The Palm Beach County Traffic Division has reviewed the traffic study (Received on
July 13, 2000) for the project entitled; Bethesda Memorial Hospital. pursuant to the
Traffic Performance Standards in Article 15 of the Palm Beach County Land
Development Code. The project is summarized as follows:

Based on our review, the Traffic Division has determined that the project (Phases 1 and
2) meets the Traffic Performance Standards of Palm Beach County, contingent to the
following improvements:
.
Addition of a second exclusive left turn lane on the northbound approach -
Intersection of Woolbright Road/Seacrest Boulevard.
Provision of a southbound exclusive right turn-lane along Seacrest
Boulevard, onto the project access driveway.
Furthermore, it is suggested that the following issues be considered by the City of
Boynton Beach:
Intersection of Woolbright Road/l-95 NB Ramps - Although the intersection is
forecasted to operate at acceptable conditions, however, peak hour left-turn
volumes for both westbound and eastbound approaches justify the need for
additional exclusive left-tum lanes (based on HCM criteria). Provision of second
left-turn lanes should be considered for both approaches.
Proper traffic control devices should be considered and installed (upon satisfaction
of warrants) at both major access driveways (east and north driveways),

INTRODUCTION
Bethesda Memorial Hospital is an existing hospital and medical care facility that is
located in the southeast quadrant of the intersection of Sea crest Boulevard and Golf
Road (S.E, 23rd Avenue) in the City of Boynton Beach, Florida. The location of the
site is shown in Figure 1.
The existing hospital is an existing 593,434 square foot hospital and medical office
facility. It is proposed to expand the facility by a total of 121,277 square feet in two
separate phases. The proposed development plan for the two phases is as follows:
. Phase 1: 32,835 square feet of medical office, proposed to be built
in 2001
. Phase 2: 88,442 square feet of medical office, proposed to be built
in 2002
The proposed expansion will add square footage to the existing hospital building but
will not increase the number of beds,
Kimley-Hom and Associates, Inc. was retained to prepare a traffic impact analysis
for the project. This document presents the methodology used and the findings of
these analyses,
The traffic impact analysis was conducted in accordance with the 1990 Traffic
Performance Code of Palm Beach County and used current data available from
Palm Beach County,

The data used in this analysis were obtained from Palm Beach County and Kimley-
Horn and Associates, Inc. They included:

Planning Data
Information on the size of the project facilities was provided by HDR Engineers &
Architects. External access to the site is currently provided through two access
driveways on Seacrest Boulevard and another driveway on Golf Road, No new
additional access driveways are planned to be created in conjunction with the
proposed expansion,

PROJECT TRAFFIC
Traffic Generation
The daily traffic generation for the development was based on the trip generation
rate for a medical office on a hospital campus as published by Palm Beach County,
This rate was used because there is no increase in the number of beds associated
with the proposed expansion, Peak hour characteristics were based on information
published in the Institute of Transportation Engineers' (ITE) report, Trip
Generation, 6th Edition, Tables IA and lB present the proposed potential trip
generation for the phase 1 and ultimate project buildout, respectively, The pass-by
rate published by Palm Beach County was used in this analysis.
Traffic Distribution and Assignment
A directional distribution was developed to be used in assigning traffic volumes
associated with the project. This was necessary to determine the impact of the
traffic from the proposed development on the surrounding roadway network. The
Florida Standard Urban Transportation Modeling Structure (FSUTMS) computer
program and lRANPLAN language were used with Palm Beach County MPO 2020
data to determine traffic distribution and assignment of the project traffic to the
surrounding road network.
Based on the net impact of the total project buildout, it was determined that the
maximum radius of development influence for Test I of Traffic Performance
Standards is two (2) miles. The maximum radius of development influence for Test
2 is one (1) mile,

within the project's radius of influence.

