hereby acknowledge, represent and agree that all plans,
specifications, drawings, engineering, and other data
submitted with this application for review by the City of
Boynton Beach shall be reviewed by the various boards,
commissions, staff personnel and other parti6s designated,
appointed or employed by the City of Boynton beach, and any
such party reviewing the same shall rely upon the accuracy
thereof, and any change in any item submitted shall be deemed
material and substantial.
The undersigned hereby agrees that all plans,
specifications, drawings, engineering and other data which may
be approved by the City of Boynton Beach, or its boards,
commissions, staff or designees shall be constructed in strict
compliance with the form in which they are approved, and any
change to the same shall be deemed material and shall place
the applicant in violation of this application and all
approvals and permits which may be granted.
The applicant agrees to allowing the City of Boynton

the plans, please contact Michael Haag (ext. 6262). To ensure that

Review". If there are additional questions regarding the review of
the plans, please contact Dan DeCarlo (ext. 6264). To ensure that
the project stays on track with the review time frame, the plans
and exhibits shall be reviewed and formal written comments on legal
size paper transmitted to the Director of the Planning and Zoning
Department no later than 5:00 P.M. on September 13. 1995. Return
plans and exhibits with formal comments. The project will be
discussed at the August 8, 1995 TRC meeting.
Due to additional staff time required to review resubmittal of the
above-referenced plans, please document in writing (under separate
memo or speed memo) the cost of your additional review time and

is to be computed as follows:

Staff has reviewed the above referenced project and offer the following comments:
1. Palm Beach Health Department permit may be required, (Sec. 26.12).
2. City water will not be supplied for irrigation. Please show source and
location on the plans, (City Compo Plan, Policy 3C.3.4).
3. A Reservation Fee of $ 790.02 is due within Thirty (30) days of
Commission approval or upon request for my Signature on HRSIDEP
forms, (Sec. 26-34 [ED.
4. The 10" water main shown on N. W. 7th Court is not correct. Utilities
requests a meeting, with the project engineer, to determine where sufficient
size water mains are located for an extension to the proposed fire hydrant.
It is our recommendation that the plan proceed through the review process.

Our fourth memo (#95-382) still applies. Items 2, 3 and 6 are
outstanding as far as we know. They are as follows:
2. Specify that pole design is capable of withstanding 110
mph wind load. This may appear on site electrical plan
SB, but that sheet is not included in the documents
forwarded to us.
3. Provide F.D.O.T. approval of storm sewer connection to
Boynton Beach Blvd. prior to permitting.
6. Provide drainage certification.
The following additional comments apply to the latest submission:
A. Photometrics must be approved for both pedestrian and
parking lot lighting before building permit can be
issued. Chap. 23, Art. II, A.1.a, pg. 23-6.
B. parking lot dimensions, striping, aisles, stalls,
radii, signs, landscaping, etc. must conform with City
codes and standards. Chap. 23, Art. II, pg. 23-6.
C. Provide stop signs and stop bars in accordance with the
"Manual on Uniform Traffic Control Devices." Chap. 23,
Art. II, B.2, pg. 23-7.
D. Inlets must be in grassy areas unless otherwise
approved. Chap. 6, Art. IV, Sec. 5.B, pg. 6-8 and
Chap. 23, Art. II.F, pg. 23-8.
We recommend that this application not go forward until a firm
cross access agreement is signed, recorded, and in your hands.
The time spent by Mr. Hall and myself on this fourth review
approximates one hour each, or about $100.

After review of the documents for the above project the Building Division offers the following:
1. All signs must be in compliance with the City Sign Code:
A. Chapter 21, Section 21.25 (c) prohibits signs on awnings.
B. The maximum wall sign allowed is 33.87 Sq. Ft.
C. Directional signs will have to be a maximum of 4 Sq. Ft. and 5 Ft. high.
D. Dimension and setbacks for the signs must be shown on the Site Plans.
E. The 8 1/2" X 11" sign detail sheet is adequate, but should be shown on the
Site Plan in compliance with dimensions and setbacks.
2. The building is still shown in excess of 500 Sq. Ft., which requires two accessible
rest rooms, one for male and one for female, in compliance with Table 922.2 of
the Standard Plumbing Code.
If this would requires window openings in the exterior walls, it would have to be
shown on the Site Plans also.

Public Works has not seen any agreement with cross access to this
