1. Mike Corbett reviewed the comments per the engineering' department
as he had picked them up on the plan to Sebastian Puda.
2. Mike Corbett indicated that he did add the oneway signs in front
of both entrances in the median and corrected the handicapped
parking detail, as its was to Boynton's earlier approved code.
3. Mr puda wanted Boynton's detail of the swale between the parking
and we showed him the pervious parking lot swale which is not the
same. We told him we were using pervious concrete, that it was
not the same as typical paving and that it did not require the
deep swales like standard paving does. I told him this would be
very good environmentally and good for storm water retent~on on
the project also.
4. Mr. puda said he could not approve the project until the product
was approved by Boynton Beach's Engineer. We told him that we
had informed the City as far back as the TRC for site plan review
on this project, that we intended to use "pervious concrete" for
the pavem~nt, ,and everyone was enthusiastic about it.
Comment: By the comments and the requirements Mr. puda was asking for
in connection to the parking lot perviqus concrete, it became
obvious that he did not understand how pervious concrete works.
r tried to explain but, the discussion became somewhat skewed.
5. We asked Mr. puda if he had ever used it before, understood how
it worked, or even knew what it was? He said no. We said that
it is so good that once he saw it he would be insisting on it all
over Boynton. We told him the swale detail was on the drawing.
6. He said that he would not approved it unless the engineer did.
We would need to submit it for approval first. We told him this
shouldn't hold up the project. He said then r can't approve it.
Mr. puda then left the meeting.
Comment: Mike Corbett then reviewed the Planning & Zoning comments with
Jim Nguyen.
7. We reviewed comments with Jim and he approved the plans and
signed them off.

1. "Sign Permit Required" (Not True)
The existing sign was granfathered, as an approved
existing sign and a sign permit was not required.
Mike Haag agreed, and said he would not require a sign
permit to construct the sign but he would need to
approve the face of the existing sign prior to when the
na~e was painted on each side.
COMMENT: We were a~ked by the planning department to show
the existing sign on our site plan approval drawings,
which we did. This sign was discussed in front of the
City Commission. Tambri Hayden confirmed to the

wer.e told by them that a sign permit would not be
required in as much as all we were doing was going to
repaint the masonry to match the building and the
Doctor's name and Dental Office with street address
number in black letters on each side. They said this
would not be a problem.
2. MiKe Haag and Don Johnson were informed by Mike Corbett
that the time was critical to clear this permit out of
the Building Department. Don informed us that Mr.
Kastarlak had informed him of that fact very directly
yesterday after the doctor had contacted the city. I
told Don and Mike Haag that I felt they were trying to
accomplish this, and, that I had let Mr. Kastarlak
know this earlier, but we needed to make this permit
ha~pen as soon as we could.

3. "Site Lighting per-mit Requir-ed" (Not Tr-ue)
Mike Corbett told Mike Haag and Don Johnson that Site
Lighting permit which was stamped on the drawings
by Mike Jacobson was not required per discussions with
Mike Rumpf and Mike Jacobson.
The Doctor, at Mike Jacobson's request, had given the
city a letter signed by him and the Architect, stating
that he was not going to have office hours past 5:00pm
in the evening. Then the building department requested
that the doctor come back in and have his signatur-e
notarized on the same letter-.
We wece under the impcession that the City would not
r-equice site lighting per the Boynton Beach Code as
this business was not open at 5:30 or later. Per Mike
Cor-bett, the County lighting code does not kick in
until one half hour- after- dusk for business.
Mike Haag called Mike Rumpf on the speaker phone for-
cler-itication.
Mike Rumpf contir-med that since the city had the letter-
and the doctor was pr-oviding some site lighting which
would be near the spaces most often used in an
emergency call that he would waive the max-min average
foot candle city requir-ements and ther-eby deleting the
necessity of the photometr-ic study for the entir-e
parking lot.
Mike Haag did ask for the photometric cut sheet on the
fixture along with the pole height. Mike Jacobson did
want to be sure that the fixture was a down light and
would not shine in neighboring yar-ds. We confirmed it
would not, and that we would get them the cut sheet for
the fixture.
Bob Donovan infor-med Mike Corbett, the Engineer-ing and
the Planning Depts. would review their comments thdt we
had picked up on the plans after we met with Mike.

picking up either the permit plans or comments, that he
had to correct for permit. The owner had called and
said to see her and that the permit was ready. She
told Mike Corbett that she would give him the plans or
the comments but not both and that he would need to see
Mike Haag with the plans and permit comments.
2. Mike Corbett showed the comments per the engineering
and planning department as he had picked them up on the
plan to Mike Haag.
3. Mike Haag was informed by Mike Corbett that he was
called by the Doctor, and told that the permit was
ready ~t the Building Department. Mike Haag said they
were tryiny to accomplish this, and, that he got my fax
letter which cut things loose at Water utilities,but we
still needed to clear Engineering and Planning &
Zoning. We told Mike Haag that we had tried to see
them but that on the scheduled day they could not see
us due to visiting dignitaries. We agreed try to see
them now and to make this permit happen as soon as we
could.
Comment: Mike Corbett then went to the Engineering Dept. to
clear their comments.

