SITE PLAN - LEON SURLES CARPET WHOLESALE AND DISTRIBUTION
Upon review of the above mentioned project, the following list of
comments must be addressed in order to conform with Boynton Beach
City Codes:
1. Identify on the site plan the approved elevation of the
finish floor with reference to the mean sea level.
2. Provide a copy of a current survey.
3. Specify on the plans the following address that has been
assigned to the site: 515 N.E. 3rd Street.
4. show on the site plan the following loading zone
information:
i. location (not obstructing traffic flow or parking
spaces)
ii. size (loading zone 12' wide by 35' long and 14'
clear unobstructed height)
iii. method of identification (pavement markings and/or
signs)
5. Provide drawings of the building and site signage complying
with the requirements of the sign code.
6. Specify on the plan the method of holding the dumpster
enclosure gates in the open and closed position.
7. Include within the drawings a detailed elevation view
drawing of the proposed site lighting pole and fixture. Show
and specify the following pole and fixture data.

8. Show and identify on the site plan drawing, the location of
the buffer wall two feet (2') in from the north and south
property lines.
9. Show and state on the site plan the required number of
parking spaces consistent with the parking ratios specified
within Section 11 H 16 f (5) of Appendix-A Zoning. To
accommodate the proposed use, I recommend that after you
re-layout your site to comply with the Technical Review
Board comments, you provide as many as possible of the
additional parking spaces that are shown on the preliminary
site plan.
10. Specify on the elevation view drawing the color of the
overhead doors.
11. Show on the site plan a handicapped accessible walkway
leading to the entrance of the building from the public
sidewalk. Specify on the plans the following walkway data:
i. type of material
ii. texture of the finish
iii. width
iv. slope for the entire length
12. Identify the height of the handicapped parking space signage
(7' from grade to bottom of sign is required). Specify the
elevation of the parking surface at the handicapped parking
space and state that the slope of the parking surface
leading to the sidewalk entrance to the building is not
greater than 1:20. show the location, width, length and
elevation of the level platform that is required at the
entrance to the building.
13. State the size specifications for all the tree landscape
material. (Minimum height is eight [8] feet)
14. Specify on the plans that the quality of all the landscape
material will be Florida #1 or better.
15. State on the plans that the landscape material will be
irrigated with an automatic water supply system. Specify the
water supply source.
16. Specify the type and delineate the location and boundary of
the sod for the entire site.
17. The outline, configuration and material shown and identified
on the project drawing(s) submitted with the plan(s) for
final sign-off shall match in every respect the colored
drawing(s) approved by the City Commission during the
approval process of the project. The color of each exterior
finish material shown on the final sign-off plans shall be
identified by name, color number and manufacturer's name
which shall match in every respect with the color(s) show on
the drawing(s) approved by the City Commission. A
manufacturer's color chart showing and identifying all
approved colors(s) shall be submitted with the plans for
final sign-off.
18. Show unobstructed cross visibility area and space through
the required site perimeter landscape strips at both sides
of all ingress and egress driveways. The cross visibility
area and space is shown by placing and dimensioning on the
site plan a ten foot (10') isosceles triangle. The point
where both ten foot (10') legs join shall be placed on the
property line at the edge of the ingress/egress with one leg
extending along the property line and the other leg along

the edge of the ingress/egress. The remainder of the
triangle is defined by placing a line across the landscaped
area connecting the end of each ten foot (10') leg. The
vertical space between thirty inches (30") [above the
adjacent pavement surface] and six feet (6') within the
triangle area shall be clear of landscape material.
19. Show unobstructed cross visibility area and space at the
intersection of two (2) or more public right-of-ways. The
cross visibility area and space is shown by placing and
dimensioning on the site landscape plan a thirty-five (35)
foot isosceles triangle. The point where the two (2) equal
sides connect shall be placed where the ultimate width of
the right-of-ways intersect each other. Both equal sides
shall extend from the intersecting point thirty-five (35)
feet along each right-of-way. The third side of the triangle
shall be a line that connects the end of each thirty-five
(35) foot side. The vertical space between thirty (30)
inches [above the adjacent pavement surface] and six (6)
feet within the triangle area shall be clear of landscape
material.
20. Specify type and location of mulch material to be used and
it's thickness when laid.
21. Specify and show on the plan the location, size, spacing and
specie of landscape material required along the outside of
the vehicle use area visual barrier/buffer wall located
along the west property line.
22. Show on the landscape plan the computations used to provide
the required 50% native specie count for the total number of
trees, hedge, ground cover and vine landscape material
specified on the landscape plan. Identify the native specie
plant material by providing a distinguishable symbol by each
native plant specified on the plan.

