Gentlemen:
The following information is being provided as requested to form an understanding of the
useage of the above referenced tenant improvement as per plans by LL. Thompson Architects
dated 15 August 1994 and our various conversations.
The meeting times for the local Church unit which will use this space and the utilization of
the various areas are as follows:
The principal useage of this facility will occur on Sundays. The meeting times will be for a
three hour block starting at 10:00 A.M. and ending at 1:00 P.M. The entire congregation will
meet in the chapel from 10:00 A.M. to 11:10 A.M. At 11:15 A.M., the children and youth are
dismissed to the classroom area and the adults continue in the chapel for their Sunday School
class until 12: 10 P.M. The men and women then separate for different classes, the men meeting
in the chapel and the women in the largest classroom. The meetings conclude at 1:00 P.M.
The other primary useage of this facility will be on one night per week, typically Tuesday or
Wednesday, but the actual night is at the discretion of the local unit leaders. This meeting
begins at 7:00 P.M. in the classroom area and involves the youth ages 12 to 18 and their
leaders (Presently approximately 10 - 12 people). This meeting usually concludes at 8:30 P.M.
On occasion, the chapel may be used for a social gathering. Such activities are usually
held on a Friday or Saturday night after 7:00 P.M., or perhaps a Saturday morning when most of
the members are off from work.
The office areas are used by the local unit leadership, with one office being designated as a
private office for the branch president, one office for the clerk and the other offices set up

On October 6, 1994, your submittal for Site Plan Review of the
above-referenced project was reviewed for completeness. It has been
determined that the submittal is substantially complete with the
exception of signed applications and accepted for further
processing. A Planning & Zoning Department staff member will be
responsible for'the review and coordination of your site plan
through the remainder of the Site Plan Review process.
The next step in the review process is for the Technical Review
Committee to review the submittal for compliance with the review
standards identified in Part III, Land Development Regulations,
Chapter 4, Site Plan Review, Section 8 and all applicable sections
of the Boynton Beach Code of Ordinances. The results of the review
will be available to you on November 4, 1994.

Find attached for your review the plans and exhibits for the above-
referenced project. Site Plan Review approval for this project
will be granted by the City Commission. To ensure that the project
stays on line with the review time frame, I request that the plans
and exhibits be reviewed and formal written comments transmitted to
the Director of the Planning and Zoning Department no later than
5: 00 P . H. on october 21. 1994 ~ Please return plans as it is
anticipated only one review will be required.
Adhering to the following review guidelines will promote a
comprehensive review and enable the applicant to efficiently obtain
Technical Review Committee approval:
1. Use the review standards specified in Part III, Land
Development Regulations, Chapter 4, Site Plan Review, Section
8 of the code of ordinances to review and formulate comments.
2. The documents submitted for the project were determined to be
substantially complete based on the submittal requirements
identified in Section 7 of the site Plan Review Ordinance.
However, if the data provided to meet the submittal
requirements is insufficient to properly evaluate and process
the project based on the review standards or the documents
show code deficiencies, additional data and/or corrections
should be requested by the reviewer through the Planning and
Zoning Department.
3. Each comment shall reference the section of the code that is
incorrectly depicted on the documents.
4. Technical Review Committee member (s) shall identify in their
comments when the plans depict or when the location and
installation of their departmental required improvements may
conflict with other departmental improvements.

Dear Ms. Heyden:
The purpose of this letter is to outline the intended
uses of the above-referenced premises by Bethesda Healthcare
Corporation. The information is provided for purposes of assisting
the City in developing a shared parking study in conjunction with
the use of the premises by the Mormon Church. In the immediate
future (within the next forty-five to sixty days), Bethesda
anticipates occupying approximately 3,500 square feet on the second
floor of the building for administrative office usage. However, in
the next twelve to twenty-four months, it is anticipated that we
will be occupying all 10,000 usable square feet on the second floor
for administrative office purposes. Additionally, within this
latter time frame, it is anticipated that we will be occupying
5,000 square feet on the first floor for medical office use.
Expected hours of operation for the administrative office
and medical office uses are Monday through Friday, 8:00 a.m. to
5:00 p.m., however, it is also anticipated that, at some point in
the future, we may wish to add medical office hours for patient
care visits on Saturdays from 8:00 a.m. to 12:00 p.m.
I hope that the foregoing provides you with the
information necessary to complete the parking study. If any
additional information is required please do not hesitate to call.

