This letter is to inform you that your project has received final sign-off
on February 24, 1992 of the plans submitted for review by the Technical
Review Board. Completing the site Plan Review process enables you to submit
working drawings and construction plans for the site work and building
construction to the Building Department for plan review and permitting. To
initiate this process present to the plans analyst in the Building
Department the following:
a) appropriate and completed permit application form
b) copy of this letter
c) appropriate plans and associated documents
d) a plan filing *permit fee
The plan filing fee is 30% of the estimated permit fee and is made payable
to the City of Boynton Beach and due at time of filing an application for
each permit. The total permit fee is determined by the Building Department,
and is due when the permit is issued.

Upon review of the above mentioned project, the following list of
comments must be addressed in order to conform with Boynton Beach
City codes:
1. Show within the plans a typical detail drawing of a regular,
90 degree and parallel parking space. Drawing must include
the following information:
i. show the size and configuration of the pavement markings
ii. specify the color of the pavement markings (white for
asphalt, black for concrete)
2. Include within the drawings a detailed elevation view
drawing of the proposed site lighting pole and fixture. Show
and specify the following pole and fixture data.
i. specify that the fixture is photocell activated
ii. state that the pole, pole base and fixture is designed
consistent with the windload requirements specified
within Chapter 12 of the standard Building Code, 1988
Edition
iii. specify the type of luminary (metal halide or high or
low pressure sodium)
iv. show the light contours and specify their foot candle
light level
v. specify, where applicable, shields to deter off-site
glare
vi. state that the height of the pole and type of fixture
shown on the plans will be used in creating the
photometric drawing(s)
vii. specify the depth of the pole and/or pole base is
placed below grade
viii. provide a detail drawing of the pole base showing and
specifying the vertical and horizontal structural
materials and components. Including the size and type
of fasteners required to attach the pole to the base
ix. show the distance from the grade to the top of the pole
base
3. The outline, configuration and material shown and identified
on the project drawing(s) submitted with the plan(s) for
final sign-off shall match in every respect the colored
drawing(s) approved by the City Commission during the
approval process of the project. The color of each exterior
finish material shown on the final sign-off plans shall be
identified by name, color number and manufacturers name
which shall match in every respect with the color(s) shown
on the drawing(s) approved by the City Commission. A
manufacturer's color chart showing and identifying all
approved color(s) shall be submitted with the plans for
final sign-off.

4. Specify on the elevation view drawing the color(s) of all
exterior finish material(s).
5. Show on the site plan drawing the handicapped accessible
route leading to the entrance of the building from the
handicapped parking space. Drawing shall include the
following information:
i. specify the elevation of the parking space surface
at each handicapped parking space
ii. show and dimension the width of the five (5) foot
wide access aisle that is required to be parallel
with the length of each handicapped parking space
(one aisle may be placed between and serve two (2)
handicapped parking spaces)
iii. specify and show the following five (5) foot wide aisle
information:
a) slope
b) slope of flared sides
c) type of material
d) texture of finish
e) length
iv. show the required handicapped accessible walkway that
leads to the entrance of the building from the five (5)
foot access aisle. Specify and show the following walkway
information:
a) slope for the entire length
b) texture of the finish
c) total length
d) cross slope
e) width
f) type of material
g) location, height, material and configuration
of handrail, where required
v. show the location, width, length and elevation of the
level platform that is required at the entrance to the
building
6. Show on the plans the location of the two (2) handicapped
parking spaces specified on sheet 1 of 6 key note number 7.
7. A continuous visual barrier is required to screen the
on-site vehicle use area from the property that is abutting
your property to the west. The visual barrier should
continue from the south end of the west buffer wall to the
south property line. Show the location and specify the
following required visual barrier information on the plans:
i. dimension the spacing of each hedge plant material
required to form the continuous visual barrier (maximum
spacing is twenty-four (24) inches on center)
ii. state the specie of each hedge plant material, fifty
(50) percent of all required hedge landscaping shall be
a native specie. show with a distinguishable symbol the
native specie hedge material.
iii. identify the total number of each specie of hedge
landscape material
iv. state the size specifications for all hedge plant
material (minimum height at planting is eighteen [18]
inches)

8. Show unobstructed cross visibility area and space through
the required site perimeter landscape strips at both sides
of all ingress and egress driveways. The cross visibility
area and space is shown by placing and dimensioning on the
site plan a ten foot (10') isosceles triangle. The point
where both ten foot (10') legs join shall be placed on
the property line at the edge of the ingress/egress with
one leg extending along the property line and the. other leg
along the edge of the ingress/egress with the remainder
of the triangle being a line across the landscaped area
connecting the end of each ten foot (10') leg. The
vertical space between thirty inches (30") [above the
adjacent pavement surface] and six feet (6') within the
triangle area shall be clear of landscape material.

