes in city requirements 0.023 acre feet of storage is being provided in a
depressed area, containing the first 2.5 inches of runoff from the parking lot.
In support of this request, I am enclosing one copy of RC-l A, six (6) copies of an
aerial photograph of the site, six (6) signed and sealed copies of the construction plans
and a permit application fee of $475,00, I trust that this will be sufficient to allow
processing; however, should you have any Questions or require any"additional information,
please let me know.

regard to the latest project additions and parking changes. The following
information is the present parking data:
l. Hospital beds = 362 x 2.5 spaces/bed = 905 parking spaces

3. Excess hospital parking spaces = 20
4. Bethesda Child Care Center parking requirements;

The plan filing fee is made payable to the City of Boynton Beach in the
amount of *** .3% *** of the valuation of the work specified on the permit
application. The total permit fee is determined by the Building Department,
and is due when the permit is issued.

For information regarding permits related to the site work contact the Site
Development Division (407 738-7484) and (407 738-7480) for permits related
to the construction of the building.

Building Department issuing the certificate of Occupancy, I would
like to ensure that you do not have any outstanding or pending
issues concerning this site that must be rectified (excluding
issues that are covered entirely with a city approved surety).
If the issues are related to permits issued by the Building
Department, please provide this department with the permit number and
nature of unresolved issues. If the unresolved issues are not
permi t related, notify the owner or the owners agent to ensure
that the issues are rectified.
Please respond to this office within ten (10) days so that we may
proceed to issue the certificate of occupancy. Thank you for your
cooperation and timely response.