LINK EVALUATION
Test 1
A link performance standards evaluation was undertaken for each link considering
its total traffic volume and the roadway laneage to be in place in year 200 I for phase
1 and year 2002 for the total project buildout. 1999 traffic counts were obtained
from Palm Beach County for all major roadway links within the radius of
development influence. Because no counts were available from Palm Beach County
for the segment of Seacrest Boulevard immediately adjacent to the hospital, Kimley-
Horn and Associates, Inc. conducted 24-hour counts to the north and to the south
of the main hospital entrance at S.E. 26th Avenue. The counts were conducted on
Thursday, June 29, 2000 and were factored to Annual Average Daily Traffic
(AADT) by using the AADT adjustment factor established by the Florida
Department of Transportation for eastern Palm Beach County. The traffic count
data are included in the appendix.
According to the requirements of the Traffic Performance Standards Ordinance of
Palm Beach County, existing traffic volumes were grown to 2001 and 2002 AADT
volumes by using an area wide growth rate determined from a review of historic
traffic count data (AADT 1996 to AADT 1999) published by Palm Beach County.
A table showing the area wide growth rate calculations is included in the Appendix,
Project traffic was then added to the 2002 background traffic for all of the roadway
segments within the radius of development influence. Roadway links within 0.5
miles of the project are found to be significantly impacted if the amount of project
traffic on the roadway exceeds 0,5% of the level of service D capacity. The
remaining study links are significantly impacted if the amount of project traffic on

the roadway exceeds 1.0% of the roadway capacity. Tables 2A and 3A show the
Test 1 Average Daily and Average Peak Hour traffic on the roadway links for Phase
I of the project. Tables 2B and 3B show the Test I Average Daily and Average
Peak Hour traffic on the roadway links for total project buildout.
A total of five roadway segments will be significantly impacted by traffic from
Phase 1 of the proposed development. As shown on Tables 2A and 3A, all five of
these significantly impacted segments meet the Test I standards for 2001. A total
of seven roadway segments will be significantly impacted by traffic from the total
buildout of the project in 2002. As shown in Tables 2B and 3B, six of the seven
significantly impacted segments meet the Test I criteria for 2002. The segment that
does not meet the Test I criteria, Woolbright Road from 1-95 to Seacrest Boulevard,
was then analyzed using Alternate Test 1 criteria.

Alternate Test 1
Woolbright Road from 1-95 to Seacrest Boulevard was analyzed for peak hour peak
season conditions in order to determine if the segment meets the Alternate Test 1
standard, Table 4 shows the peak hour directional link volumes for this link. As
can be seen from Table 4, Woolbright Road will meet the peak hour link level of
service standard through 2002.
In addition to the link level of service analysis, intersection critical volume analyses
were performed on the two signalized intersections that form the termini of this
segment. Year 2000 turning movement volume data for the intersection of
Woolbright Road and 1-95 was provided by Palm Beach County. For the
intersection of Woolbright Road and Seacrest Boulevard, turning movement
volumes were counted by Kimley-Horn and Associates from 7 a,m. to 9 a.m. and
4 p,m. to 6 p.m, on Thursday, July 6, 2000. The observed volumes at both
intersections were adjusted to peak season and increased, based upon a weighted
growth rate, to the 2002 buildout year, The worksheets in the Appendix show the
development of total 2002 intersection traffic and the critical movement analyses
at each intersection.
The critical movement analyses indicate that the intersection ofl-95 and Woolbright
Road will operate acceptably with total 2002 traffic with no additional
improvements. The intersection of Woolbright Road and Seacrest Boulevard,
however, will exceed the critical sum. In order to address this deficiency, the
intersection was analyzed to determine which improvements would be necessary in
order to meet the Traffic Performance Standards. It was found that dual northbound
left turn lanes would be necessary to relieve the capacity problem at this
intersection. This conclusion is based on a count made outside of the peak season
and factored to peak season. It may be desirable to perform a count during peak
season 2001 to develop a more accurate analysis of conditions at this intersection.