In accordance with the City of Boynton Beach, Florida Code of Ordinances,
specifically Chapter 19, Section 19-17, "Plan Required", including Chapter 5,
Article X, Section 5-142 inclusive, "Required Improvements", the applicant for
th~ above referenced project shall submit the following technical data, plan
revisions and information:
1. The applicant shall submit a current survey in accordance with Chapter 19,
Section 19-17(k), "Requested Technical Data". The Engineering Department,
for the City of Boynton Beach, Florida reserves the right to make additional
comments upon receipt of the required survey document.
2. The applicant shall submit a photometric plan depicting levels of parking lot
and pedestrian walkway illumination consistant with the requirements set
forth in Section 5-142(a) , "Required Lighting", Section 5-l42(b), "Pedestrian
Walkway Lighting", including Section 5-l42(g), "Lighting Standards". The
Engineering Department, for the City of Boynton Beach, Florida recommends
that the applicant obtain a copy of Chapter 5, Article X, "Boynton Beach
Parking Lot Regulations" and the associated appendix A & B.
3. In accordance with Section 5-142(c), "Traffic Control", the applicant shall
revise plans to depict internal directional arrows which shall delineate
traffic flow through this facility. Additionally, provide construction
detail sheets depicting details for all traffic control signage, including
traffic control signage foundations and depth of imbedment, etc. All
traffic control devices shall conform with the Federal Highway Administration's
Manual on Uniform Traffic Control Devices, pursuant to Florida Statutes and
the City of Boynton Beach, Florida Code of Ordinances.
4. Provide contruction details for the required raised continuous concrete
curbing in accordance with Section 5-l42(e), "Raised Continuous Concrete
Curbing" Additionally, provide a construction detail for raised continuous
concrete curbing which complies with minimum City of Boynton Beach, Florida
Engineering Department Standards. Raised continuous concrete curbing shall
be a minimum depth of l3~", 8" in width at the base and 6" in width at top
of curb. Section 5-l42(g), "Raised Continuous Concrete Curbing Standards".
5. Address or confirm that drainage system will meet water quality and
3 yr. - 24 hr. criteria.

7. Provide geotechnical test data (approximately three (3) locations) specifying
asphalt and base course thickness determinations. Should the existing in-place
asphalt thicknesses be less than the required one inch per City Code, the
applicant shall be responsible to bring the asphalt surface into compliance
regarding required thicknesses. Should the base course thickness not meet
with the required minimum 6 inches of base course, the applicant shall be
required to augment the base course in order to bring the minimum base course
thickness into compliance with our City's Code of Ordinances. Section 4-l42(g),
lIParking Lot Construction Standards".
8. In accordance with Section 5-l42(h), "Driveway", the applicant shall provide
parking lot driveway radiuses which shall be constructed with a radius at the
intersection of the vehicular traffic lane of 25 ft. or as, otherwise approved by
the City Engineer.
9. The swale area within the public rights-of-way of SW 25th Avenue, shall be
fully developed into a swale which complies with minimum City of Boynton Beach
Engineering Department Standards. Grass swales shall be fully irrigated and
maintained in accordance with applicable Landscape Codes.
10. The submitted handicap stall, as depicted upon site plan sheet SP-l, does not
comply with minimum City Standards. Submit a handicap parking stall detail
which complies with the Department of Community Affairs, latest edition of the
Accessibility Requirements Manual, a copy of which is available in the
Building Department. Section 5-l42(k),"Handicap Requirements".
11. Provide a parking lot striping detail for standard and handicap parking stalls.
None was submitted by the applicant. Section 5-142(1), lIParking Lot Striping".
12. Identify what grates at NE and SE corner of building are for.

