Please be advised that on Tuesday, March 15, 1988, the City
Commission approved the referenced site plan, subject to staff
comments, copies of which are attached.
After you have amended your plans to reflect these stipulations,
please have the appropriate Department Heads' initial the attached
check-off sheet prior to your submission to the Building
Department for permits.
The approval of the City entitles you to construct only the
improvements shown on the site plan. The site plan will be
viewed procedural~y as an as-built or record drawing.

As a condition of Site Plan Approval, the following:comments should be
incorporated into the related documents by the applicant:
1. Landscape material adjacent to the right of way must be 36" in
height at the time of planting..
2. The landscape strip adjacent to an abutting property must be a
minimum of 2.5'.
3. Indicate the finished floor elevation for the proposed structure on
the plans.. This structure must be accessible to the handicapped.
4. Sidewalks in the public right of way must be ramped at the turnouts
for handicapped accessibility.
5. The 2" water line must be removed if it will be under the proposed
structure.
6. The maximum height of the structure is 25'. Please indicate the
actual dimension on the plan.
7. A tie beam must be provided on the dumpster wall enclosure.
8. The overhang on the rear porch encroaches into the setback.

9. A Department of Transportation permit is required for all work
in the Boynton Beach Boulevard right-of-way.
10. A demolition permit is required for the removal of the existing
structure.
The applicant's prompt compliance with the preceding comments will"insure
a timely permitting process.

COMMENTS:
1. The paving and drainage plans should be signed and sealed.
2. The reproduction (zerox) of the paving and drainage plan
is very difficult to read. A better reproduction is
requested.
3. Lighting plan does not show contours for lighting intensity.
A field check should be made after installation.
4. Details for light standard mounting and embedment should be
shown, ie., design standard for 120 M.P.H. wind.
5. Handicapped space striping and symbol to be blue.
6. A no left turn sign is required at the north exit.

7. A stop sign mounting detail is required.
8. New sidewalk in N.W.' 1st Avenue to extend thru turnout.
The sidewalk in S.R. 804 to be rebui1e and conform with
D.O.T. turnout details (permit from D.O.T. required).
9. Percolation test data is required to verify calculations

We can approve this plan, subject to the following conditions:
1. The reduced pressure backflow preventer must be relocated to
the building side of the water meter, and installed above
ground in accordance with the manufacturer's specifications.
The device should be by Febco, Watts, or an equal type which
is compatible with the City's testing equipment.
2. A 12' wide utility easement must be shown encompassing the
proposed on-site water main and meter box.
3. The sanitary service and cleanout must conform to the City's
standard criteria. The cleanout must be located away from
hedges and trees, in either the right-of-way or in a utility
easement.
4. The relocated 211 water main must be of galvanized steel, not
PVC, with a ball corporation stop at the 1011 water main, in
accordance with the City's standards.

5. Add a note to the utility plans stating that all water and
sewer utilities will meet the standard criteria of the City of
Boynton Beach.
6. Black olive trees are not permitted within utility easements.

Please be advised of the Planning Department's comments in
connection with the above-referenced request for site plan

1. Sealed engineering drawings prepared by a Florida
registered engineer must be submitted which provide
for the construction of all roadway improvements that
were a condition of the previous parking lot variance
approval for this site.
2. Roadway improvements outlined under the conditions of
the previous parking lot variance approval for this
site must be completed prior to the issuance of a
Certificate of Occupancy.
3. On sheet no.S of 1S (elevations), the previous site
plan drawing should be removed as it has been superceded.

This communication is in response to your memorandum, dated January 18, 1989,
regarding the subject intersection. Please find attached a copy of the Florida
Department of Transportation's position concerning the signal study conducted
by.this office. The Department feels that a traffic signal is not warranted,
and that the situation can be better addressed by implementing median
modifications which prevent left turn movements onto Boynton Beach Boulevard.
By copy of this communication we are requesting that Mr. William A. Lewis, P.E.,
District Traffic Operations Engineer, for District 4, Department of
Transportation, inform this office of their design and implementation time-table.
Once we have received this information we will forward it to your office.