4. Mike Corbett met with Ken in Engineering and reviewed
Mr. Sabastian puda's comments on the project. Ken was
extremely helpful to explain and clarify the comments
carefully and we determined that we could accomplish
almost all the comments without a problem. We did add
the oneway signs in front of both entrances and correct
the handicapped parking detail as ours was to Boynton's
earlier code. We told him, we would be using pervious
concrete and, that it would be a very good thing for
Boynton to get on board with for other projects too.
Comment: Mike Corbett then went to the Planning & Zoning
Dept. to clear Jim Nguyen's comments.
5. Mike Corbett met with Jim next and reviewed his
comments on the project. Jim was also very helpful and
clarified any questions about his comments.
We told Jim that we were not required to have a
dumpster per a letter from trash collection department.
We had requested and were granted curb side pick up.
Jim's only other question that we discussed was that
our Landscape Architect had discussed the deletion of
the vines on the back side of the wall. He was told by
Kevin he thought, that they would omit that comment as
the vines would die due to lack of water and there was
no way to maintain them on the back side of the wall.
6. Mike Haag asked us, when all the comments were picked
up, to bring the plans directly back into him, so they
would not sit on a desk by mistake.

We offer the following comments on this project:
GENERAL COMMENTS
1) All utilities easements shall be shown on the rectified landscaping drawings so that we may
determine which trees may interfere with utilities. In general, palm trees will be the only tree
species allowed within utilities easements. Canopy trees may be planted outside of the easement so
that roots and branches will not impact those utilities within the easement in the foreseeable future.
LDR Sec. 7.5-18.1 gives public utilities the authority to remove any trees that interfere with utility
services, either in utility easements or public rights-of-way.
2) Fire flow calculations will be required demonstrating the City Code requirement of 1500 g.p.m. as
stated in LDR chap. 6, Art. , Sec. 16, or the requirement imposed by insurance underwriters,
whichever is greater. (see Sec. 26-16(a)).
SPECIFIC CONDITIONS
1) Water and sewer lines to be owned and operated by the City shall be included within utilities
easements. Please show all existing and proposed easements on the site plan drawings, using a
minimum width of 12 feet (for proposed easements). The easements shall be dedicated via separate
instrument to the City as stated in Sec. 26-33(a) of the Code.
2) This office will not require surety for installation of the water and sewer utilities, on condition that
the systems be fully completed, and given to the City before the first permanent meter is set. Note
that setting of a permanent water meter is a prerequisite to obtaining the certificate of occupancy.
3) LDR chap. 6, Article IV, Sec. 16 requires that all points on each commercial building be within
200 feet of an existing or proposed fire hydrant. Please demonstrate that the plan meets this
condition, by showing all hydrants.
4) Show existing and proposed water and sewer services to the site.
5) Appropriate backflow preventers will be required on the domestic water service to the building,
and the fire sprinkler line if there is one, in accordance with Sec. 26-207.
6) A building permit for this project shall not be issued until this office has approved the plans for the
water and/or sewer improvements required to serve the project. (Sec. 26-15)

We have reviewed the subject plans and offer the following comments that should be addressed prior to
the project being forwarded for Board review:

I. Land Development Regulations, Chapter 9 - Community Design Plan, Section 10. I. I
states, "Signs shall be designed and treated as part of the architecture of the building and
shall be reviewed at time of site plan review," therefore, add to the submittal, the detail
drawings of proposed site and building signage, The detail drawings shall include:
a. Identify where the building .~ignage is located on the face of the building by
dimensioning their location on the elevation view drawings, The location shall
show the distance from the edge of the building to the edge of the proposed
signage including dimensioning the distance from the finish floor to the top of
the proposed signage,
b. Identify the size of the building signage by dimensioning on the elevation
view drawings, the width and height of the proposed building signage. Also
indicate in tabular form, on the elevation view drawing, the square foot area of
each sign, Note; the allowed size of signage is identified in Section 2, Article
IV, of Chapter 2 J - Sign Code and the method to determine the sign area is
defined in Chapter J - General Provisions, Article II - Definitions (sign, area
of).
c. Please refer to the sign code Chapter 21 - Sign Code of the Land Development
Regulations for other applicable regulations that may apply to the proposed
site and building signage.
d. Please refer to Chapter 9 - Community Design Plan for other requirements