Dear Dr. Hopkins:
As previously discussed with Mrs. Joyce Hopkins, the Planning
Department will need a unity of title document unifying lots 16,
17, 18, and 19 (less the north 41 feet) of the Westchester
Heights subdivision in order to sign-off on the Hopkins Office
Building application for a site plan modification (File 653).
The unity of title document, however, does not preclude you from
selling one or more of the lots in the future, as long as a site
plan modification is submitted showing how each separate site
conforms with the Code of Ordinances. For example, if one or two
of the lots are divided and sold to a different owner, those lots
would need to show the required parking, landscaping, dumpster,
etc.

Following the T.R.B. meeting held today, I visited the site to
compare the plans with the existing parking lot design. I found
that the only usable stop sign is the one at the exit onto S.W.
25th Avenue. The remaining three must be replaced due to wear.
Additionally, the stop sign positioned on the driver's left when
travelling south from the canopy area must be relocated to the
driver's right, or west side of that driveway, and positioned at
required height.
The plans show only three stop signs while the site actually
contains four. The one not on the plans includes a stop bar and
is located opposite the dumpster area. It is designed to stop
north-bound traffic travelling thru the lot before they turn
right to exit onto Seacrest or left to go around the back of the
building. These control devices must remain.
The pavement contains several sets of directional arrows,
although they are faded, that are not on the plans. These
should all be repainted and as a result, traffic flow should be
enhanced.

19. A sealed survey, by a surveyor registered in the state
Florida, and not older than six (6) months, showing
property lines including bearings and dimensions; north
arrow, date, scale, existing structures and paving,
existing elevations on site, rights-of-way and easement
on or adjacent to the site, utilities on or nearest
one-hundredth (1/100) of an acre, location sketch, and
surveyor's certification. Also, sizes and locations of
existing trees and shrubs, including common and botanic
names, and indication as to which are to be retained,
removed, relocated, or replaced.

more net trips per day, the traffic impact analysis
must be submitted to the City at least 60 days prior
to the deadline for site plan approval~ in order to
allow for timely processing of the site plan
application and review by the City'S traffic
consultant and Palm Beach County. The applicant
shall be billed for the cost of review by the City's
traffic consultant. ,. ..
b. For projects that generate between five hundred
(500) and two thousand (2,000) net trips per day,
the traffic impact analysis must be submitted at
least 30 days prior to the deadline for site plan
approval, in order to allow for timely processing of
the site plan application and review by Palm Beach
County. However, if it is the desire of the
applicant to utilize the City's traffic consultant
for review of the traffic impact analysis prior to
review by Palm Beach County, then the procedure and
requirements outlined under item "a" above shall be
followed.
NOTE: Failure to submit traffic impact analysis in the manner
prescribed above may delay approval of the site plan
application.

29. For projects which require review by the Planning and
Zoning Board, the following items, in addition to the
above requirements, shall be submitted to the Planning
Department no later than the site plan deadline:
a. One copy of colored elevations for all buildings c
signage to be constructed on site. These elevati(
must be of all sides of each type of building and
signage proposed and the colors proposed must be
accompanied by a numerical code from an establishE
chart of colors. Elevations must also include
information related to building materials. All
elevations must be submitted on 24" x 36" drawings
Buildings constructed will be inspected on the bas
of the elevations submitted to the City and appro~
by the City Commission. Failure to construct
buildings consistent with elevations submitted wil
result in the Certificate of Occupancy being
withheld.
b. An 8-1/2" x 11" transparency of the site plan
drawing (reduced) to be used on an overhead
projector at board meetings. No application
will be accepted without a transparency.
c. Colored photographs of surrounding buildings
(minimum size 8" x 10").
30. Any other engineering and/or technical data, as may
be required by the Technical Review Board to
determine compliance with the provisions of the
City's Code of Ordinances. .
NOTE: Any of the above requirements may be waived by the Technical
Review Board, if such information is deemed to be non-essential
by the Board.

RIDER TO SITE PLAN APPLICATION
The undersigned as applicant for Final Site Plan Approval does
hereby acknowledge, represent and agree that all plans,
specifications, drawings, engineering, and other data
submitted with this application for review by the City of
Boynton Beach shall be reviewed by the various boards,
commissions, staff personnel and other parties designated,
appointed or employed by the City of Boynton Beach, and any
such party reviewing the same shall rely upon the accuracy
thereof, and any change in any item submitted shall be deemed
material and substantial.
The undersigned hereby agrees that all aspects of the proposed
project shall be constructed in strict compliance with the form ir
which they are approved, and any change to the same shall be deemE
material and shall place the applicant in violation of this
