The undersigned as applicant for Final Site Plan Approval does hereby acknowledge, represent and agree that
all plans, specifications, drawings, engineering, and other data submitted with this application for review by the
City of Boynton Beach shall be reviewed by the various boards, commissions, staff personnel and other parties
designated, appointed or employed by the City of Boynton Beach, and any such party reviewing the same shall
rely upon the accuracy thereof, and any change in any item submitted shall be deemed material and substantial.
The undersigned hereby agrees that all plans, specifications, drawings, engineering and other data which
may be approved by the City of Boynton Beach, or its boards, commissions, staff or designees shall be
constructed in strict compliance with the form in which they are approved, and any change to the same shall be
deemed material and shall place the applicant in violation of this application and all approvals and permits which
may be granted.
The applicant agrees to allow the City of Boynton Beach all rights and remedies as provided for by the

To Whom It May Concern:
Please be advised that the improvements as shown on the proposed site plan
for the above referenced project will create impervious space. This
impervious space will create surface water runoff, which we propose to
contain on site by existing stormwater vaults and new exfiltration trench, The
containment volume created will meet both SFWMD requirements as well as
any additional requirements by the City of Boynton Beach.
The project is located on the hospital grounds at the main entrance (east side
of building). The project proposes a new addition which Hinfills" between two
existing wings of the hospital. The main entry will be relocated to the exterior
limits of the new addition. The existing drive to the entrance "ill be
reconfigured and a new canopy constl'UCted. Also, the first row of existing
parking will be reconflgured. Future stormwatet runoff from the proposed
reconfigured paved areas (excluding that under the new canopy), shaH be less
than current flows. and is planned to flow to existing vaults as under existing
conditions. The stormwater runoff created by the new addition and canopy
shall be collected. contained and stored on site in a new exfiltration trench.
We are currently performing drainage calculations and will submit those along
with drainage plans for the next available submittal.