AND TURN LANE REQUIREMENTS
Driveway Classification
According to the Palm Beach County "Guide Parking Lot and Street Access Design
Criteria and Standards", it is necessary to classify project entrances as minor,
intermediate, or major according to the following criteria:
. Minor - Provides for a maximum average daily traffic of 500.
. Intermediate - Provides services for a maximum average daily traffic
from SOl to 2000.
. Major - Provides service for a maximum average daily traffic greater than
2,000.
Using these criteria, the project access driveways would be classified as follows:
. North Driveway (Golf Road) - Major
. East Driveway (Seacrest Boulevard at N,E, 26th Avenue) - Major
. Southeast Driveway (Seacrest Boulevard at south hospital boundary)-
Intermediate
The traffic from the proposed expansion will not result in a change in classification
for any of the driveways,
Turn Lane Requirements
Palm Beach County access design standards'require a left-turn lane when inbound
peak hour left-turning traffic is 30 vehicles or more. A right-turn lane is required

when inbound peak hour right-turning traffic is 75 vehicles or more. Left-turn lanes
into the site already exist on Seacrest Boulevard and on Golf Road. With the
existing traffic volumes, a right-turn lane into the site is warranted for the
southbound approach of Sea crest Boulevard at S.E. 26th Avenue. Therefore, with
the proposed expansion, a right-turn lane would also be warranted at this entrance.
However, the volume of through traffic on Seacrest Boulevard is well below the
roadway capacity. If the southbound right-turn volume becomes high enough to
require a right-turn lane, the outer through lane may act as a defacto right-turn lane
because the inner through lane can easily accommodate the southbound through
traffic. The intersection analysis of future volumes with the existing lane
configuration shows that the intersection will operate at level of service B or better.
Site Access Analysis
Intersection analyses were performed at the signalized intersection of Seacrest
Boulevard and N,E, 2(j1:h Avenue, which is the primary entrance to the project site,
Intersection turning movement counts were conducted between 7:00 a.m, and 9:00
a.m. and 4:00 p,m. and 6:00 p.m. on Wednesday, June 28, 2000. These traffic
volumes were factored to 2000 peak season conditions using the Florida Department
of Transportation's peak season adjustment factor of eastern Palm Beach County.
To determine intersection traffic volumes for Phase 1 of the project, the existing
peak season volumes were increased to 2001 conditions using the area-wide growth
rate, Project traffic for phase I was then added to determine total 2001 traffic
conditions. To determine intersection volumes for the project buildout, the existing
peak season volumes were factored to 2002 conditions using the same area-wide
growth rate. Total project traffic was then added to determine the total 2002 traffic
conditions at the intersection. The corresponding volume development worksheets
are included in the Appendix,

Using the Highway Capacity Software, version 3.1c, operating conditions at the
intersection were evaluated. The intersection is currently operating at level of
service B. The analyses show that it will continue to operate at level of service B
or better for both 2001 and 2002 total traffic conditions.

CONCLUSION
The expansion of Bethesda Memorial Hospital is proposed to occur in two phases,
with buildout in 2001 and 2002. This traffic impact analysis took into account the
amount of development projected to occur in each phase and the traffic conditions
on the roadways within the radius of development influence during both horizon
years,
Phase I of the proposed project will meet the requirements of the Palm Beach
County Traffic Performance Standards with no required improvements.
Ultimate buildout of the project would occur with the completion of Phase 2 in
2002. Considering the total project traffic and 2002 roadway conditions, it is
expected that the intersection of Woolbright Road and Seacrest Boulevard will fail
to meet Palm Beach County Traffic Performance Standards, As shown in the
analysis, improving the northbound approach to include dual left-turn lanes would
relieve the capacity problem at the intersection.

