1. A Conceptual Water & Wastewater Plan was received on Wednesday,
September 6th. The General Comments noted below are continued; Specific
Conditions following those comments address the submitted olano
GENERAL COMMENTS
2. All utility easements shall be shown on the rectified site plan and landscaping
drawings so that we may determine which trees may interfere with utilities.
In general, palm trees will be the only tree species allowed within utility
easements. Canopy trees may be planted outside of the easement so that roots
and branches will not impact those utilities within the easement in the
foreseeable future, LDR Sec. 7.5-18.1 give public utilities the authority to
remove any trees that interfere with utility services, either in utility easements
or public rights-of-way,
3. Palm Beach County Health Department permits may be required for the
water and sewer systems servin!! this project (CODE Sec. 26-12).
4. Fire flow calculations will be required demonstrating the City Code
requirement of 1,500 g.p.m. as stated in LDR Chap. 6, Art. IV, Sec. 16, or
the requirement imposed by insurance underwriters, whichever is greater (see
CODE Sec. 26-16(a)).
5. CODE Sec. 26-34(E) requires that a capacity reservation fee be paid for this
project either upon the request for the Department's signature on the Health
Department application forms or within 30 days of site plan approval,
whichever occurs first. This fee will be determined based upon final meter
size, or expected demand.
6. Comprehensive Plan Policy 3.C.3.4 requires the conservation of potable
water. City water may not, therefore, be used for irrigation where other

sources are readily available.
7. Provide an engineer's written certification that drainage will conform to all
rules of the City and the South Florida Water Management District
(SFWMD) (per LDR Chap. 4, Sec. 7F).
8. Water and sewer lines to be owned and operated by the City shall be included
within utility easements. Indicate the proposed easements on the engineering
drawings, using a minimum width of 12 feet (not 10 feet),as required in the
Utility Design Handbook, Article One - Required improvements Division,
Sec. III.EA. thru 6. The easements shall be dedicated via separate instrument
to the City as stated in CODE Sec. 26-33(a).
9. LDR Chap. 6, Art. IV, Sec. 16 requires that all points on each building will
be within 200 feet of an existing or proposed fir hydrant. Please demonstrate
that the plan meets this condition, by showing all hydrants,
10. PVC material not permitted on the City's water system. All lines shall be
DIP.
11. Appropriate backflow preventers will be required on the domestic water
service to the building, and the fire sprinkler line if there is one, in accordance
with CODE Sec. 26-207.
12. A building permit for this project shall not be issued until this Department has
approved the plans for the water and/or sewer improvements required to
service this project, in accordance with CODE Sec. 26-15
13. LDR Chap. 3, Art IV, Sec. 3(P) requires a statement be included that all other
utilities are available and will be provided by appropriate agencies. This
statement is lacking on the submitted plans.
SPECIFIC CONDITIONS
14, The gravity slope of the wastewater system shall be at minimum slope of
0044%, as stated in the Utility Design Handbook, Article Three - Wastewater
System, Sec. VI.A.3. This can be accomplished on the detailed design plans
to be submitted for permitting purposes.
15. The waste stream from the medical building or the drugstore must meet the
city's pre-treatment requirements. A sampling point must be provide to allow
monitoring of this waste stream (refer to CODE Sec. 26-143 and Sec. 26-
147).
16. The proposed jack & bore easing shall be 18" diameter instead of the 16"
diameter shown as called for in the Utility Design Handbook, Standard detail
G-9.
17. The jack & bore designated area on Congress Avenue is shown short of the
full right-of way width. If this is the design intention, it will be required on