We would like to present several ite:ms for your consideration as we continue the process of complying
'with the requirements of the Planning and Zonin.g Department in relation to the above project and the
occupancy of this: spsce,
Prior to leasing this tenant space~ we requested a cursory inspection from the Building Department as an
indication of the requirem.ents that would have to be met in order for om local church unit to occupy tills
space. This inspeotion was performed by ~Ir. Joh..1"JSOn and the results indicated that tena.nt separation
I'CI<luttements needed to be verified (2 hours at the corriclor and 2nd floor slab) and that the mechanic~l
and electrical syste.ms needed to bt' chocked out, but otherwise concluded that there would be; no other
major additional requirements. ll1ete was no mention of a change of occupan.cy and the corresponding
l'equllen.lents at that time. Negl;ltiations followed tl1i.... favorable inspection report and a lease was signed.
The' .cotrl.l:nitJr.ent of the Church was now:fi..rmly establimed at that point in regard to this: tenant
ito.proYement l),nd the pending OC{..'Upancy of the location. Had the rt:quircmenl:$ to which We arc now
being subj~'1:ed been noted prior to our signmg of the lease, it is doubtful that tlUs space would have been
considered.
Without bel.6.borlng the point, the Church is now paying rent on. a tenant improvement that has become
much more mvolved that il; was suggested at the outset of t.lte initial site acquisition process. However, as
evidenced by our application for the parking lot variance for an existing driveway. and our recent
submittal for a major site plan revision indicating our striving to comply with the requirements of the
Flanning and Zoning Department, ~ process has not been ~o ea~ly c:o.ncl1J.ded as originally $:upposed,

The meeting times for the local Church unit which will use this space and dIe utilization of
the various areas are as follows:
The principal useage of this facility will occur on Sundays. The meeting times will be for a
three hour block starting at 10:00 A.M. and ending at 1:00 P.M. The entire congregation will
meet in the chapel from 10:00 A.M, to 11:10 A.M. At 11:15 A.M., the children and youth are
dismissed to the classroom area and the adults continue in the chapel for their Sunday School
class nntil12: 10 P.M. The men and women then separate for different cla3ses, the men meeting
in the chapel and the women in the largest classroom. The meetings conclude at 1 :00 P.M.
The other primary useage of this facility will be on one night per week, typically Tuesday or
Wednesday; bur the acmal night 1s at the discretion of the local unit leaders. This meeting
begins at 7:00 P.M. in the classroom area and involves the youth ages 12 to 18 and their
leaders (Presently approximately 10 - 12 people). This meeting usually concludes at 8:30 P.M.
on occasion, the chapel may be used for a social gathering. Such activities are usually
held on a Friday or Saturday night after 7:00 P.M., or perhaps a Saturday morning when most of
the members are off from work.
The office areas are used by the local unit leadership. with one office being desiO'nated as a
private office for the branch president, one office for the clerk and the other oiiices set up

are there any paid church employees, including the clerk staff. These are atllay leaders,
members oftfie local unit and many have fun-time occupations which make them unavailable for
church service during normal working bours, No one will be employed to answer the telephone
in this facility and no mail v.ill be delivered to this site, except perhaps for unsolicited
mail. All leaders receive mail at their home addresses. The office areas will typically be
used before and after the three hour meeting block on Sundays and on the activity night during
the week.
This con~re2ation presently numbers 40 people. including children. At the present growth
rate which the church has experienr-ed in this area, the e.:~.pected increase in membership
should be approximately 13 - 15 % per year for the three year term of the lease.