Site Characteristics: The site is fully developed as the Bethesda Memorial Hospital. The proposed four-story
addition is located at the front entrance of the building. Demolition and site reconstruction is
required to accommodate the renovation and addition.
Proposal:
The developer, Bethesda Memorial Hospital, proposes to construct an 88,442 square foot,
four-story addition to the front of the existing hospital. The expansion and renovation will
affect the emergency room, operating room, pediatrics and the hospital lobby. (See Exhibit
liB" - Proposed Site Plan).
Concurrency:
a. Traffic-
A traffic statement for the project has not been submitted. Prior to final approval,
staff must receive confirmation from Palm Beach County's Traffic Division regarding

Conceptual drainage information was provided for the City's review. The
Engineering Division has no additional comments regarding drainage. Specific
drainage solutions will be finalized at time of permit review.
The main entrance to the hospital is off of SW 26th Street. No entrance drive modifications
are proposed.
The hospital and ancillary uses require a total of 750 parking spaces, A total of 1183
parking spaces are provided both on and off site. This includes 29 handicapped spaces
provided by the hospital. The proposed addition will require the relocation of some parking
spaces on site. No additional spaces will be provided.
The landscaping for the main entrance area will be fully enhanced to include a variety of
plant material and a decorative circular fountain. Plant material includes several types of
palm trees, a Gumbo Limbo tree, two (2) Oak trees and numerous flowering shrubs and
ground cover. Several of the existing trees on site will be relocated within the new
landscape area. The landscaping on the site will exceed the code requirements when staff
comments are incorporated.
Site regulations will be fully met when staff comments are incorporated into the permit
drawings.
Community Design: The design and colors for the hospital addition will match the existing building. The new
main entrance fa<;ade will consist of an aluminum framed glass curtainwall with stucco
accent features.
Signage:
RECOMMENDATION:
No new signage is proposed for this site.
Staff recommends that this site plan request be approved, subject to the comments included in Exhibit "C" -
Conditions of Approval. The Technical Review Committee (TRC) recommends that the deficiencies identified in
Exhibit "C" be corrected on the set of plans submitted for building permit.

This letter is written to address the items that you requested from the hospital on
February 29, 2000, reference project, P-10-F (Patient Floor Expansion).
We will be adding fifty additional patient rooms, however, we will not be adding any
additional beds, The Hospital's licensed bed count has been 362 for the last 15-20
years and will remain at that count. Enclosed with this letter are four (4) pages that
indicate the bed counts by floors and semi-private and private rooms.
Architecturally, the expansion building design conforms to the rest of the building.
The column lines and beam lines are consistent with the existing building. The
painting scheme will remain the same.

We have reviewed the subject plans and recommend that the request be forwarded for Board review
with the understanding that all remaining comments will be shown in compliance on the working
drawings submitted for permits.
Buildine Division (Site Specific and Permit Comments) - Michael E. Haa~ (561) 742-6352
1. At time of permit review, submit signed and sealed working drawings of the proposed
construction, The maximum size drawing allowed is 30" by 42". [Departmental Policy, Section
A. 1. page A-4]
2. At time of permit review, provide detailed drawings and documentation on the plans that will
verify the accessible route that is shown from the accessible parking spaces to the accessible
building entrance is in compliance with the regulations ofthe Florida Accessibility Code for
Building Construction, The documentation shall include, but not be limited to, identifying the
type of material proposed for the route, type of surface finish, width, cross slope, slope and
length of the entire accessible route including, where applicable, landings, ramps and handrails.
3. The minimum scale allowed for building drawings is 1/8" = 1 '0", Amend the drawings
accordingly. [Departmental Policy, Section B, 1. c, page A-4]
4. Within the site data found on sheet C-2, identify the proposed finish floor elevation of the
proposed building (lowest floor elevation). Verify that the proposed elevation is in compliance
with regulations of the code by adding specifications to the site data that address the following
Issues:
a) The design professional-of-record for the project shall add the following text to the site data:
"The proposed floor elevation _' _ NGVD is above the highest 100-year base flood
elevation applicable to the building site, as determined by the South Florida Water
Management District's surface water management construction development regulations."

b) From the FIRM map, identify the title of the flood zone that the building is located within,

6, Permits are required to construct the improvements that are shown on the approved site
plan/conditional use documents, Permit application forms are available in the Building
Division of the Development Department. A permit submittal checklist is available in the
Building Division. The list identifies the basic documents that the Plans Analyst checks for
when an applicant submits for permit review.