the detail construction plans to reflect the construction method to be used, the I
Maintenance of Traffic (MOT) plan, the required testing method in a
congested area as well as all of the casing details.
18. This office will not require surety for installation of the water and sewer
utilities, on condition that the system be fully completed, and given to the
City Utilities Department before the first permanent water meter is set. Note
that setting of a permanent water meter is a prerequisite to obtain the
Certificate of Occupancy.
FIRE
Comments:
19. A fire hydrant flow test is required - LDR 6-16, A fire lane will be required
on south side of retail drug store - BBCC 9-21.
20. Access roads and fire hydrants must be in place prior to above grade
construction - NFP A 1.
POLICE
Comments:
21. Need to review site plan once photometric study has been completed.
ENGINEERING DIVISION
Comments:
22. All plans submitted shall be signed and sealed by the appropriate design
professional.
23. Add the following notes to the Site Plan and Paving and Drainage Plan and
correct all details accordingly.
a. All striping and signage shall conform to City Standard Drawings B-
98001 and B-980013.
b. The drainage system shall conform to City Standard Drawing B-91 007 and
the Utilities Engineering Design Handbook and Construction Standards.
24. Show the sidewalks having a minimum concrete thickness of 6 inches at all
driveways. [LDR Chapter 23, Article n. P.]
25. At the end of the 5 foot handicap access aisles, show 6 foot length of
sidewalk at the same grade with accessible ramps up to the sidewalk. Curb
ramps must be located outside of access aisles per Section 4.7 of the Florida
Accessibility Code.

26. On the Site and Landscape plan, use FOOT Index 546 criteria for the sight
triangle at the Congress A venue driveway.
27. Specify FOOT "0" curb for all 6-inch concrete curb.
28. Add a" $250 Fine" plate to the bottom of the handicap sign on the Site Plan,
Also, specify that the bottom of the handicap sign shall be located 7 feet
above sidewalk. [City Standard Drawing B-98001]
29. Pollution retardant baffles shall be required for all drainage structures that
outfall to exfiltration trenches. Drainage structures must be large enough to
accommodate the baffles and allow silt removal.
30. The loading area may be reduced to a 12' feet x 35 feet space.
31. As an alternate, the paved area between the wheel stops and the end of the
parking spaces may be landscaped in addition to the required landscaping.
[LOR Chapter 7.5, Article II, Section 3 B 4.]
32. As an option, sidewalk along the building side(s) may be widened an
additional two feet to serve as a continuous wheel stop in lieu of individual
wheel stops.
33. It is recommended to reduce the two - 12 foot egress lanes onto Golf Road to
11 feet widths and make the ingress driveway 14 feet wide to facilitate safer,
quicker transition off from Golf Road.
34. The width of the east/west aisleways along the north side of the buildings
bottlenecks from 27 feet to 19 feet. To provide a smooth, safe transition
between the two aisleways, the following is recommended for your
considerati on:
a. Relocate the curbing along the detention area so that it is in alignment
with the 27 foot aisleway north of the office building. Transition the
curb back to its proposed location within the radial portion. This will
guide westbound traffic thru the site.
b. Show a 6 inch white, thermoplastic stripe beginning at the NW corner
of the pharmacy with a radial line in alignment with the back of the
parking spaces abutting the office building. Include at least one set of
directional arrows within this area.
35. Since two way traffic between the buildings can be a minimum of 20 feet and
since the proposed bank "by-pass" lane creates a traffic conflict, the following
is strongly recommended:
a. Close the opening by making it part of the median separator. Move the
entire median five feet to the east. This provides a 22-foot, two lane
accessway between the buildings and an 11 foot by-pass lane, within the
bank drive-in area. Please note! If these recommendations are acceptable,
then the median separator and the two stop bars can be extended north to
the white thermoplastic guideline mentioned above. This relocation will

