This letter is to inform you that your project has received final sign-off
on January 15, 1992 of the plans submitted for review by the Technical
Review Board. Completing the Site Plan Review process enables you to submit
working drawings and construction plans for the site work and building
construction to the Building Department for plan review and permitting. To
initiate this process present to the plans analyst in the Building
Department the following:
a) appropriate and completed permit application form
b) copy of this letter
c) appropriate plans and associated documents
d) a plan filing *permit fee
The plan filing fee is 30% of the estimated permit fee and is made payable
to the City of Boynton Beach and due at time of filing an application for
each permit. The total permit fee is determined by the Building Department,
and is due when the permit is issued.

For information regarding permits related to the site work contact the Site

November 12, 1991
Please be advised that the Planning and Zoning Board met on
Tuesday, November 12, 1991 and took the following action:
I. Continued the public hearing for the Cross Creek Centre
parking lot variance to the December 10, 1991 meeting at
7:00 p.m., as per the applicant's request for another 30 day
postponement.
2. After conducting a public hearing, recommended approval of
Kilday & Associates request for Tradewinds Development
Corporation (Boynton Beach Boulevard rezoning) to amend the
Future Land Use Element of the Comprehensive Plan from
"Moderate Density Residential" to "Office Commercial".
Motion was made by Mr. Collins and seconded by Mrs. Huckle.
The vote was 4-2 with Mr. Lehnertz and Mrs. Stevens
dissenting.
Also, recommended approval to rezone from RlAA/PUD
(Single-family Residential) to C-I (Office and Professional
Commercial), subject to provisos, which are necessary for
consistency of the proposed rezoning of the property with
the Comprehensive Plan. Motion was made by Mr. Collins and
seconded by Mrs. Huckle. The vote was 4-2 with Mr. Lehnertz
and Mrs. Stevens dissenting. (PM91-287)
3. Approved the request submitted by William Turner for the
Lakeshore/Heatherlake Homeowners' Association to amend the
Meadows, Tract "L", Heatherlake master plan to reduce the
rear setback for hard-roofed screen enclosures, from 25 to
15 feet, sUbject to proviso. Vote was 5-1 with Mr. Lehnertz
dissenting.
4. Unanimously recommended approval of the request for an
amended site plan for Bethesda Memorial Hospital to
construct a parking lot expansion, subject to proviso. The
motion was made by Mr. Collins and seconded by Mrs. Huckle.
Mrs. Stevens and Mr. Rosenstock left the room prior to the
motion being made. (PM91-288).
5, Unanimously recommended approval of the request for an
amended site plan for Motorola P.I.D. to construct phase III
of the P.I.D. to include 83,564 square feet of additional
office place, changes to the existing parking lot layout and
design and the addition of 112 parking spaces, subject to
proviso. The motion was made by Mrs. Huckle and seconded by
Mr. Collins. (PM91-289)

6. Tabled the determination of consistency with the
Comprehensive Plan for the proposed amendment to Appendix
A-Zoning, to amend the regulations governing service
stations and convenience stores until further analysis,
including mapping of all existing service station locations,
potential locations under the existing code and potential
locations under the proposed amendment and evaluation of the
existing distance separation against current safety
standards, is prepared by the Planning Department and
presented to the Planning and Zoning Board at the December
meeting.
7. Unanimously made a finding of consistency with the
Comprehensive Plan for the proposed amendment to Appendix
A-Zoning, to include limitations on restaurants located in
the C-I zoning district, as conditional uses, subject to
proviso. Motion was made by Mr. Lehnertz and seconded by
Mrs. Huckle. First reading of the ordinance for this
amendment is proposed for the December 3, 1991 City
Commission meeting.
8. At the request of the City Commission, revisited the issue
of towing companies as a permitted use in the M-I zoning
district and unanimously decided not to amend the Planning
and Zoning Board's September 10, 1991 finding of consistency
with the Comprehensive Plan for the proposed amendment to
Appendix A-Zoning, to allow towing companies in the M-I

In accordance with the provisions set forth in the City of Boynton
Beach, Florida. Code of Ordinances, specifically Chapter 19-17,
"Plan Required" and Chapter 5, Article X, "City of Boynton Beach
Parking Lot Regulations", the applicant, in connection with a
re~ue5t far a permit to construct a park~ng lot. shall subm~t an
application and plan in accordance with Chapter 19, section 19-17,
which shall include, but not be limited to, information, materials
or any other engineering or technical data, as may be required by
the technical review board to determine compliance with the
prov~sions of this article and the standards referred to herein.
Requirements are as follows:

Upon review of the above mentioned project, the following list of
comments must be addressed in order to conform with Boynton Beach
City Codes:
1. Show on the landscape plan the location, size and specie of
the trees that are required to be spaced a maximum of forty
(40) feet on center along s.w. 23rd Avenue (Golf Road).
Include said trees within the plan list on Sheet L-1. The
size specifications must comply with the minimum
requirements identified within the Landscape Code. Fifty
(50) percent of the required trees shall be a native specie.
Identify the native specie by placing a distinguishable
symbol by each native specie and list the total number of
trees required and provided along S.W. 23rd Avenue.
2. Show on the plans the ratios and figures used to determine
the total number of required parking spaces for the entire
site. The ratios must be consistent with Section 11 H of
Appendix A-Zoning. Identify the total number of spaces
required and the total number provided.

Gentlemen:
Our client. Bethesda Memorial Hospital, is requesting that the requirement to install
planting islands every ten parking spaces be waived for the parking lot expansion currently
under review by the City of Boynton Beach. The parking lot is intended for employee use, and
the loss of approximately eight (8) parking spaces caused by construction of these planting
islands would create a hardship on the hospital in its effort to alleviate congestion during shift
changes. Also. the addition of those planting islands, would be inconsistent with the
remainder of the parking lot at the hospital.

Dear Mr. Bardt:
This letter concerns the results of our recent exfiltration
test performed at the above referenced site. Verbal auth-
orization for this work was received from you on April 10,
1984.
The test was performed on April 10 at the location you ver-
bafly indicated to us. Results of the test and pertinent
test data are presented on the attached sheet. The water
table was not encountered within the six (6) foot depth of
the test hole.

the site plan application and review by Palm Beach
County. However, if it is the desire of the
applicant to utilize the City's traffic consultant
for review of the traffic impact analysis prior to
review by Palm Beach County, then the procedure and
requirements outlined under item "a" above shall be
followed.
NOTE: Failure to submit traffic impact analysis in the manner
prescribed above may delay approval of the site plan

a. One copy of colored elevations for all buildings and
signage to be constructed on site. These elevations
must be of all sides of each type of building and
signage proposed and the colors proposed must be
accompanied by a numerical code from an established
chart of colors. Elevations must also include
information related to building materials. All
elevations must be submitted on 24" x 36" drawings.
Buildings constructed will be inspected on the basis
of the elevations submitted to the City and approved
by the City Commission. Failure to construct
buildings consistent with elevations submitted will
result in the Certificate of occupancy being
withheld.
b. An 8-1/2" x 11" transparency of the site plan
drawing (reduced) to be used on an overhead
projector at board meetings. No application
will be accepted without a transparency.
c. Colored photographs of surrounding buildings
(minimum size 8" x 10").