The following responses are in regards to review comments dated May 26, 2000 on the above referenced
project:

GENERAL COMMENTS
1. All utility easements shall be shown on the rectified site plan and landscape drawing so that
we may determine which trees may interfere with utilities. In general, palm trees will be the
only tree species allowed within utility easements. Canopy trees may be planted outside of
the easement so that roots and branches will not impact those utilities within the easement in
the foreseeable future. (LOR Sec. 7.5-18.1) gives public utilities the authority to remove any
trees that interfere with utilitv services, either in utility easements or Dublic riahts-of-wav.
Response: There are no utility easements in the area of new landscape plantings. The
existing easements are noted on the various site plans.
2. Palm Beach County Health Department permits may be required for the water and sewer
systems servin.a this proiect (CODE Sec. 26-12). (See also Comments #4 throuqh #6 below.)
Response: BMH will acquire all necessary permits for this project.
3. Since additional beds/toilet facilities are being added, the CODE Sec. 26-34(E) requires that a
capacity reservation fee be paid for this project either upon the request for the Department's
signature on the Health Department application forms or within 30 days of site plan approval,
whichever comes first. This fee will be determined based upon final meter size, or expected
demand of the added facilities.
Response: The capacity reservation fee will be paid by BMH upon request from the
City.
SPECIFIC CONDITIONS
4. ft the water and sewer lines are to be owned and operated by the City, they shall be included
within utility easements. Please show all proposed easements on the engineering drawings,
using a minimum width of 12 feet. The easements shall be dedicated via separate instrument
to the City as stated in CODE Sec. 26-33(a).
ft water and sewer lines are to remain private, an agreement to that effect will be required
stating that the hospital facility will take full responsibility for the operation and maintenance of
the utilitv lines.
Response: BMH owns and operates the water line from the meter in. There is no new
water in this project. BMH owns and operates the sewer lines from the property line
in.

5. Site Plan reflects the new 10-inch sanitary sewer connecting to an existing lift station within
Seacrest Boulevard. This is in error, and existing drop manhole would be appropriate as an
identification tag.
Response: The label has been changed as recommended by the City.
6. Provide an acceptable detail of the sanitary sewer through the "soakage pit"; reflect what
changes to the drainage "soakage pit" will be made and what effect this will have on the storm
drainaae calculation.
Response: The detail has been added to Sheet C-4. Sheet C-4 has been added to the
TRC review set. The affect on the soakage pit is negligible.
7. Add a note that the pavement repair over the new sanitary line shall be repaired per Utilities
Department Standard Detail G-7.
Response: The note has been added to Sheet C-2.
8. Appropriate backflow preventer(s) will be required on the domestic water services to the
building addition (if not currently available), and the fire sprinkler line if there is one, in
accordance with CODE Sec. 26-207.
Response: There is no new water in this project.
9. A building permit for this project shall not be issued until this Department has approved the
plans for the water and/or sewer improvements required to service this project, in accordance
with CODE Sec. 26-15.
Response: Sheets C-2, 0-1 and 0-2 have been submitted for review.
10. A scaled drawina showina the site drainaae Dlan shall be submitted (LOR Chao. 4 Sec. 7F).
Response: A sealed drawing has been submitted with this response.
11. Utility related construction shall be in accordance with the standards of the Utilities
Department's Desion Handbook.
Response: This note has been added to Sheet C-2.

Response: BMH will have a fire hydrant flow test performed.
13. The location of hydrants/main sizes and Fire Department sprinkler connection must be