provide safer traffic movements into the east/west aisle.
36. It is recommended that the applicant delete the 5 foot cross-hatched area and
related curb ramp at the SE comer of the drug store and add 2.5 feet to each
of the terminal islands.
37. Apply FDOT Index 546 for the sight triangles for the Congress Avenue
driveway.
38. Locate the stop/bar sign at the Golf Road driveway 4 feet back of sidewalk or
at least to the property line.
39. Consider deleting the stop/bar sign for eastbound traffic just south of the
drive thru lanes.
40. Verify that PBC will allow a stormwater tie-in to their system from your
control structure. Provide a copy of the PBCED RIW permit.
41. Sod and irrigate those portions of the site designated as "phase two".
BUILDING DIVISION
Comments:
42. Add to the buildings that are depicted on the drawing titled site plan a labeled
symbol that identifies the location of the handicap accessible entrance door to
each building.
43. On the floor plan drawings, add a labeled symbol that identifies the location
ofthe handicap accessible entrance door to each building. The location of the
doors shall match the location of the accessible entrance doors that are
depicted on the site plan drawing.
44. Add a labeled symbol to the site plan drawing that represents and delineates
the path of travel for the accessible route that is required between the
accessible parking spaces and the accessible entrance doors to each building.
The symbol shall start at the accessible parking spaces and terminate at the
accessible entrance doors to the building. The symbol shall represent the
location of the path of travel, not the location of the detectable warning or
other pavement markings that are required to be installed along the path.
Identify on the site plan the width of the east accessible route to the east of the
building. (Note: The minimum width required by the code is forty-four (44)
inches). Add text to the drawing that would indicate that the symbol
represents the accessible route and the route is designed in compliance with
Section 4.3 (Accessible Route) and 4.6 (Parking and Passenger Loading
Zones) of the Florida Accessibility Code for Building Construction. Please
note that at time of permit review, the applicant shall provide detailed
documentation on the plans that will verify that the accessible route is in
compliance with the regulations specified in the Florida Accessibility Code
for Building Construction. This documentation shall include, but not be

limited to, providing finish grade elevations along the path of travel.
45. The flood zone information identified within the site data does not match the
flood zone information listed on the survey. Correct the plans accordingly.
46. Identify within the site data the finish floor elevation (lowest floor elevation)
that is proposed for the buildings. Verify that the proposed elevation is in
compliance with regulations of the code by adding specifications to the site
data that address the following issues [Section 3107.1.2, Chapter 31 of the
Boynton Beach amendments to the 1997 edition of the Standard Building
Code ]:
a) The design professional-of-record for the project shall add the
following text to the site data: "The proposed finish floor elevation
_'_ NGVD is above the highest lOa-year base flood elevation
applicable to the building site, as determined by the South Florida
Water Management District's surface water management construction
development regulations. "
b) From the FIRM map, identify in the site data the title of the flood zone
that the building is located within. Where applicable, specify the base
flood elevation. If there is no base flood elevation, indicate that on the
plans.
c) Identify the floor elevation that the design professional has established
for the buildings within the footprint of each building that is shown on
the drawings titled site plan, floor plan and paving/drainage (civil
plans).
47. On the drawing titled site plan, identify and label the symbol that represents
the property line.
48. The building plans are not being reviewed for compliance with the applicable
building codes. Therefore, add the words "Floor plan layout is conceptual"
below both floor plan drawings found on sheets D 1 and A2.0 1.
49. On the elevation view drawing of the medical office building, dimension the
overall height of the building. The overall height of a building shall be
measured from finish floor to the highest point of the roof or parapet wall.
The dimension shown on the plan does not show the dimension to the highest
point of the building. The overall height of the building shall not exceed the
height limitations of the Zoning Code. Correct the plans accordingly.
50. Add to the site plan drawing all proposed easements. The location, type and
size of the easement(s) shall be shown and identified on the site plan. Where
applicable, amend the plans so that structures do not encroach into an
easement.
51. On all floor plan drawings, indicate within the footprint of the building the
number of stories that are in the building including, where applicable,
mezzamnes.
52. Indicate on the site plan within the floor area of the new and existing building

