Ehasz Giacalone Architects, PC, agent for Palm Walk Associates, property owner, has requested the
following parcel located approximately 351 feet east of Congress Avenue on the north side of S.W. 19th
Avenue to be considered for Conditional Use approval to construct an Alzheimer care living facility to be
known as the Palm Walk adult congregate living facility:

We recommend approval of the request subject to the following comments:

I. The architectural drawings shall bc scaled, signed and datcd by a Florida registered
architect.
2. At time of permit submittal provide an executed and recorded easement release form that
indicates the roof overhang of the administration building may encroach into the thirty (30)
foot wide roadway/utility easement that is identified on the survey. Also provide an
executed and recorded casement release form that indicates the concrete block walls that arc
shown on the plans may encroach into the twenty-five (25) foot wide road & utility
easement that is identified on the survey.
3. At time of permit submittal provide a sample ofthe pink and white color specified on the
plans for the monument sign.
4. Amend the paving & drainage and landscape plans to show and label the accessible routes
that match those depicted on the site plan.
5. Modify the slope of the flare sides of the curb cut detail drawing found on the typical
parking detail drawing to reflect I: 12 side slopes. Also, show a 6' long level landing at the
top and bottom of the curb ramp.
6. Add to the wheel chair ramp detail drawing found on sheet 4 of 5, the code required slope
for the flare sides of the ramp. Also show on the plan view drawing the location of the
proposed wheel chair ramp.
7. Add to the section A-A detail drawing, the north property line and indicate thc dimension
from the property line to the edge of the parking area.
R. Add to the section C-C detail drawing, the east property line and the accessible sidewalk that
is located on the west side ofthe entrance driveway access aisle.

2. Indicate on all appropriate plans all traffic control markings and signage in the vicinity of
the covered entry. (Refer to note I above)
3. Provide an engineer's certification (letter to be scaled by engineer) that drainage will
conform with all rules, regulations, codes, etc. including LOR Chapter 6, Article IV, Section
5. [LOR Chapter 4, Section 7.F.2.]
4. All plans submitted for specific permits shall meet the City's code requirements at time of
application. These permits include, but are not limited to the following: site lighting,
paving, drainage. curbing, landscaping, irrigation and traffic control devices. Permits
required from agencies such as FOOT, PBC, SFWMO, OERM, LWOD, OEP and any other
permitting agency should be included with your permit request.
5. Site plan shows 21 parking spaces and paving and drainage plan shows 20 parking spaces.
Please revise plans to match.
6. Correct all plans in the dumpster area to agree.
7. Provide a 12'x35' loading zone for the receipt or distribution of materials. [LOR Chapter 2,
Section IIJ.2.a.]

1. A turn-around area with a 50' radius is to be provided in the parking lot at the
NW corner of the project.
2. All roadways and parking areas must be designed to support 56,000 Ibs. and
a vehicle with a minimum width of 16'.
3, The patient loading area must be adjacent to the ACLF and the roadway.
4. A hydrant is required at SW 19 Avenue; at the entrance to the driveway.
5. The location of the Fire Department sprinkler connection fire lane is not
shown.
There is a serious problem with the plan and our ability to respond to fire and
medical emergencies and not have to back up long distances to exit the parking
lot. The distance that will be required for stretchers to cross pavers to respond to
and pick up patients is unacceptable.
We do not recommend forwarding this project at this time.

Find attached for your review the plans and exhibits for the above-referenced project. Please review the
plans and exhibits and transmit formal written comments to the Director of Planning and Zoning
no later than 5:00 P.M. on January 13. 1998. When preparing your comments, please separate
them into two categories; code deficiencies with code sections referenced and
recommendations that you believe will enhance the project.
Adhering to the following review guidelines will promote a comprehensive review and enable the
applicant to efficiently obtain Technical Review Committee approval:
1. Use the review standards specified in Part IV, Land Development Regulations, Chapter 4, Site
Plan Review, Section 8, Chapter 2, Zoning, Section 11.2 and the applicable code sections of the
Code of Ordinances to review and formulate comments.
2. The documents submitted for the project were determined to be substantially complete, however,
if the data provided to meet the submittal requirements is insufficient to properly evaluate and
process the project based on the review standards or the documents show code deficiencies,
additional data and/or corrections should be requested by the reviewer by contacting Jerzy
Lewicki.
3. Each comment shall reference the section of the code that is incorrectly depicted on the
documents.

4, Technical Review Committee member(s) shall identify in their comments when the plans depict or
when the location and installation of their departmental required improvements may conflict with
other departmental improvements.
5. When a TRC Member finds a code deficiency that is outside of his/her review responsibility, the
comment and the specific code section may be included in their review comments with the name
of the appropriate TRC Member that is responsible for the review specified.
6. If a TRC member finds the plans acceptable, he/she shall forward a memorandum, within the time
frame stated above, to the Planning and Zoning Director. The memorandum shall state that the
plans are approved and that they do not have any comments on the plans submitted for review
and that they recommend the project be forwarded through the approval process.
All comments shall be typed, addressed and transmitted to the Director of Planning and Zoning for
distribution to the applicant. Please include the name and phone number of the reviewer on this
memorandum. Jerzy Lewicki will be the Planning and Zoning staff member coordinating the review of the
project.
Planning and Zoning staff will send the applicant a cover letter which includes the comment(s) and
directions to amend the plans to comply with the comments and the procedures for resubmission. When
Planning and Zoning staff receives the amended plans, we will distribute the plans with a cover
memorandum to all TRC Members for review and approval.