ENGINEERING DIVISION
16. On the Site Plan add a general note that all plans submitted for specific permits shall meet I
the City's code requirements at time of application. These permits include, but are not limited I
to the following: site lighting, paving, drainage, curbing, landscaping and irrigation. Permits
required from other permitting agencies, such as FOOT, P8C, SFWMD, P8CDERM, L WOO,
FDEP and anv others shall be included with the permit request.
Response: The note has been added to Sheet C-2.
17. Add a note to the Site Plan that all striping, markings, and signage shall conform to City
Standard Drawing 8-98001. Correct the width of the handicap stall son the Site and
Landscape Plans to be 12 feet wide.
Response: The note has been added to Sheet C-2. The handicap stall width has been
revised.
18. ft van accessible handicap stalls are designed, the width of the crosshatched aisle shall be 8
feet and the parking stall - 9 feet each. It is recommended that the van accessible stalls be
located on either side of the pedestrian walkway for the shortest accessible route to the front
entrance. !
Response: Sheets C-2, C-3, and C-4 have been revised per this comment.
19. Add a note that the sanitary sewer trench repair shall be done in accordance with City
Standard Drawina 8-98002.
Response: The note has been added to Sheet C-2.

Response: The existing handicap spaces are now noted on Sheet C-O.
26. Add a labeled symbol to the site plan that represents and delineates the path of travel for the
accessible route that is required between the accessible parking spaces and the accessible
entrance door(s) to the building. The symbol shall start at the accessible entrance door(s) to
the building. The symbol shall represent the location of the path of travel, not the location of j
the detectable warning or other pavement markings that are required to be installed along the
path. The location of the accessible path shall not compel the user to travel in a drive/lane
area that is located behind parked vehicles. Identify on the plan the width of the accessible
route (Note: The minimum width required by the code is forty-four (44) inches). Add text that
would indicate that the symbol represents the accessible route and the route is designed in
compliance with Section 4.3 (Accessible Route) and 4.6 (Parking and Passenger Loading
Zones) of the Florida Accessibility Code for Building Construction. This documentation shall
include, but not be limited to, Drovidina finish grade elevations alona the Dath of travel.
Response: A symbol for the accessible path from the parking area to the front door
has been added to Sheet C-2.
27. Within the site data found on sheet C-2, identify the proposed finish floor elevation of the
proposed building (lowest floor elevation). Verigy that the proposed elevation is in
compliance with regulations of the code by adding specifications to the site data that address
the following issues:
a. The design professional of record for the project shall add the following text to the site
data: 'The proposed floor elevation _ NGVD is above the highest 100-year base
flood elevation applicable to the building site, as determined by the South Florida

31. The applicant will be relocating a number of Palm trees existing on the site to an adjoining

clearing and grubbing permit stage of the project. The remainder of the trees will be removed
and replaced as shown on the landscape plan.
Response: Palms will be transplanted and the new plants will be added per the

We offer the following comments on the above noted project:
GENERAL COMMENTS
1. All utility easements shall be shown on the rectified site plan and landscaping drawings so that we
may determine which trees may interfere with utilities. In general, palm trees will be the only tree
species allowed within utility easements. Canopy trees may be planted outside of the easement so
that roots and branches will not impact those utilities within the easement in the foreseeable future.
LDR Sec. 7.5-18.1 give public utilities the authority to remove any trees that interfere with utility
services, either in utility easements or public rights-of-way.
2. Palm Beach County Health Department permits may be required for the water and sewer systems
serving this project (CODE Sec. 26-12). [See also Comments #4 thru #6 below).
3. Since additional beds/toilet facilities are being added, the CODE Sec. 26-34(E) requires that a
capacity reservation fee be paid for this project either upon the request for the Department's
signature on the Health Department application forms or within 30 days of site plan approval,
whichever occurs first. This fee will be determined based upon final meter size, or expected
demand of the added facilities.
SPECIFIC CONDITIONS
4. Ifthe water and sewer lines are to be owned and operated by the City, they shall be included within
utility easements. Please show all proposed easements on the engineering drawings, using a
minimum width of 12 feet. The easements shall be dedicated via separate instrument to the City as
stated in CODE Sec. 26-33(a).
If the water and sewer lines are to remain private, an agreement to that effect will be required
stating that the hospital facility will take full responsibility for the operation and maintenance of the
utility lines.