We have completed our review of the study data which accompanied
your letter of October 4, 1988. In addition to this infonnation, we
have collected accident data frum 1986 to the present.

After analysis of the study data, including pavement anu geo-
metric characteristics, we have determined that the installation of a
traffic signal would not be appropriate.
We recommend that the median be reconstructed to allow only left
and right turns from Boyntol1 Beach onto West Industrial Avenue. After
proper notification of our intention to the property owners along West
Industrial Avenue, we will implement the changes in our "push-button

As a result of your July 25, 1989 letter to Dr. Roberts in which
you attached marked-up plans depicting the median revisions that
Florida Department of Transportation required, I have revised
the original driveway permit application which originally was
designed to improve the traffic flow/traffic safety. The
revisions of the plan are not a reflection of Motorist Design
Data MovementCMDDM) recommendations. The design revisions
were mandated and required by FDOT. Therefore, I did not seal
the revised plan sheets but I am sealing this cover letter which
is attached to each set of plans. Please advise if there are
any questions concerning the impl~mented changes.

First. I would like to advise you that my partner, Michael
Dudeck. has accepted a position as the City Engineer for Palm
Bay, Florida. He started employment as of 6/13/88. By
establishing Motorist Design Data Movement, Inc. with offices in
Plantation and Merritt Island. ALL traffic engineerina services
can be fully maintained. Please note the new address.
As a result of our recent meeting, I have prepared a detailed
plan survey with recommended roadway improvements and traffic
control devices, as shown on the attached plan. Also, a cost
estimate analysis is 'attached. This package is being submitted to
the Department of Transportation to initiate the PERMIT
APPLICATION.
In reassessing the traffic operational conditions. due to the
change in the U-turn configuration. the traffic flow and safety
issues will not be adversely impacted. The access and traffic
circulation for the Snyder's site plan will be insignificantly
affected by the median opening revisions.
You indicated that it would be necessary to submit a new parking
lot variance application. I would appreciate receiving copies of
ALL necessary forms and instuctions so that a proper application

by the Technical Review Board. The basis for rejection ~s
outlined in this memorandum and the attached staff comments.
Location of driveway onto Boynton Beach Boulevard
is not consistent with Section 5-142(h)(3) of the
Parking Lot Regulations. A variance for this property
has previously been approved for this property to be
located a distance of 170 feet from the intersecting
right-of-way lines between West Boynton Beach Boulevard
and N.W. 7th Street (adjacent to the east property
boundary). A copy of the Conditions of Variance Approval
has been attached. If the site plan is revised to
reflect ,the previously approved variance, then the
Conditions of Variance Approval outlined in the attached
correspondence must be met.
The next deadline for site plan submittal is February 22,
1988. Prior to submittal, the site plan should be
revised to incorporate all staff comments.

RE: Humana Hedfirst - Parking Lot Variance Request
Section 5-144(c)4 of the Code of Ordinances requires that when a
variance to Section 5, Article X, Parking Lots is requested, the
Technical Review Board must forward to the Planning and Zoning
Board a recommendation, and that the recommendation forwarded is
to be made a part of the Public Hearing proceedings. To that end,
this memo is forwarded, consistent with 5-144(c) (4).
Sally s. Benson, agent for Humdeco, Inc., has requested a variance to
Section 5-141(g) (3) Driveways, of the Parking Lot regulations which
requires, among other things, that no driveways may be constructed
closer than 180 feet to the intersection of the rights-of-way lines
on streets of a higher classification (collectors/arterials). In
this instance, the applicant is requesting to be permitted to retain
an existing curb cut located on Boynton Beach Boulevard approximately
170 feet from the intersection with N.W.7th street. For an ex-
planation of the Code requirement, the nature of the variance.
requested, and the variance justification, please refer to the
attached Notice of PUQlic Hearing and application. '
On Tuesday, February 4, 1986 the Technical Review Board (TRB) met
to review the plans a~d documents submitted, and to formulate a
recommendation with regard to the variance requested. After re-
view and discussion, the TRB recommended that the variance requested
be approved, subject to the following conditions:

1. That the developer close the existing median cuts on
Boynton Beach Boulevard at N.W. 7th Street (both sides)
and construct the U-turn at N.W. 7th Court as per the
approved DOT plan for reconstruction of median cuts in
this area of the City.

