Please be advised that on Tuesday, February 19, 1991, the
City Commission approved the above referenced site plan, subject
to staff comments, copies of which are attached. Additionally,
the' Community Appearance Board is requiring that the entrance
door color be Sierra Gray #12211 and that Chalkas be planted
around the dumpster enclosure.
These plans were approved subj ect to your compliance with the
attached stipulations and must now be modified .to incorporate
same. To initiate your sign-off procedure, please submit two (2)
sets of modified plans to the Office of the site Development
Division of the Building Department, which will record the
required identification markings, on each set of the modified
. plans.
After both sets of modified plans ;have the appropriate markings,
you should proceed with your sign-off on both sets of plans from
each Technical Review Board member.. The sign-off procedure
requires that the Engineering Department sign-off first, followed
by all other departments with the exception that the Planning
Department sign off next-to-last' and the Building Department
sign-off last. The Zoning & Site Development Administrator will
provide the Building Department sign-off. One (1) set of final
signed-off plans will remain with the Building Department and the
other set of plans will be returned to you to be retained at the

To help facilitate the sign-off process, appointments should be
made to meet with each Technical Review Board member to allow
them sufficient time to review and sign-off the modified plans.
After securing all the required Technical Review Board member
signatures and completing processing by the Site Development
Division, you may apply for building permits by submitting the
appropriate documents and fee to the Plan Review and Permitting
Division of the Building Department. The Building Department
will advise you as to any additional permits which may be
required, such as Palm Beach Health Department (water and sewer),
clearing/grubbing, excavation/fill, drainage, paving, site
lighting, landscaping and irrigation.

Upon review of the above mentioned project, the following list of
comments must be addressed in order to conform with Boynton Beach
City Codes:
1. Show the location, type and size of all site signage.
2. Amend the handicapped parking space detail drawing to
include the following information:
i. blue pavement striping
ii. specify on the plan that the handicapped parking sign
is mounted seven feet from grade to the bottom of the
sign

platform that is required at the entrance of the building.

Specify on the plans that the landscape material planted

architectural and engineering plans.
6. The outline, configuration and material shown and identified
on the project drawing(s) submitted with the planes) for
final sign-off shall match in every respect the colored
drawing(s) approved by the City commission during the
approval process of the project. The color of each exterior
finish material shown on the final sign-off plans shall be
identified by name, color number and manufacturers name
which shall match in every respect with the color(s) shown
on the drawing(s) approved by the City Commission. A
manufacturer's color chart showing and identifying all
approved color(s) shall be submitted with the plans for
final sign-off.

HAYES-MACK INSURANCE BUILDING
In accordance with the City of Boynton Beach Code of Ordinances, Chapter 19,
Section 19-17, Plan Required, including Chapter 5, Article X, Boynton Beach
Parking Lot Regulations, the applicant for the above referenced project shall
submit the following information, technical data, details and plan revisions.
1. Provide a photometric plan depicting illumination levels within the parking
facility. The photometric plan shall include all pavement areas including
pedestrian walkway illumination levels. Section 5-l42(a) and Section
5-l42(b).

2. Provide a plan depicting the location of standard D.O.T. approved stop signs
and associated details including a double line consisting of two normal
solid yellow lines which will delineate the separation between travel paths
in opposite directions where overtaking and passing is prohibited in both
directions, specifically at stop bar locations. Section 5-l42(c) Traffic
'Control and Section 5-l42(g) Traffic Control Standards which reference the
Manual on Uniform Traffic Control, specifically Section 3A-7, Types of
Longitudinal Lines, Subsection 3A-7,6.

3. Provide a landscape plan which compliments the site plan. The site plan
indicate 900 parking, landscape plan indicates angle parking. The site
plan indicates a handicap stall situated at the extreme southeast corner of
the property, landscaping plan indicates it center of east parking area. The
landscape plan indicates a different geometry to the northwest ingress/egress
approach that does not mirror the geometry of the site plan. Please revise
all plan conflicts. Section 5-l42(g) Construction Standards.
4. Provide a detail for curbing and car stops which comply with the requirements
set forth in Section 5-l42(g) Curbing Standards and associated appendix.
5. The general notes and specifications indicates the subgrade soils to be
stabilized to a Florida Bearing Value of 50. Should the applicants Architect
wish to diminish the cost of this project, the Florida Bearing Value of 50
is not required by Article X, Section 5-142 inclusive and may be deleted should
the Architect or Design Engineer deem that action reasonable.
6. Provide construction details for all ingress/egress approaches and obtain Palm
Beach County approval for construction of ingress/egress approaches within the
County rights-of-way which shall be a prerequisite procedent to the issuance
of permits for the construction of the subject parking facility.
7. The southern ingress/egress approach is closer than 180 feet to the intersecting
L~gl1L-o[-way lines of Guli Stream Boulevard and Seacrest Boulevard, thereby
constituting the necessity for variance. Section 5-l42(h) Driveways.
8. Provide a handicap stall detail which complies with the lastest requirements
of the Department on Community Affairs and the lastest edition of the
Accessibility Requirements Manual. Section 5-l42(k) Handicap Requirements.
9. The site plan and engineering plans are to show Palm Beach County traffic
signal pole, traffic controller box and Palm Beach County easement for traffic
signal. Storm water retention pond is not to conflict with the traffic signal
easement. Survey submittal requirement of Chapter 19 including Section 5-l42(g)
Contruction Standards.