This parcel is located south of the LWDD L-26 Canal, but is not adjacent to same.
Therefore the District has no objection to this item at this time.

We offer the following comments on this project:
GENERAL COMMENTS
1) All utilities easements shall be shown on the rectified landscaping drawings so that we may
determine which trees may interfere with utilities. In general, palm trees will be the only tree
species allowed within utilities easements. Canopy trees may be planted outside of the easement so
that roots and branches will not impact those utilities within the easement in the foreseeable future.
LD R Sec. 7.5-18.1 gives public utilities the authority to remove any trees that interfere with utility
services, either in utility easements or public rights-of-way.
2) Fire flow calculations will be required demonstrating the City Code requirement of 1500 g.p.m. as
stated in LDR chap. 6, Art. , Sec. 16, or the requirement imposed by insurance underwriters,
whichever is greater. (see Sec. 26-16(a)). Please submit these calculations with your HRS permit
submittal.
3) Comp Plan policy 3C.3.4 requires the conservation of potable water. City water may, therefore, not
be used for irrigation where other sources are readily available.
SPECIFIC CONDITIONS
1. We still have concerns that a 4-foot high wall on the west side of the lift station will obstruct our
drivers' view when they exit the station, and pose a hazard to pedestrians, bicyclers, etc. on the
sidewalk.

The following is a list of 1 st review comments regarding the site plan review of the plans
for the above-referenced project. The applicant shall understand that all documents
and plans submitted for site plan review are subject to additional comments. I
recommend that the applicant/agent contact me regarding questions related to the
comments. If the applicant is not intending to correct code deficiencies, he should
contact me regarding the procedures, application forms, fees and submittal deadline
dates for seeking relief from the code requirement.
1. Any roof top equipment will have to be screened from view at a distance of 600 feet.
If such equipment is planned, indicate its location and the height of the equipment
above the roof level. Provide elevations of the structure showing the roof top
equipment and the required screening (all reduced to a small scale) and draw in on
these elevations the lines-of-sight connecting the highest point of the equipment
with the eye level from a distance of 600 hundred feet from the building (LDR,
Chapter 9, Section 11.E). If necessary, provide architectural details of screening
structures.
2. Provide a detailed drawings of dumpster enclosures meeting the design standards
as described in LOR, Chapter 9, Section 10.C.
3. Submit traffic study (or traffic statement) for the proposed use. The study shall be
evaluated by the Palm Beach County Traffic Division prior to the City Commission
meeting.
4. In order to eliminate potential conflicts between trees and utility lines show on the

landscape plan all easements that are existing or proposed with this development
plan.
5. Provide a recent, not older than six months, survey of the property meeting the
requirements of LOR, Chapter 4, Section 7.A. Specifically, provide a clause on the
survey stating that the property was abstracted for right-of-ways and easements.
6. Provide a unity of title for all lots and parcels comprising the site.
7. Land Development Regulations, Chapter 4, Section 7.C.1 requires that the
landscape plan shall be drawn at the same scale as the site plan. With the re-
submittal, include a reduced landscape plan matching the scale of the site plan.
8. The building footprints shown on the landscape plan do not match the building
footprints shown on the site plan. For consistency, alter the landscape plan to
reflect the proposed changes.
9. On the Landscape Plan, provide a tabular summary of the planting material to
include a "landscape material required by the code" category. Also, provide relevant
calculations for this category. For the landscape buffer adjacent to the right-of way
and along abutting properties, provide perimeter distances of the parcel for which
the landscape buffer material is being calculated (Chapter 7.5, Article II -
Landscape Code).
10. Incorporate the above calculation into the tabulation of the landscape material
provided for the site. All native species shall be distinguished from non-native
species with a recognizable symbol. Note, that the native requirement is fifty
percent of the required landscape planting (Comprehensive Plan Policy 4.4.6).
11. Provide calculations for parking area interior landscaping as required by LDR,
Chapter 7.5, Art. II, Section 5.G.
12. On the elevation drawings depict color name in addition to provided color code and
manufacturer's name of all exterior building materials or finishes.
13. With the second submittal, include a presentation board containing samples of all
exterior building materials to be used for construction and colored elevations of all
structures.

