I am writing to provide you with jnformation regarding the building purchased by
Caretenders Healthcorp located at 4745 NW 1'" Court i.n Boynton Beach. Our proposal
is to operate an adult day care center for seniors in this facility. Caretenders is the largest
...provider ofadutt day health services in the United States. Once we have received
approval fOf construction from the Planning Board, we would hope to be providing
service, for our clients by August of 1998.
This SeniorCare Day Center will have a total daily capacity of approximately 65 seniors.
Caretendera staffing pattern is based o.n a one to six ratio of staff members to clients.
Most of the senion who attend our centers are ambulatory with an approximate tell ;\b.ry A. Yannuth
percent requiring tbe assistance of some type of a.ssistive device such as a wheelchair, Sr. Vic.- P,'eqid..nt
cane or walker Our centen are required by policy to conduct 8 monthly fire dolJ and
are able to exit tbe building in less than two minutes. StafF members ate trained in CPR
and buic First Aid.
The types of medical emeT8cncy calls you might receive from our d.ay center would
involve accidents~ faUs, choking or cardiac/respiratory arrests. All of our staff members
"are trained in CPR and able to begin resUScitation efforts in an emergency situation I
would estimate no more than one call a month at the most from our center. Most of our
senior guests are neither terminal nor severely medically disabled while attending the
center. I would not anticipate II great deal of activity from the Caretenders Day Center
that would requite a response fTom the Boynton Beach Fire - R.c!roue unit.

ADMISSIONS CRITERIA:
Caretendcrs SeniorCare Day Center is a day health service for the elderly whose activities
are limited by injury and disease We provide a safe and therapeutic environment
supervised by a caring and medically trained staff. Adult day care is designed to serve
elderly persons with impairments which prohibit their living independently without
supportive services.
1. Age: The prospective guest is 6S years of age or older.
2. Behavior: The prospective guest's behavior appears to be acceptable for a group
setting~ the guest is ambulatory, either independently or with aides (crutches, walker,
wheelchair). Approximately 90% of the guests are ambulatory on their own, l()Ojo are
ambulatory with assistance. All guests are capable of elciting the building in two minutes
or less.
3 Consent: The prospective guest has agreed to enroll in the adult day care program.
4. Responsible Person: There is a person that accepts or is willing to accept responsi-
bility for the guest. This person could be a spouse, other family member, a conservator
of the guest and/or estate or one with power of attorney.

RE: CARE TENDERS AT STAMFORD PARK (NEWPORT PLACE)
Dear Ms. Heyden:
The Palm Beach County Traffic Division has reviewed the traffic
information for the project entitled Care Tenders, pursuant to the Traffic
Performance Standards in Article 15 of the Palm Beach County Land
Development Code. It is located on the south side of Hypoluxo Road.
The project will change an existing S,300 square foot medical office
building to an adult day care center. The medical office will generate
215 daily trips. The study assumes the 70 occupants will arrive by 7
facility owned vans. If this assumption is true, the proposed use will
generate less than 100 daily trips.
Since there will be a reduction in traffic, the Traffic Division has
determined that the project meets the requirements of the Palm Beach
County Traffic Performance Standards. If you have any questions
regarding this determination, please contact me at 684-4030.

is now proposed that this building be converted to an adult day care center. The adult day care center
expected to serve 70 occupants with 12 staff members. You requested that we examine the traffic, parking
and sewage requirements to determine if the change would result in any needed improvements.
Traffic
According to the Palm Beach County Land Development Code and the Institute of
Transportation Engineers Trip Generation 5th Edition, a medical clinic of this size would
be expected to generate 215 trips per day with 0% of the traffic being pass-by trips. All
clients arriving and departing from this facility shall be transported by seven 12 to 15
passenger vans operated by Care Tenders. Only the staff shall drive their vehicles to the
facility. Therefore, we conservatively estimate the trips that will be generated for the new
facility will be under 100 per day. The transportation furnished by Care Tenders will
reduce the impact of the public roadway system that was previously generated by the
medical office.
Parkin~
A medical office typically requires one parking space per 200 square feet. This results in
31.5 spaces for a medical office which corresponds to the existing parking that was
originally provided. A day care center with a capacity of less than 100 typically requires
5 transient spaces plus one per employee. This results in 17 spaces indicating that the
existing parking facilities will be adequate for the proposed use.

office space per doctor, the existing building could accommodate 6 doctors. This results
in 1,500 gallons per day. A day type school is expected to generate 15 gal/day/occupant
and employee plus 5 gal/day/shower. Allowing for one shower in the facility, this results
in 1,235 gallons per day.
As described above, the traffic, parking and sewer demands are lower for the proposed adult day care
center than they are for the existing medical office. Therefore, we do not recommend any improvements.

Dear Mike,
This letter is to request a modification to the Master Site Plan for Newport Place.
The existing 6,300 s.f medical office building on Tracts 2 & 3 has been purchased by
Caretenders, Inc. Caretenders, Inc. proposed to remodel the interior of this building to an
Adult Day Care Center. The maximum number of guests shall be 70 per day and have a
maximum staff of 12. The facility is generally open during "normal business hours" and
"extended hours" as requested by guests. There is not overnight care at this facility.
There is also no cooking at this facility.
This use does not impact the P.U.D. or surrounding area more than the medical
office. Only the staff drives their vehicles. Guests are transported to the facility via 12 -
15 passenger vans operated by Caretenders. This significantly reduces the traffic impact
from many vehicles driving to a medical office.
The existing parking lot, with 32 spaces, will be more than adequate to handle the
12 staff vehicles and several Caretender's vans.
The sewage generated will be less than the amount generated by a medical office
as described in Crossroads Engineering letter.
The existing water service is 1 1/4"~ copper, which is adequate for 90 fixture
units. This facility has 82 fixture units which is less than the 90 fixture units.