DESIGN CONSIDERATIONS
The proposed medical facility will be a one-story, lightly loaded
structure which will be constructed at the approximate location shown by
the Plan. We assume that the structure will be~imilar to other MedFirst
facilities in that a slab-on-grade will be'utilized, and structural loads
will be transmitted to the foundation system by means of.interior 'and
exterior load-bearing walls. Column loads are not expected to exceed
30 kips; wall loads will probably be less than 2 KSF. Surrounding
pavements will be subjected only to light automobile traffic. We

estimate that yery little grading will be required to achieve final
grades for the project.

Because this report has been developed based on preliminary design
criteria, we must review our comments and recommendations in the light of
the final design.

COMMENTS AND RECOMMENDATIONS
Site Preparation .
Initially, the eXisting structure, ground vegetation and organic soil
must be removed from within the proposed building and pavement area.
Following satisfactory stripping, the exposed surface within the building
and new paved areas, including an area extending ten feet beyond the
proposed building limits, should be proofrolled with a loaded,.
tandem-axle dump truck in order to help define any zones of unstable soil
or buried debris. Any such unsuitable material must be excavated to firm
ground, and resulting excavations must be backfilled with engineered fill
compacted as stated below.

Thereafter, the building and new pavement subgrades must be uniformly
densified to the requirements for engineered fill uSing a vibratory
roller having a minimum static. weight of ten tons. The vibratory

compaction should consist of at least ten complete coverages over the
. stripped subgrade, but should continue until at least the upper three
feet of the existing subgrade have been compacted to the requirements for
engineered fill. .'

Engineered fill required to grade the proposed building and paved areas
should contain less than 10% particles finer than the 200 mesh sieve 'and
should be compacted to at least 98% of its maximum dry density as per
ASTM D 698 (standard Proctor). Compaction tests should be performed as
the fill is p1~ced in order to verify that it complies with these
requirements.
Foundation Design
We recommend that the building be founded by means of a conventionally
designed, shallow foundation system bearing on the stable, properly
,
co~pacted fill. In order to improve the bearing capability of the
subgrade soil and to provide a uniform subgrade reaction, it will be
necessary to thoroughly densify the bearing surface within each
. foundation excavation. Specifically, we reco~end that each foundation
subgrade be densified with a vibrating sled compactor until it is
compacted to at least 98% of the soil's maximum dry density as per
ASTM D 698. In order to facilitate compaction of the sand below the
footing subgrades to the greatest depth practicable, specifications
should require at least five complete coverages with the compactor,
regardless of the compaction achieved at the surface.
If the subgrade is prepared as recommended above, footings may be
proportioned based on an allowable bearing pressure of 3 KSF. In order
to enable the foundations to span minor inconsistencies in the subgrade,
footings should be at least eighteen inches in width, regardless of
loading. Furthermore, all footings should bear at least eighteen inches
below finished, exterior grade~in order to provide adequate confinement.

A conventionally designed slab-on-grade should perform satisfactorily
provided that the subgrade has been properly prepared in accordance with
the site gratllng recommendations presented in this text. A vapor barrier
should be included in the floor slab design.

Pavement Design
Considering the type and frequency of traffic likely to be associated
with the operation of this facility, it is our opinion that a light-duty

p~vement section consisting of 1.5 inches of asphalt surface course (Hot
Mix) overlying six inches of compacted limerock base will perform
satisfactorily. The limerock base should be compacted to at least 98% of ,
its maximum dry density as per AASHTO T 180 (modified Proctor). In
addition, the pavement subgrade should be stabilized to have an FBV
(Florida Bearing Value) of at least 50. All pavement subgrades should be
proofrolled again immediately prior to paving in order to detect any soft
zones that have developed during construction. Any such zones should be
repaired in the manner outlined earlier in this text.

All paving materials and procedures should conform to the applicable
requirements of the Florida Department of Transportation's "Standard
Specifications for Road and Bridge Construction," except as modified
above.