Ms. Heyden indicated this variance related to a driveway
onto Seacrest Blvd. for a future office building_ This was
located within 180' from the 'intersecting rights-of-way
lines of Seacrest and Gulfstream. The driveway ,was pro-
posed as far from the intersection as possiblei'namel~; ,a
distance of 93'. The TRB recommended approval'/ 'subject to
the driveway being limited to'right turns on1y.''''The''Po1ice
Dept. had observed some problems and felt left'''turn: movement
would worsen an already bad situation. Several boa~d'~em-
bers were familiar with the situation at this"s"ite~ t'Mr.
Richter felt the only way the owner could utilize his pro-
perty would be by obtaining this .variance. ,'He:agreed I wi th
staff and the Police Dept.
Chai rman Rosenstock commented on the Communi ty Des ign:' Plan
and asked if staff was satisfied with the way this was going
to look. Ms. Heyden believed so and indicated if"they
weren't, there would be another opportunity for~eview at
the site plan level.

George Davis, Architect was present to represent the appli-
cant. He explained the reason for the variance1was'because
of the dumpster location. He commented on difficulty with
the property accommodating movement of a big garbage'truck.
He referred to the Ordinance requiring no driveway 'within
180' of the corner and the fact that the property is'only
150' long. Therefore, there could be no driveway to accom-
modate the trucks without the variance. Mr. Davi~'indicated
the owner accepted the staff comment about the right turn
only sign. .' .
Chairman Rosenstock asked if there was any input from the
public. As there was no response from the audience,"THE
PUBLIC HEARING WAS CLOSED.
Vice Chairman Lehnertz moved to recommend approva1"ofthe
Hayes-Mack Insurance Office Building variance to Section
S-142(h)(3) Driveways of Article X, Parking Lots, subject to
all staff comments. Mr. Howard seconded the motion:which
carried 7-0.

Please be advised of the following Planning Department comments
with respect to the above-referenced request for site plan
approval. You may ~lant to address the comments below, as well as
the attached comments from other departments, on your next ,
submittal of revised drawings. please ~ote that the number of
drawings required ~or a submittal has been increased. Seven
complete sets are required, plus nine sets comprised of the site
plan, landscape plan, elevations and typical floor plan only are
also required. The next available deadline is January 18,1991
at 5: 00 P. M. :
1. Add a graphic scale to all drawings. Site plan application,
page 5, item 2.
2. Parking for this inaurance office is required at the rate of
one space for every 300 square feet of 'gross floor area.
Revise site plan data table and application accordingly.
Please note that providing enough parking to meet the 1:200
ratio would enable this building to be occupied in the
future by other uses permitted in the C-1 zoning district
which ~re more parking intensive, such as medical and dental,
offices, which require parking at the rate of 1:200.
3. Label the land use and zoning of the surrounding properties

is superimposed on the site plan-which shows catch basins
located within grassed areas, and one on a separate sheet
which shows a catch basin located with the pavement. The.
latter design does not meet the requirements of the
comprehensive Plan, Drainage Subelement, Policies 3B.2.3
since catch basins are not permitted within paved areas.

HAYES-MACK INSURANCE BUILDING
In accordance with the City of Boynton Beach, Code of Ordinances, specifically
Chapter 19-17, "Plan Required" (Site Plan Approval Process), including Chapter 5,
Article X, Boynton Beach Parking Lot Regulations, Section 5-142 inclusive, the
applicant for the above referenced project shall submit the following technical
information, data and plan revisions;
1. The pedestrian concrete sidewalks shall continue through the ingress/egress
approaches and be transitioned to facilitate the movement of handicapped
individuals. The required thickness through these areas is 6 inches in
accordance with Section 5-l42(g) , Sidewalk Standards.
2. Provide stormwater drainage calculations in accordance with Section 5-l42(f) ,
"Drainage". Drainage calculations shall include percolation test data.
3. Provide a photo-metric plan depicting actual light levels at pavement surface.
Section 5-l42(a) "Lighting" and Section 5-l42(g) "Lighting Standards".
4. Landscaping located within the island situated center of north parking
facility appears to conflict with lighting pole standards. The trees shall
be located further north in the landscaped island so as not to obscure the
luminaire, thereby negating the dispersion characteristics of the fixture.
Section 5-l42(g) "Construction Standards" and Section 19-17(k) "Technical
Information and Data".
5. The applicant shall fully develop the swales within the public rights-of-way
for Seacrest Boulevard and Gulfstream Boulevard in accordance with City and
County standards. The grassed swales shall be fully sodded and irrigated.
6. Provide on-site handicap striping and handicap ramps in accordance with the
Department of Community Affairs latest edition of the accessibility
requirements. Section 5-l42(k) "Handicap Requirements".