the primary use of the structure/facility.
53. Indicate on the floor plan drawing within the footprint of the east building the
primary use of the structure/facility.
54. Identify on the site plan drawing the actual distance that each building is set
back from the north, east, south and west property lines. Please note that
overhangs, covered walkways, canopies, awnings or other appurtenances that
are attached to the building shall be considered when identifYing building
setbacks. Therefore, identify the width of the proposed overhangs, covered
walkways, canopies, awnings, and/or other roofed areas that extend out
beyond the main walls of the building. The building setbacks shall comply
with setback regulations specified in the Zoning Code.
55. At time of permit review, provide a copy ofa city-approved waiver of plat
showing approval for subdividing the property. The waiver shall describe
each lot, parcel or tract ofland. At time of permit review, submit separate
surveys of each lot, parcel or tract. For purposes of setting up property and
ownership into the computer, provide a copy of the recorded deed for each
lot, parcel or tract. The recorded deed shall be submitted at time of permit
reVIew.
56. At time of permit review, submit signed and sealed working drawings of the
proposed construction.
57. Add to the submittal a partial elevation view drawing of the pre-cast wall.
Identify the type of wall material and the type of material that supports the
wall, including the typical distance between supports. Also, provide a typical
section view drawing of the wall that includes the depth that the wall supports
are below finish grade and the height that the wall is above finish grade. The
location and height of the wall shall comply with the regulations specified in
the Zoning Code.
PARKS AND RECREATION
Comments:
58. Provide notes on plan directing the contractor to contact the Parks Division
prior to any construction activity to avoid disturbing any irrigation lines on
the Congress Ave. or Golf Road right -of-way.

59. The following comments relate to Chapter 4, Section 7, Site Plan submission
requirements:
- Indicate both required and provided setbacks for each parcel under
Site Data.
- The parking requirement for parcel "B" is calculated as 110 spaces.
Only 107 spaces are provided on the site plan. Review and revise to
accommodate required parking for this parcel.
- The parking spaces shown on the site plan do not match the
calculations provided under Site Data. The parking count on site is
181. Three (3) rows of parking in front of the MedicallProfessional
Office are labeled "8" spaces when only seven (7) exist on the plan.
Revise accordingly.
- A total of eight (8) handicapped parking spaces are required. Revise
site data to reflect this requirement.
- Indicate roof tile color and type on the elevation drawing A3.01 for
the Medical/Professional Office building.
- The elevation drawings are mislabeled regarding compass direction.
Revise accordingly to match site plan.
60. The following comments relate to Chapter 21, Article IV, Section 1, Signs
Allowed:
- Provide a sign detail for the Medical/Professional Office building
showing material, color and square footage dimensions.
- The sign face for both monument signs exceed the 64 square foot
maximum allowed. Reduce accordingly.
- Sign Size and style should be consistent with the scale and
architectural design of the buildings on site. (Also refer to Chapter 9
Community Design Plan)
- Provide a space for the parcel address on the Walgreens monument
sign.
61. Indicate the total number of trees required and provided on the landscape
plan. (Ch. 7, Article II, Sec. 5)
62. The entrance drive along Golf Road exceeds the maximum permitted width of
32 feet. An administrative approval is required to allow the driveway to be 36
feet as shown. (Ch.23, Art. II, H.1.)
63. The application incorrectly describes the project location. Should read: the
NE comer of Congress Ave. and Golf Road. Correct the application.
64. A conditional use is only required for the Walgreens drugstore on Parcel "A"
contingent upon rezoning to C-3.

2. Add the following notes to the Site Plan and Paving and Drainage Plan and correct all details
accordingly:
a. All striping and signage shall conform to City Standard Drawings B-98001 and B-90013.
b. The drainage system shall conform to City Standard Drawing B-91 007 and the Utilities
Engineering Design Handbook and Construction Standards.
3. Show the sidewalks having a minimum concrete thickness of 6"at all driveways. [LDR Chapter
23, Article ILP.]
4. At the end of the 5' handicap access aisles, show a 6' length of sidewalk at the same grade with
accessible ramps up to the sidewalk. Curb ramps must be located outside of access aisles per
Section 4.7 of the Florida Accessibility Code.
5. On the Site and Landscape plan, use FDOT Index 546 criteria for the sight triangles at the
Congress Avenue driveway.
6. Specify FDOT "D" curb for all 6-inch concrete curb.
7. Add a "$250 Fine" plate to the bottom ofthe handicap sign on the Site Plan. Also, specify that the
bottom of the handicap sign shall be located 7' above sidewalk. [City Standard Drawing B-