10. All plans submitted for specific permits shall meet the City's code requirements at time of
application, These permits include, but are not limited to the following; site lighting, paving,
drainage, curbing, landscaping, irrigation and traffic control devices, Permits required from
agencies such as the FDOT, PBC, SFWMD, DERM, L WDD, DEP and any other permitting
agency shall be included with your permit request.
11. Provide a detail showing a ramp or curb cut to meet ADA requirements from the pavement to
the front porch area, [Florida Accessibility Code]
12, Provide handicap accessible route from U,S, #1 sidewalk to dental office. [Florida

Recommendations
1, Since the plan provides more parking spaces than required, consider locating the two
handicap spaces to the west side of the east parking area closest to the building. This will
provide more pervious area for drainage and landscaping.
2, If the handicap spaces are to remain as is, the west five foot access path is not required (one 5'
access aisle can serve 2 handicap stalls), It will also eliminate 275 sq, ft, of pavement. Note:
Handicap spaces and related striping need only to be 18' long.

Recommendation
1, Since only southbound traffic can access site, consider making the existing circular driveway one-
way from north to south, If done, the width of the driveway could be reduced creating a larger
landscape area around the existing sign,

We offer the following comments on this project:
GENERAL COMMENTS
1) All utilities easements shall be shown on the rectified landscaping drawings so that we may
determine which trees may interfere with utilities. In general, palm trees will be the only tree
species allowed within utilities easements. Canopy trees may be planted outside of the easement so
that roots and branches will not impact those utilities within the easement in the foreseeable future.
LDR Sec. 7.5-18.1 gives public utilities the authority to remove any trees that interfere with utility
services, either in utility easements or public rights-of-way.
2) Fire flow calculations will be required demonstrating the City Code requirement of 1500 g.p.m. as
stated in LDR chap. 6, Art. , Sec. 16, or the requirement imposed by insurance underwriters,
whichever is greater. (see Sec. 26-16(a)).
SPECIFIC CONDITIONS
1) Water and sewer lines to be owned and operated by the City shall be included within utilities
easements. Please show all existing and proposed easements on the site plan drawings, using a
minimum width of 12 feet (for proposed easements). The easements shall be dedicated via separate
instrument to the City as stated in Sec. 26-33(a) of the Code.
2) This office will not require surety for installation of the water and sewer utilities, on condition that
the systems be fully completed, and given to the City before the first permanent meter is set. Note
that setting of a permanent water meter is a prerequisite to obtaining the certificate of occupancy.
3) The proposed fire hydrant does not provide sufficient coverage for the proposed phase 2 dental lab.
We suggest the hydrant be properly located with phase 1 so as to avoid relocation in the future.
LDR chap. 6, Article IV, Sec. 16 requires that all points on each commercial building be within
200 feet of an existing or proposed fire hydrant.
4) Show existing and proposed sewer services to the site.
5) A building permit for this project shall not be issued until this office has approved the plans for the
water and/or sewer improvements required to serve the project. (Sec. 26-15)

Find attached for your review the plans and exhibits for the above-referenced project. Please
review the plans and exhibits and transmit formal written comments to me no later than
5:00 P.M. on March 17. 1998. When preparing your comments, please separate them into
two categories; code deficiencies with code sections referenced and recommendations
that you believe will enhance the project.
Adhering to the following review guidelines will promote a comprehensive review and enable
the applicant to efficiently obtain Technical Review Committee approval:
1. Use the review standards specified in Part IV, Land Development Regulations, Site Plan
Review and the applicable code sections of the Code of Ordinances to review and
formulate comments.
2. The documents submitted for the project were determined to be substantially complete,
however, if the data provided to meet the submittal requirements is insufficient to
properly evaluate and process the project based on the review standards or the
documents show code deficiencies, additional data and/or corrections should be
requested by the reviewer by contacting Scott Barber.

3. Each comment shall reference the section of the code that is incorrectly depicted on the
documents.
4. Technical Review Committee member(s) shall identify in their comments when the plans
depict or when the location and installation of their departmental required improvements
may conflict with other departmental improvements.
5. When a TRC Member finds a code deficiency that is outside of his/her review
responsibility, the comment and the specific code section may be included in their review
comments with the name of the appropriate TRC Member that is responsible for the
review specified.
6. If a TRC member finds the plans acceptable, he/she shall forward a memorandum, within
the time frame stated above, to me. The memorandum shall state that the plans are
approved and that they do not have any comments on the plans submitted for review and
that they recommend the project be forwarded through the approval process.
All comments shall be typed, addressed and transmitted to me for distribution to the applicant.
Please include the name and phone number of the reviewer on this memorandum. Scott Barber
will be the Planning and Zoning staff member coordinating the review of the project.
Planning and Zoning staff will send the applicant a cover letter which includes the comment(s)
and directions to amend the plans to comply with the comments and the procedures for
resubmission. When Planning and Zoning staff receives the amended plans, we will distribute
the plans with a cover memorandum to all TRC Members for review and approval.