14. With the second submittal, include a presentation board containing color elevations
of the building
15. On the site plan depict all setbacks, and their distances from the property lines, as
required by Land Development Regulations, Chapter 4, Section 7.B.2.
16. On the site plan depict traffic control markings, as required by Land Development
Regulations, Chapter 4, Section 7.B.2.
17. Provide all elevations of all proposed building and structures, as required by Land
Development Regulations, Chapter 4, Section 7.0.1.
18. On the elevations of all buildings and structures denote the height of structures
(ridge of the roof).
19. On the floor plans of the buildings provide information regarding square footage of
the living area.
20. On the 2nd submittal site plan amend the tabular summary for the project to include
all project characteristics specified in Chapter 4, Section 7.E.
21. Provide dimensions of a typical parking stall in compliance with the City's design
standards (9x18).
22. Increase the back-up space to the minimum of 27 feet.

specifying the slope and surface finish. Where the slope requires an accessible ramp show
and identifY the location of the ramp including specifYing the type of material, surface finish,
width, length, slope and where required the appropriate handrails. The width, slope and
surface material of the accessible route shall comply with the regulations specified in the

4. To properly review the proposed site wall sign, shown on the Site Plan, for compliance with
the regulations ofthc Sign Code add to the Site Plan drawing, an cnlarged detailed and
dimensioned elevation view drawing of the sign including the structure that supports the sign.
The drawing shall include, but not be limited, to showing and identifying the following:
a. Configuration of the wall and sign,
b. Location, length, width, and area of the sign (maximum area allowed is 32 square feet).
e. Length of the wall,
d. Height the top of thc wall is above finish grade.
e. Height the top of sign is above finish grade,

ten (10) feet is the minimum setback distance allowed by the Sign Code. Also show and
dimension the unobstructed cross-visibility area required between the west side ofthe paved

7. The configuration of the site depicted on the Landscape Plan does not match the

4. Indicate on all appropriatc plans all traffic control markings and signage including yellow
delineator lines at thc entrance and in the vicinity of the covered entry. (Refer to note 3
above)
5. Provide an engineer's certification that drainage will conform with all rules, regulations,
codes, etc. including LDR Chapter 6, Article IV, Section 5. [LDR Chapter 4, Section 7.F.2.]
6. On the site plan change the arrow note "indicates location of a single luminaire" to
"indicates light levels in foot candles at grade".
7. The dumpster opening shall have a minimum unobstructed opening of I 0 feet and a slab
with a minimum width dimension of I 0 feet. [City Drawing A-88007 and LDR Chapter 7.5,
Article II, Section 5.1.]
8. The site development plan scale shall be at the same scale as the landscaping and paving and
drainage plans or vice versa. [LDR Chapter 4, Section 7.B.C. & F.]
9. Provide a tabular summary on the site plan listing those items specified in LDR Chapter 4,
Section 7.E.
10. On the paving and drainage plans show the handicap ramp from the accessibility path to the
pedestrian walkway.
1 l. Correct the landscaping plan regarding the size of the administrative building, the handicap
parking spaces, the "walled patio" and any other discrepancies to agree with the paving and
drainage plan.
12. All plans submitted for specific permits shall meet the City's code requirements at time of
application. These permits include, but are not limited to the following: site lighting,
paving, drainage, curbing, landscaping, irrigation and traffic control devices. Permits
required from agcncics such as FOOT, PBC, SFWMD, DERM, L WOD, DEP and any othcr
permitting agency should bc included with your permit request.

151 Review
We offer the following comments on this project:
GENERAL COMMENTS
1) All utilities easements shall be shown on the rectified landscaping drawings so that we may
determine which trees may interfere with utilities. In general, palm trees will be the only tree
species allowed within utilities easements. Canopy trees may be planted outside of the easement so
that roots and branches will not impact those utilities within the easement in the foreseeable future.
LDR Sec. 7.5-18.1 gives public utilities the authority to remove any trees that interfere with utility
services, either in utility easements or public rights-of-way.
2) Fire flow calculations will be required demonstrating the City Code requirement of 1500 g.p.m. as
stated in LDR chap. 6, Art. , Sec. 16, or the requirement imposed by insurance underwriters,
whichever is greater. (see Sec. 26-16(a)). Please submit these calculations with your HRS permit
submittal.
3) Comp Plan policy 3C.3.4 requires the conservation of potable water. City water may, therefore, not
be used for irrigation where other sources are readily available.
4) Provide an engineer's written certification that drainage will conform with all rules of the City and
the South Florida Water Management District. (LDR Chap. 4, sec. 7F)
SPECIFIC CONDITIONS
1. Oak trees and other landscaping must be removed from the lift station area, so as to allow safe
access and maintenance.
2. Provide appropriate sight lines for service vehicles backing out of the lift station site. This will
likely require reduction in wall height and landscaping.
3. The entrance to the lift station shall include a 12 foot wide concrete driveway, (minimum 4" thick
with wire mesh reinforcement), and a double wide gated entrance also 12 feet wide, in accordance
with Departmental criteria adopted pursuant to Section 26-33 of the City's Code.
4. The drainage plans indicate that the proposed pavement around the lift station site will be higher
than the existing concrete pad. The plans must be altered so as to prevent water from draining onto
the lift station pad during rain events, and allowing for proper drainage off of the pad. Please alter
and clarify these accommodations on the drainage plan.
