No application shall be accepted unless it is presented on the official form of the City of Boynton Beach, Florida. The applicant must submit two copies of this application and other required information. Before an application may be accepted, it must fully comply with all required information as stated below. ATTACH THE FOLLOWING REQUIRED INFORMATION TO THIS APPLICATION: 1. a. A copy of the recorded subdivision plat if the subject property is a lot or lots of record. or b. All other property must be shown on a certified legal survey made by a surveyor registered in the State of Florida and drawn to a scale appropriate to the size of the subject property. The legal survey shall include an accurate legal description of the subject property to be annexed, and a computation of the total area of the subject property, computed in square footage and to the nearest one-tenth (1/10) of an acre. 2. A statement of the applicant's interest in the property to be annexed, including a copy of the last recorded Warranty Deed, and: a. If joint and several ownership, a written consent to the annexation petition by all owners of record, 2r b. If a lessee, a copy of the Lease Agreement and written consent of the owner, or c. If an authorized agent, a copy of the Agency Agreement or written consent of the seller/owner, or d. If a corporation or other business entity, the name of the officer or person responsible for the application and written proof that said representative has the delegated authority to represent the corporation or other business entity, or in lieu thereof, written proof that he is in fact an officer of the corporation or other business entity, or e. If a contract purchase, a copy of the Purchase Contract and written consent of the seller/owner. FIoI1da33G MANOR FOREST REGULATING REPORT AND LAND USE JUSTIFICATION Introduction Manor forest PUD is located at the intersection of Lawrence Road and Lateral Canal #20. On 11/24/93 the 91.58 acre PUD master plan, (Pet. # 82-71), was certified. By request of Lawrence Oaks HOA, the PUD was divided along the phase III line and the southern portion was annexed into the town of Boynton Beach on 6/20/95, The north portion, (Manor Forest HOA) protested the annexation, causing the pud to be divided. The PUD being divided between Boynton Beach and Palm Beach County has created the need to split the PUD and it's development regulations. Both subdivisions were built out along the proposed boundary line at the time of annexation, This split has forced Manor Forest by code to clear up the development order, The intention of this DOA submittal is to amend the Manor Forest development order which will reduce the gross acreage ofthe PUD to 49.6 ac., establish a new southern PUD boundary, and transfer 9 zero lot line units from phase III to phase I. Phasing Plan The development has been built out in three phases, The following units numbers reflect the plated unit counts for those phases. The first phase contains 73 dwelling units consisting of7 single family, 57 multi-family and 9 transferred zero lot line units from phase III, Phase II contains 83 dwelling units consisting of21 single family, 39 patio homes and 23 town homes, Phase III contains 2 existing zero lot line homes, Recreation Facilities The Manor Forest PUD will supply 4.02 acres of passive and active recreation area, Transportation Program The sites entry lies along Palm Tran bus route 63. Adequate pedestrian access has been supplied to the PUD entry, but no bus stop structure has been required or constructed as per the previously approved master plan. A bicycle circulation system will coexist within the pedestrian access paths, and bike racks will be provided within the recreation areas. Land Use Justification The proposed development does comply with the Palm Beach County Future Land Use Designation on the property.
