We recommend approval of the request subject to the following comments:

1. Add to sheet 7 of8 a detail drawing of the proposed 4-foot wide sidewalk. The drawing
shall include a section view that identifies the type of material and thickness. Also, include
either a partial plan view drawing or add to the section view drawing specifications that
identify the distance between control joints and the type of surface finish proposed for the
sidewalk.
2. Add to sheets 2 of 18,3 of 18, 4 of 18, 5 of 18 and 6 of 18 notes that will identify the
location, type of material and width of the on-site sidewalks. Add a note to sheets 5 of 18
and 6 of 18 that indicates the construction specifications for the on-site sidewalk is located
on sheet 7 of 18.
3. Modify the Parking Stall detail drawing to reflect actual conditions.
4. To verify the proposed finish floor elevations submit a copy of the South Florida Water
Management District permit.
5. Add a note to both ramp detail drawings found on sheet 7 of 18 indicating that ramps with
a rise greater than 6" handrails, constructed in compliance with the accessibility code, shall
be installed on both sides of the ramp. Add to both detail drawings a 6' long level landing
at the top and bottom of the ramps. Also, modify both drawings to show return curbs or
flare sides on both sides of the ramp.

6. To verify the total number of regular and accessible parking spaces provided on the plans. ..