5. Site Plan reflects the new lO-inch sanitary sewer connecting to an existing lift station within
Seacrest Boulevard. This is in error; an existing drop manhole would be appropriate as an
identification tag.
6. Provide an acceptable detail of the sanitary sewer through the "soakage pit"; reflect what changes
to the drainage "soakage pit" will be made and what effect this will have on the storm drainage
calculation.
7. Add a note that the pavement repair over the new sanitary line shall be repaired per Utilities
Department Standard Detail G- 7.
8. Appropriate backflow preventer(s) will be required on the domestic water service to the building
addition (if not currently available), and the fire sprinkler line if there is one, in accordance with
CODE Sec. 26-207.
9. A building permit for this project shall not be issued until this Department has approved the plans
for the water and/or sewer improvements required to service this project, in accordance with
CODE Sec. 26-15.
10. A scaled drawing showing the site drainage plan shall be submitted (LDR Chap. 4, Sec. 7F).
11. Utility related construction shall be in accordance with the standards of the Utilities Department's
Design Handbook.

1. On the Site Plan add a general note that all plans submitted for specific permits shall meet the City's
code requirements at time of application. These permits include, but are not limited to the following:
site lighting, paving, drainage, curbing, landscaping and irrigation. Permits required from other
permitting agencies, such as FDOT, PBC, SFWMD, PBCDERM, L WDD, FDEP and any others,
shall be included with the permit request.
2. Add a note to the Site Plan that all striping, markings and signage shall conform to City Standard
Drawing B-9800 1. Correct the widths of the handicap stalls on the Site and Landscape Plans to be
12' wide.
3. If van accessible handicap stalls are desired, the width of the crosshatched aisle shall be 8' and the
parking stall - 9' each. It is recommended that the van accessible stalls be located on either side of
the pedestrian walkway for the shortest accessible route to the front entrance.
4. Add a note that the sanitary sewer trench repair shall be done in accordance with City Standard
Drawing B-98002
5. Provide a typical cross-section through the reconstructed entrance and the handicap parking areas
with specific.ations regarding the new subgrade, base and asphalt thickness.

Find attached for your review the plans and exhibits for the above-referenced project. Please review the
plans and exhibits and transmit formal written comments to Blythe Williamson no later than 5:00
P.M. on Mav 19. 2000 (10 days from "/an distribution date). When preparing your comments,
please separate them into two categories; code deficiencies with code sections referenced and
recommendations that you believe will enhance the project.
Adhering to the following review guidelines will promote a comprehensive review and enable the
applicant to efficiently obtain Technical Review Committee approval:
1. Use the review standards specified in Part IV, Land Development Regulations, Site Plan Review
and the applicable code sections of the Code of Ordinances to review and formulate comments.
2. The documents submitted for the project were determined to be substantially complete, however,
if the data provided to meet the submittal requirements is insufficient to properly evaluate and
process the project based on the review standards or the documents show code deficiencies,
additional data and/or corrections should be requested by the reviewer by contacting Lusia
Galav.
3. Each comment shall reference the section of the code that is incorrectly depicted on the
documents.

4. Technical Review Committee member(s) shall identify in their comments when the plans depict or
when the location and installation of their departmental required improvements may conflict with
other departmental improvements.
5. When a TRC Member finds a code deficiency that is outside of his/her review responsibility, the
comment and the specific code section may be included in their review comments with the name
of the appropriate TRC Member that is responsible for the review specified.
6. If a TRC member finds the plans acceptable, he/she shall forward a memorandum, within the time
frame stated above, to me. The memorandum shall state that the plans are approved and that
they do not have any comments on the plans submitted for review and that they recommend the
project be forwarded through the approval process.
All comments shall be typed, addressed and transmitted to me for distribution to the applicant. Please
include the name and phone number of the reviewer on this memorandum. Lusia Galav will be the
Planning and Zoning staff member coordinating the review of the project.
First review comments will be transmitted to the applicant along with a list of Technical Review Committee
(TRC) members. The applicant will contact TRC member(s) to clarify comments. Amended sets of plans
will be submitted prior to, or brought to the TRC meeting. Amended plans and outstanding comments will
be reviewed at the TRC meeting to confirm that the project is ready to move forward to the Planning and
Development Board meeting.