occupancy per table 500 of the 1994 Standard Building Code.
No signage facing Hypoluxo Road shall be installed to conform to the P.UD.
requirements.
Based upon all the above factors, plus Crossroads Engineering report, this Adult
Day Care Center does not impact the P. UD. or surrounding areas more than the Medical
Office Building. Therefore, we are requesting approval of this Site Plan Modification.

office space per doctor, the existing building could accommodate 6 doctors. This results
in 1,500 gallons per day. A day type school is expected to generate 15 gal/day/occupant
and employee plus 5 gal/day/shower. Allowing for one shower and seven toilets in the
facility, this results in 1,235 gallons per day.
As described above, the water demands are lower for the proposed adult day care center than they are for

As requested, we reviewed the sewer and parking needs for the proposed adult day care center in
comparison to the existing medical offices. The existing 6,300 square foot building with 32 parking spaces
on Hypoluxo Road in the Sanford Park PUD (Newport Place) was originally used as a medical office. It
is now proposed that this building be converted to an adult day care center. The adult day care center
expected to serve 70 occupants with 12 staff members. You requested that we examine the traffic, parking
and sewage requirements to determine if the change would result in any needed improvements.
Traffic
According to the Palm Beach County Land Development Code and the Institute of
Transportation Engineers Trip Generation 5th Edition, a medical clinic of this size would
be expected to generate 215 trips per day with 0% of the traffic being pass-by trips. All
clients arriving and departing from this facility shall be transported by seven 12 to 15
passenger vans operated by Care Tenders. Only the staff shall drive their vehicles to the
facility. Therefore, we conservatively estimate the trips that will be generated for the new
facility will be under 100 per day. The transportation furnished by Care Tenders will
reduce the impact of the public roadway system that was previously generated by the
medical office.
Parkin~
A medical office typically requires one parking space per 200 square feet. This results in
31.5 spaces for a medical office which corresponds to the existing parking that was
originally provided. A day care center with a capacity of less than 100 typically requires
5 transient spaces plus one per employee. This results in 17 spaces indicating that the
existing parking facilities will be adequate for the proposed use.

units. This facility has 82 fixture units which is less than the 90 fixture units.

occupancy per table 500 of the 1994 Standard Building Code.
No signage facing Hypoluxo Road shall be installed to conform to the P.U.D.
requirements.
Based upon all the above factors, plus Crossroads Engineering report, this Adult
Day Care Center does not impact the P. U.D. or surrounding areas more than the Medical
Office Building. Therefore, we are requesting approval of this Site Plan Modification.

Mter a closer evaluation of your plan to open an adult day care center
within the Newport Place PUD (4745 NW 7th Ct), some significant
issues have been revealed and should be communicated for your
consideration.
Your location is an existing Planned Unit Development, which is a
zoning district that allows, in part, residential uses, houses of worship,
schools, nursing homes, nursery schools, kindergartens, hospitals, and
commercial uses which are determined to be compatible with the
existing and future development of adjacent and nearby land outside
the PUD. Commercial uses must solely serve the PUD and cannot
have signage which is oriented toward Hypoluxo Road. These were the
same requirments conducted on the original use in this same building,
a medical office building. Since there is a large ACLF within the PUD,
you may have determined that your market is within this immediate
vicinity.
Also, the PUD is reviewed for impacts generated collectively from all
uses within the PUD. With the change of the office use, we must
review for changing impacts. This review would include a traffic
analysis submitted by you as part of the master plan modification.
This whole review must be initiated by a petitioner such as yourself
with a letter explaining in detail the nature of your amendment to the
Newport Place PUD. This submittal would ultimately include the
revised master plan to show your proposed use, and in part, required
and available parking spaces. To provide you further direction on this
process, I have included an excerpt from the zoning code. Please

Section 12.
Plan modification.
Master plans approved during the process of rezoning to PUD may be considered for
modification upon application to the director of planning and zoning, submission of acceptable
documentation, and payment of the master plan modification fee. The modificati':>n will be first
reviewed by the planning and zoning department to determine whether the modification is
consistent with not only all current regulations but also the intent and purpose of the
comprehensive plan; and to determine whether the change is substantial.
Upon completion of the administrative review, findings will be forwarded to the planning
and development board which will review all submitted documents as well as staff comments
with all the authority, functions, powers and duties vested in it by Chapter 1.5, Article I, Section
4 of the Land Development Regulations.
The planning and development board shall recommend to the City Commission that they
approve the modification unconditionally, approve the modification with conditions and/or
recommendations, or deny the modification. The board shall also recommend to the City
Commission that the modification be considered either major or minor.
Upon completion of the planning and development board review, findings will be
forwarded to the City Commission. The City Commission will first determine whether the
modification is major or minor. If the Commission finds the modification to be major, the
request will be returned to the applicant for processing as a new zoning application. The
determination of what constitutes a substantial change shall be at the sole discretion of the
Commission. If the Commission determines that the modification is minor, it will review all
submitted documents as well as staff comments and planning and development board
recommendations. The City Commission may then approve the minor modification
unconditionally, approve the minor modification with conditions, or deny the minor
modification.

Non-substantial (minor) modifications will not extend time limitations for development of
property as stipulated in LDR Chapter 2, Section 9, paragraph C13.
Section 13.
Zoning administrator.
Building permits. No building permit or certificate of occupancy or zoning compliance
shall be issued in or for development in a PUD district except in conformity with all provisions
of the zoning or pun classifications and plans submitted under Section 11 of these zoning
regulations.