CARPET WHOLESALE SALES AND DISTRIBUTION FACILITY FOR LEON SURLES
In accordance with the City of Boynton Beach, Florida, Code of Ordinances,
specifically Chapter 19, Section 19-17, "Plan Required", including Chapter 5,
Article X, "Boynton Beach Parking Lot Regulations", the applicant for the above
referenced project shall submit the following technical data, plan revisions
and information:
1. Submit a site lighting plan with associated photometrics in accordance with
Section 5-l42(a), "Required Lighting" including plan notations which indicate
photocell activation and a certification for 110 MPH lateral wind loading.
All parking facility areas, including ingress/egress approaches, aisles and
pedestrian sidewalks shall be illuminated in accordance with City standards.

curb standards in accordance with Section 5-l42(g), "Parking Lot Construction",
"Curbing Standards and Specifications".
5. In accordance with Section 5-l42(f), "Drainage", the applicant shall submit
a stormwater drainage plan to be accompanied by associated percolation test
data, percolation tests location plans and drainage calculations. The
Engineering Department, for the City of Boynton Beach, Florida, reserves the
right to provide additional comments upon receipt of the requested information.
6. The applicant shall place a note on the submitted plans indicating that the
existing five foot concrete sidewalk, where broken and/or avulsed shall be
removed and repaired with new concrete sections. Repair potholes and damaged
(*) edge of pavement. Concrete sidewalk areas thru the ingress/egress approaches,
shall be removed and reconstructed in accordance with Section 5-l42(g),
"Sidewalk Standards", wherein the sidewalks thru these areas shall be a
minimum thickness of 6 inches. The sidewalks thru the ingress/egress approaches
shall be so transitioned as to facilitate handicap circulation thru these areas.
7. In accordance with the City of Boynton Beach, Florida, Code of Ordinances,
all drainage areas shall be fully sodded in order to abate erosion in these
areas. Delete the alternate proposal for seeding the drainage areas.
Section 7.5-35, "Particular Requirements", Subsection 7.5-35(c)(6),--"Lawn
Grass" which states the following, "Solid sod or other acceptable erosion
control measures shall be used in swales or other areas subject to erosion".

8. Placea note on plans indicating that the paved area at the rear of the one
story CBS building will not be utilized for off street parking or storage
of vehicles and/or materials.
9. The handicap parking stall in accordance with DCA Accessibility Requirements
Manual shall be placed parallel to and facing the traffic aisle. Provide a
standard detail for handicap signs which shall be placed a minimum of 7 ft.
above grade to the bottom of the sign, fully grouted in place in order to
minimize dangers occasioning hurricanes. Section 5-l42(k), "Handicap
Requirements".
10. Provide construction details for all parking lot striping in accordance
with Section 5-142(1), "Parking Lot Striping".
11. Swales within the public rights-of-way of NE 5th Avenue and NE 3rd Street
shall be fully sodded and irrigated. All swales shall conform to the
specifications set forth within City standard drawings for swale construction.
Sodded areas for the drainage reserve and open drainage areas shall be fully
irrigated. Section 7.5-35, "Particular Requirements", Subsection 7.5-35(b),
"Maintenance" which states "All landscaped areas shall be provided with an
automatic irrigation water supply system".

We can approve this project, subject to the following
conditions.
1. Show ,all utility easements.
2. Indicate locations of fire hydrants, water meters,
and sanitary services on site plans. Location of
water meters in paved areas is not desirable.
3. only those tree species approved by the Utility
Department may be placed in utility easements.
4. Fire hydrants must be installed to within 200' of
all points of the building.
5. Double check valve assembly will be required on the
fire service line if fire protection is required.
6. Water services with either end under pavement are
to be of Type K copper.
7. Irrigation is not to be supplied by city water.
8. Provide 6" cleanouts on the sanitary services at
the property line.
9. Sewer cleanouts are required at a maximum spacing
of 75' on private sanitary sewer services.
10. Work to be in accordance with City of Boynton Beach
Details and Criteria sheets.
11. PVC sleeve is required on water services running
under pavements.
12. Owner shall provide fire flow requirements and
details of fire protection system.
13. Palm Beach County Health Department permits may be
required depending on fire protection requirements.
Some of these comments may not apply to the final utility
plan. We reserve the right to revisit this review upon
submittal of the final utilities plan.