Dear Mr. Lake:
Thank you for your notes regarding drainage at subject property.
Interestingly, I called the number you had left (407-395-4126)
earlier today but there was no answer. I was calling to inform you
that last Thursday I personally visited 500 Gulfstream Blvd. toward
the end of a rather heavy storm and observed the drainage system.
It was operating very well, and based on that observation we see no
need to modify the system.
While on site, I also observed that all light fixtures were
working. Mr. Hall will return to the site after dark to determine
whether the light provided is sufficient.
Thank you for the test summary and testimonials with respect to
drainage. They also entered into our decision.

Dear Mr. Lake:
Mr. Hall of this office has determined that parking lot lighting
levels are not sufficient to meet City standards. Readings vary
from a low of 0.10 foot candles in the east parking lot to a high
of 3.5 foot candles immediately below the fixture in the west lot.
Other readings range from a high at one point of 1.5 foot candles
to a low of 0.25 foot candles. The mean level appears to be
slightly above 0.5 foot candles, or about half our standard.
Please let us know when you have corrected the lighting and we will
revisit the site.

further landscaping is proposed. All property lines have eXisting
hedges.
Building and Site Regulations: No exterior changes are proposed to
be made to the existing building and it meets all setback
requirements.

community Design Plan: The site is located in the coastal Village
community Design District. The proposed work does not activate
compliance with the requirements of the Community Design Plan.
Parking Facility: The proposed shared parking analysis (see
Exhibit "A") points out that business office use, medical office
use, and church use including chapel, church classrooms and church
office use, are all proposed within the Gulfstream building. The
analysis of the proposed agreement shows that the most intensive
use occurs between 8:00 a.m. to 5:00 p.m. on weekdays when the
church is not being used. This time period would be used by the
medical offices and non-church business offices concurrently. It
would require a total of 77 spaces, plus a 10% buffer or 85 spaces.
The proposed parking facility has 89 spaces which is a combination
of the existing 10 foot by 20 foot spaces and restriping of some
spaces to 9 feet by 18 feet in size. A partial re-striping is
required to create the appropriate twenty-seven (27) foot wide
access aisles and to show compliance with the current handicapped
parking space requirements. Re-striping of the entire parking lot
would increase the number of parking spaces from 89 to 91 spaces.
Signage: No additional signage is proposed at this time. Any
additional signage area would comply with the Sign Code.

The following chart depicts the daily demand for parking spaces
at the site. The demand is based on the spaces required by the
code as derived from the data provided by the prospective tenants
of the building. The prospective tenants would not occupy the
entire building, therefore an assumption was made that the
unknown business office tenant space of 9,562 square feet would
have 8:00 A.M. to 5:00 P.M. office hours:

'" The parking demand includes 77 spaces for business and
medical office use between the hours of 8:00 a.m. and 5:00
p.m. and the remaining spaces are for the church use which
includes the chapel on Friday and the classrooms on Tuesday
and Wednesday with both uses occurring between 7:00 p.m. and
10:00 p.m.
Conclusion: Based on the daily use of the premises, excluding
the church use of the property after 5:00 p.m., the maximum
number of parking spaces needed at the site would be 77 spaces.
Consistent with the code, a 10% buffer of spaces must be added to
the 77 spaces thus, a total of 85 spaces are required to
accommodate the maximum demand for parking spaces. The 85
required spaces does not exceed the 89 spaces provided at the
site, therefore the shared parking is acceptable based on the
data provided by the proposed tenants.

We have reviewed the plans and documents and support forwarding this
project through the approval process, contingent upon consent
regarding landscaping, site lighting and parking lot requirements by
Planning & Zoning and Engineering.
Compliance with the state handicap requirements must be met by showing
access to the main entrance of the building from the street right-of-
way, either from Gulfstream Boulevard or Old Dixie Highway.
We also agree with the request for shared parking, subject to site
plan approval.
Plans for signs should be site approved prior to submittal for
permitting.