8. Pollution retardant baffles shall be required for all drainage structures that outfall to exfiltration
trenches. Drainage structures must be large enough to accommodate the baffles and allow silt
removal.
9. The loading area may be reduced to a 12' x 35' space.
10. As an alternate, the paved area between the wheel stops and the end of the parking spaces may be
landscaped in addition to the required landscaping. [LDR Chapter 7.5, Article II, Section 3 B 4.]
11. As an option, sidewalk along the building side( s) may be widened an additional two feet to serve as a
continuous wheel stop in lieu of individual wheel stops.

12. It is recommended to reduce the two - 12' egress lanes onto Golf Road to 11' widths and make the
ingress driveway 14' wide to facilitate safer, quicker transition off from Golf Road.
13. The width ofthe east/west aisleways along the north side of the buildings bottlenecks from 27' to
19'. To provide a smooth, safe transition between the two aisleways, the following is
recommended for your consideration:
a. Relocate the curbing along the detention area so that it is in alignment with the 27' aisleway
north of the office building. Transition the curb back to its proposed location within the radial
portion. This will guide westbound traffic thru the site.
b. Show a 6" white, thermoplastic stripe beginning at the NW comer of the pharmacy with a
radial line in alignment with the back of the parking spaces abutting the office building.
Include at least one set of directional arrows within this area.
14. Since two way traffic between the buildings can be a minimum of 20 feet and since the proposed
bank "by-pass" lane creates a traffic conflict, the following is strongly recommended:
a. Close the opening by making it part of the median separator. Move the entire median five
feet to the east. This provides a 22-foot, two lane accessway between the buildings and an
eleven foot by-pass lane, within the bank drive-in area. Please note! Ifthese
recommendations are acceptable, then the median separator and the two stop bars can be
extended north to the white thermoplastic guideline mentioned above. This relocation will
provide safer traffic movements into the east/west aisle.
15. It is recommended that the applicant delete the 5' cross-hatched area and related curb ramp at the
SE comer of the drug store and add 2.5' to each of the terminal islands.
16. Apply FDOT Index 546 for the sight triangles for the Congress Avenue driveway.
17. Locate the stop bar/sign at the Golf Road driveway 4' back of sidewalk or at least to the property
line.
18. Consider deleting the stop bar/sign for eastbound traffic just south of the drive thru lanes.
19. Verify that PBC will allow a stormwater tie-in to their system from your control structure.
Provide a copy of the PBCED R/W permit.
20. Sod and irrigate those portions of the site designated as "phase two".

We have reviewed the plans for the above-referenced project. The applicant must acknowledge
and address the below listed comments before the Building Division will recommend that the
project be presented to the Board for their review.

1. Add to the buildings that are depicted on the drawing titled site plan a labeled symbol that
identifies the location of the handicap accessible entrance door to each building.
2. On the floor plan drawings, add a labeled symbol that identifies the location of the
handicap accessible entrance door to each building. The location of the doors shall match
the location of the accessible entrance doors that are depicted on the site plan drawing.

Add a labeled symbol to the site plan drawing that represents and delineates the path of
travel for the accessible route that is required between the accessible parking spaces and the
accessible entrance doors to each building. The symbol shall start at the accessible parking
spaces and terminate at the accessible entrance doors to the building. The symbol shall
represent the location of the path of travel, not the location of the detectable warning or
other pavement markings that are required to be installed along the path. Identify on the
site plan the width ofthe east accessible route to the east building. (Note: The minimum
width required by the code is forty-four (44) inches). Add text to the drawing that would
indicate that the symbol represents the accessible route and the route is designed in
compliance with Section 4.3 (Accessible Route) and 4.6 (Parking and Passenger Loading
Zones) of the Florida Accessibility Code for Building Construction. Please note that at
time of permit review, the applicant shall provide detailed documentation on the plans that
will verify that the accessible route is in compliance with the regulations specified in the
Florida Accessibility Code for Building Construction. This documentation shall include,
but not be limited to, providing finish grade elevations along the path of travel.