Building Department, please provide this department a memorandum
containing a description of the unresolved conditions. If the
unresolved conditions are not permit related, please notify the
owner or the owner's agent to ensure that the matters are rectified.

within ten (10) days in order that we may proceed in a timely manner

SITE PLAN
1. Leon Surles Carpet Wholesale Facility: (New wholesale
warehouse)
One (1) colored elevation. City Commission approved,
September 19, 1991, subject to staff comments and C.A.B.
recommendation that 6 Queen Sago Palms along the east and
west sides of the building be substituted with paurotis Riri
of the same size.
CBD SIGN APPROVALS
2. Bank Boynton: (Wall sign)
One (1) colored elevation, color chips and photographs of
surrounding buildings. C.R.A. approved subject to staff
comments; colors approved as submitted.
3. precision Tune: (Wall sign and sign face change to existing
off-premise freestanding sign)
One (1) colored elevation. C.R.A. approved subject to staff
comments with the exception that the colors of both sign
backgrounds be changed from yellow to white, maximum capital
letter height be limited to 14 inches and the height of the
freestanding sign be reduced to 20 feet. (The legality of
C.R.A.'s decision on the freestanding sign is under debate
with the City Attorney and Planning Department and will be
resolved with the applicant in the near future.)

SUBJECT: Planning & Zoning Board Meeting - September 10, 1991
Please be advised that the Planning and Zoning Board met on
Tuesday, September 10, 1991, and took the following action:
1. Deleted the SeaMist Centre and Marina conditional use
request from the agenda. Further processing of this request
cannot occur until the applicant resubmits plans to the
Technical Review Board. Although advertised as such, a
public hearing before the Planning and Zoning Board is not
required by code. (PM91-221)
2. After conducting a pUblic hearing, unanimously approved the
request for a parking lot variance to Section 5-142(h)(7)
"Driveway" of Article X, Parking Lots, for Abita Springs
Water Co. Inc., located at 3030 S.W. 13th Place, sUbject to
proviso. The motion was made by Mr. Collins and seconded by
Mr. Cwynar.
3. The Board accepted the property owner's agent's request to
withdraw, from the agenda, the site plan approval for Best
Homes at Boynton Lakes, Plat 5, for the construction of 41
townhouse units. After the issue regarding the requirement
to construct various improvements on common areas is
resolved, this request will be placed on a future Planning
and Zoning Board agenda.
4. Unanimously recommended approval of the request submitted by
Leon o. Surles for site plan approval to construct a 4,000
square foot carpet wholesale distribution facility on .77
acres, subject to proviso. Motion was made by Mrs. Huckle
and seconded by Mr. Collins. (PM91-222)
5. Unanimously recommended approval of the request submitted by
Mega Mini for Space Plus for approval of an amended site
plan to construct an outdoor rental storage lot, subject to
proviso. Motion' was made by Mrs. Huckle and seconded by Mr.
Collins. In addition, the Board requested the Planning
Department to notify the residential property owners
abutting the subject parcel to the north, prior to the
September 19, 1991 City Commission meeting. (PM91-223)
6. The Board removed from the table and unanimously made a
finding of consistency with the Comprehensive Plan for the
proposed amendment to Appendix A-Zoning, Section 11.1 of the
Code of Ordinances to allow for expansions of nonconforming
structures used for conforming uses, in the Central Business
District, subject to proviso. Motion was made by Mrs.
Huckle and seconded by Mrs. Greenhouse. (PM91-224)
7. Make a finding of consistency with the Comprehensive Plan
for the proposed amendment to Appendix B-P1anned Unit
Developments of the Code of Ordinances, Section 6.F-Planned
Commercial District and Section 7-Planned Industrial
District of Appendix A-Zoning of the Code of Ordinances to
create time extensions for master plan approvals and specify
that a master plan modification shall not extend the
expiration of a master plan approval. Motion was made by
Mrs. Huckle and seconded by Mr. CWynar. The vote was 4-2
with Mr. Rosenstock and Mrs. Greenhouse dissenting; Mr.
Collins had left the meeting. (PM91-225)

8. Deleted the consistency review determination of the proposed
Administrative Procedures Ordinance from the agenda since
this ordinance has not been completed by staff. This item
will be placed on a future Planning and Zoning Board agenda.
9. Made a finding of consistency with the Comprehensive Plan
for the proposed amendment to the zoning code to allow
towing companies in the M-1, Light Industrial, zoning
district. Motion was made by Mrs. Huckle and seconded by
Mr. Cwynar. The vote was 4-2 with Mr. Rosenstock and Mrs.
Greenhouse dissenting; Mr. Collins had left the meeting.

Please place the referenced item on the City Commission agenda
for Thursday, September 19, 1991, under Development Plans,
Consent Agenda.
DESCRIPTION: Request submitted by Leon o. Surles for site plan
approval to construct a 4,000 square foot carpet wholesale
distribution facility on .77 acres. The project is located at
the southwest corner of N.E. 3rd Street and N.E. 5th Avenue.
RECOMMENDATION: The Planning and Zoning Board unanimously
recommended approval of this request subject to staff comments,
copies of which are attached. In addition, the Planning and
Zoning Board asked staff to coordinate with the applicant and the
property owner to the north, initiation of an abandonment
application for N.E. 5th Avenue, which abuts the subject property
along the north property line.