The following is a list of 1st review comments regarding the site
plan review of the plans for the above-referenced project. It
should be noted that the comments are divided into two (2)
categories. The first category is a list of comments that identify
deficiencies that are required to be corrected and shown in
compliance on the plans and/or documents submitted for second (2nd)
review in order for the project to continue through the site plan
review process. The second set of comment(s) are recommendations
that the Planning and Zoning Department staff believe will enhance
the proposed development. The applicant shall understand that all
documents/plans submitted for site plan and/or permit review are
subject to additional comments. I recommend that the applicant/
agent contact me regarding questions related to the comments. If
the applicant is not intending to correct code deficiencies they
should contact me regarding the procedures, application forms, fees
and submittal deadline dates for seeking relief from the code
requirement.
I. SITE PLAN REVIEW COMMENTS:
1. Parking access aisles with parking on both sides require a
width of 27 feet. The plans show a width of 25 feet with 20
feet deep parking stalls. Modify the drawing titled "Proposed
Site Plan" to show no less than 18' feet deep parking space
and 27' feet wide access aisle.
(City Standard Drawing B-90013).
2. A variance is being requested for relief from the requirement
for a distance of 180 feet from the intersection of major
street right-of-way lines to a driveway. This request is
being processed concurrently with the site plan modification.
Modify the drawings to show compliance with the final
determination of the variance. Chapter 5 Article X, Section
5-142(d)(3).
3. The required parking spaces for the major uses exceeds the
total number of spaces provided at the' si te. Therefore,
shared parking is requested as part of this application (see
attachment "A".) The planning and zoning analysis made part
of attachment "A" and shows that the parking spaces proposed
with this modification will meet the parking demand based on
different hours of operation for the church use versus the
standard hours of operation for the office and medical office
uses. Modify the drawings to show compliance with the final
determination of the shared parking requirement. (Appendix A -
Zoning section 11.H.13).

4. Modify the drawing (Proposed Site Plan) to show the location
of the city Standard curbing required to protect the landscape
areas. (Chapter 5, Article X, Section 5-142(e).
5. Specify in note form on the plan titled "Proposed Site Plan"
the following landscape specifications:
a. All la,ndscape material shall conform to standards of
Florida No. 1 or better.
b. All landscaped areas shall be irrigated with an automatic
irrigation water supply system.

c. All landscaping shall be installed in a sound workmanlike
manner and according to accepted good planting
procedures.
(Chapter 7.5 Article II, Section 7.5-35(a),(b) and (c).
II. RECOMMENDATION(S):
It is recommended that the parking facility be restriped to omit
the 10 feet wide spaces and create the current requirement of 9
feet wide parking spaces, in light of the need for shared parking.

We would like to present several items for your GorlSlderation as we continue the process of complying
with the requirements of the Plann:ing and Zoning Department in relation to the above project and the
occupancy of ~his l>pace,
Prior to leas:ln~ this tenant space, we requested a cursory inspection from the Building Depalttnem 3S an
indicaticJ!i of dle requirements that would have to be met in order for our local church unit to occupy this
space. This i~pection was peIfonned by ~Mr. Johnson and the results indicated that tenant separation
requiremerds t~eede.d to hi, verified (2 hours at the o,orridor and 2nd floor slab) and that the meohanical
and electrie;al systems needed to be checked out, but othern'ise concluded that there would be no othcr
major additi,Qnal re,qu:lremenm. There was no mention of a change of occupancy and the corresponding
raquire.ments ~t that time. Ne2otiations followed th.is favorable inspection report and a lease was signed.
The' conunitment of the Church was now fIrmly established at that p:>int in regard to this. tenant
improvement and d)e pending occypanc)" of the location. Had the requirements to which we am now
being subjected been noted prior to our signing of the lease, It is doubtful that this space would Thwe been
conside.red.
Without bclabbrlng the point, the ChUrch it< now paying rent on u tc!ltlnt improvement that has become
much more intolv~d that It was suggested at the outset of the inItial site acquisition process. However, as
evidenced by our application fO!" the parking lot variance for an existing driveway and our recent
submittal for a: major site pum revision indicatirtg our striving to comply with the re.quirements of the
Planning a.nd Zoning~. tb.is ptocer>s h.'lS not been so easily concluded as; originally ;oupposed..
The time that is required to c,omplete the ahoy>: approval processes will mean that we will pay four or five.months of rent without ever having occupied the space.