7. Clarify plan note relative to cross-section of construction materials and
specify type of base material, including type of asphaltic concrete surface.
Section 5-l42(g) "Parking Lot Construction Standards".

8. The drainage plan submitted by GBC Engineers does not conform to the drainage
plan submitted by Architect; George Davis. The drainage plan submitted by
Gerald B. Church, P.E. provides for inlet structures situated within paved
areas. Inlet structures situated within paved areas is prohibited by our Code
of Ordinances. Revise plans accordingly.
9. Grading elevations, relative to the Gerald B. Church plans, do not conform to
the grading elevations of the GeorgeC. Davis plans. The George C. Davis
plan and the Gerald B. Church plan shall be revised to provide for proper
drainage gradients. Section 5-l42(g) "Construction Standards".
10. The George C. Davis plan indicates approximately 220 l.f. of drain field.
They shall provide a connection from the northwest stormwater drainage structure
to the southwest swale area. Additionally, the George C. Davis plan shall
incorporate a construction detail depicting a proper section for said drain
field. Section 5-l42(g) "Drainage Standards".
11. Provide a typical section through the asphalt areas and adjoining swales
areas. Section 5-l42(g) "Required Dimensions and Elevations including
Construction Details".

Upon review of the above mentioned project, the following list of
comments must be addressed in order to conform with Boynton Beach
City Codes:
1. Specify on the plans the height of the building.
2. Show on the plans a setback distance of thirty feet from the
south property line to the face of the building.
3. Revise the regular and handicapped parking space detail
drawing to include the following information:
i. width and color of pavement striping
ii. dimension the overall width of each type of parking
space pavement markings
iii. dimension the width of the handicapped parking space
including the required abutting aisle
iv. show the required striping for the handicapped parking
space
v. specify on the plans that the handicapped parking sign
is mounted seven feet from grade to the bottom of the
sign

The outline, configuration and material shown and identified
on the drawing(s) of the project submitted with the planes)
for final sign-off shall match in every respect the colored
drawing(s) and conditions approved by the City Commission
during the approval process of the project. The color of
each exterior finish material shown on the final sign-off
plans shall be identified by name, color number and
manufacturers name which shall match in every respect with the
color(s) shown on the drawing(s) approved by the City
Commission. A manufacturer's color chart showing and
identifying all approved color(s) shall be submitted with
the plans for final sign-off.
5. Specify on the plans the color of the dumpster enclosure.

equipped with shields to eliminate glare on adjacent
residential properties.

in representing the finish elevations of the parking area.
10. Specify on the plans the slope of the handicapped accessible
path that leads to the entrance of the building from the
handicapped parking space. (slope greater than 1 to 20 will

11. Specify on the plans the location, width, length and
elevation of the level platform that is required at the
entrance to the building.
12. Show and specify on the plan the location, width, slope,
length and type of material proposed for the required
handicapped accessible "walkway" that leads to the entrance
of the building from the public sidewalk.
13. Show on the plans the required landscape line of sight
triangles at all ingress/egress driveways and the street
intersection. (specify that the landscape material will be
maintained to allow an unobstructive line of sight between
30" to 72" within the triangles)
14. specify on the plans that the .required perimeter shrub
landscape material will be planted two feet (2') on center.
15. The landscape plan should show a plant directory, indicating
the total number, species, spacing and height of all
landscape material. All specifications must comply with the
Landscape Code. Include the percent of native species. (50%
is the minimum native species percent)
16. State on the plans that the landscape material will be
irrigated with an automatic water supply system. Specify the
water supply source.
17. Specify on the plans that the quality of the landscape
material will be Florida #1 or better.
18. Landscaping proposed to be installed within an easement
shall be allowed only after consent of all utility companies
that have access right to use the easement.
19. Specify the type of mulch material to be used and it's
thickness when in place.
20. Shrubs planted to screen rights-of-way need to be 3 feet
high at planting.
21. Specify type of sod to be used.

landscaping shall meet the requirements of the required

This letter is to inform you that your project has received 'final sign-off
on June 13, 1991 of the plans submitted for review by the Technical Review
Board. Completing the site Plan Review.process enables you to submit
working drawings and construction plans for the site work and building
construction to the BUilding Department for plan review and permitting. To
initiate this process present to the plans analyst in the Building
Department the following:

a) appropriate and completed permit application form
b) copy of this letter
c) appropriate plans and associat~d documents
d) a plan filing *permit fee
The plan filing fee is 30% of the estimated permit fee and is made payable
to the City of Boynton Beach and due at time of filing an application for
each permit. The total permit fee is determined by the Building Department,
and is due ~hen the permit is issued.

For information regarding permits related to the site work contact the Site