Identify within the site data the finish floor elevation (lowest floor elevation) that is
proposed for the buildings. Verify that the proposed elevation is in compliance with
regulations of the code by adding specifications to the site data that address the following
issues [Section 3107.1.2, Chapter 31 of the Boynton Beach amendments to the 1997 edition
of the Standard Building Code]:
a) The design professional-of-record for the project shall add the following text to the site
data: "The proposed finish floor elevation _' _ NGVD is above the highest 100-
year base flood elevation applicable to the building site, as determined by the South
Florida Water Management District's surface water management construction
development regulations."
b) From the FIRM map, identify in the site data the title of the flood zone that the
building is located within. Where applicable, specify the base flood elevation. If there
is no base flood elevation, indicate that on the plans.
c) Identify the floor elevation that the design professional has established for the buildings
within the footprint of each building that is shown on the drawings titled site plan, floor
plan and paving! drainage (civil plans).

On the drawing titled site plan, identify and label the symbol that represents the property
line.
7.
The building plans are not being reviewed for compliance with the applicable building
codes. Therefore, add the words "Floor plan layout is conceptual" below both floor plan
drawings found on sheets Dl and A2.01.
8.
On the elevation view drawing of the medical office building, dimension the overall height
of the building. The overall height of a building shall be measured from finish floor to the
highest point of the roof or parapet wall. The dimension shown on the plan does not show
the dimension to the highest point of the building. The overall height of the building shall
not exceed the height limitations ofthe Zoning Code. Correct the plans accordingly.
9.
Add to the site plan drawing all proposed easements. The location, type and size of the
easement(s) shall be shown and identified on the site plan. Where applicable, amend the
plans so that structures do not encroach into an easement.
10.
On all floor plan drawings, indicate within the footprint of the building the number of
stories that are in the building including, where applicable, mezzanines.
11.
Indicate on the site plan within the floor area of the new and existing building the primary
use of the structure/facility.
12.
Indicate on the floor plan drawing within the footprint of the east building the primary use
ofthe structure/facility.

13. Identify on the site plan drawing the actual distance that each building is set back from the
north, east, south and west property lines. Please note that overhangs, covered walkways,
canopies, awnings or other appurtenances that are attached to the building shall be
considered when identifying building setbacks. Therefore, identify the width of the
proposed overhangs, covered walkways, canopies, awnings, and/or other roofed areas that
extend out beyond the main walls of the building. The building setbacks shall comply with
setback regulations specified in the Zoning Code.
14. At time of permit review, provide a copy ofa city-approved waiver of plat showing
approval for subdividing the property. The waiver shall describe each lot, parcel or tract of
land. At time of permit review, submit separate surveys of each lot, parcel or tract. For
purposes of setting up property and ownership into the computer, provide a copy of the
recorded deed for each lot, parcel or tract. The recorded deed shall be submitted at time of
permit review.
15. At time of permit review, submit signed and sealed working drawings of the proposed
construction.
16. Add to the submittal a partial elevation view drawing of the pre-cast wall. Identify the type
of wall material and the type of material that supports the wall, including the typical
distance between supports. Also, provide a typical section view drawing of the wall that
includes the depth that the wall supports are below finish grade and the height that the wall
is above finish grade. The location and height of the wall shall comply with the regulations
specified in the Zoning Code.

into two categories; code deficiencies with code sections referenced and recommendations that
you believe will enhance the project.
Adhering to the following review guidelines will promote a comprehensive review and enable the
applicant to efficiently obtain Technical Review Committee approval:

frame stated above, to me. The memorandum shall state that the plans are approved and that
they do not have any comments on the plans submitted for review and that they recommend the
project be forwarded through the approval process.
All comments shall be typed, addressed and transmitted or e-mailed to Blythe Williamson and I for
distribution to the applicant. Please include the name and phone number of the reviewer on this
memorandum or e-mail. Lusia Galav will be the Planning and Zoning staff member coordinating the
