CONTENTS OF THE CONDITIONAL USE APPLICATION Application for
conditional use shall contain two (2) copies of the following
items
a Statement of the applicant's interest in the property to be
developed, including a copy of the last recorded Warranty
Oeed, and a certificate from an attorney-at-law or a title
insurance company certifying who the current fee simple
title holders of record of the subject property are, and the
nature and extent of their interest therein, and

If joint and several ownership, a written consent to
the development proposal by all owners of record, or
2
If a contract purchase, a copy of the purchase contract
and written consent of the seller/owner, or

If an authorized agent, a copy of the agency agreement
and written consent of the principal/owner, or
4
If a lessee, a copy of the lease agreement and written
consent of the owner, or
5
If a corporation or other business entity, the name of
the officer or person responsible for the application,
and written proof that said representatives have the
delegated authority to represent the corporation or
other business entity, or in lieu thereof, written
proof that he is in fact an officer of the corporation

of Florida, showing an accurate legal description of the
subject property, and the total acreage computed to the

Environmentally Sensitive Lands Ordinance is
required, an Application for Alteration of
Environmentally Sensitive Lands (Environmentally
Impact Study) must be submitted to the Palm Beach
County Oepartment of Environmental Resources
Management (copy to City) prior to or concurrent
with the submittal of the site plan to the City
Submit a traffic impact analysis for the proposed
use The analysis shall comply with the Palm Beach
County Traffic Performance Standards Ordinance
Six (6) copies of the analysis shall be submitted
with all conditional use applications

constructed will be inspected on the basis of the
elevations submitted to the City and approved by
the City Commission Failure to construct

will result in the Certificate of Occupancy being

The follOWing will be a bnef explanation of Umsite, Inc. who is a proVider of
commurucation Sites and reqUired eqUipment for major "PCS" (personal CommurucatlOns
SefYlce) carners and of the Ommpo1Ot Network ("Ommpo1Ot") who is a proVider of Wire
less mobile telecommurucatlOns services, hcensed by the FCC to provide PCS 10 the South
Flonda area, mclud10g Palm Beach County, itS murucipahties, and elsewhere.
Sunilar to other Wireless mobile telecommumcation sefYlce proViders, Ommpo1Ot's
telecommumcations network is compnsed of a number of unmanned facihties wmch serve
as relay statlOns for the tranSInlSSiOn of radlO Signals wmch makes Wireless mobile
telecommurucatlOns possible. The antennas must be placed at heights enabling the Signals
to be relayed Without 10terference from surround1Og structures.

To obtam the needed height, the antennas are placed on rooftops where feasible or on
antenna support structures such as lattice towers or less obtruSive "monopoles"
Due to present technology and the lirmted strength of these radio SignalS,
telecommurucations facihties must be located at regular geograpluc mtervals, m effect the
"coverage" provided by a Site is generally hmited to less than 2-3 miles. The speCific
location at wluch the facihty is needed and the height at wluch the antennas must be placed
depend on a number of factors related to the geograpluc locatiOns of the other relay
facihties already m place, the charactenstics of the land and man-made structures m the
area, and the volume of the service expected or eXistmg at a speCific locatiOn. Generally,
the "search nng" for a Site is lirmted to a speCific small geographiC area where coverage has
been determmed to be needed With very httle leeway for placement of the Site. Conversely,
Without a Site at a location where one is determmed to be needed, coverage "gaps" will
eXist resultmg m an mterruption of service m those areas. For these reasons, Sites are
chosen only after careful analysis and testmg by radiO frequency engmeers.
II. The speCific Project and Site:
Based upon careful radio frequency engmeenng analysis of the coverage needs of the
planned network m Palm Beach County and the City of Boynton Beach, it has been
determmed that a relay facihty is needed at the locatiOn mdicated below to provide
adequate quality coverage for the network m tlus area of Boynton Beach, as reqmred by
Omrupomt's FCC license
To proVide coverage, Urusite seeks to construct an unmanned telecommurucatiOns
monopole to be located on leased space located Wltlun the easterly most NE corner of Lot
92 of Quantum Park at Boynton Beach Plat No 3 (Plat book 60 at pages 29-31) , legally
described as Exhibit "A" attached to the apphcatiOn. The property is zoned PID owned by
the City Of Boynton Beach, planned and approved for a recreatiOnal facihty Wlth baseball
and playmg fields Wlth more than adequate parkmg and open space. The property located
to the east is Wltlun the lirmts of un-mcorporated Palm Beach County, deSignated on the
County's ComprehenSive Plan as Industnal (IND), therefore conSidered mdustnal, but
zoned Smgle Family Residential (RS) Attached please find a letter marked Exhibit "B"
from Kathleen Girard, Pnncipal Planner of Palm Beach County, dated November 4, 1998
The facility will conSist of a 140' monopole, non-contrastmg gray m color, Wlth additiOnal
spaces for co-location of two PCS carners therefore elirmnatmg any necessary use for
addiTIonal structures Wlthm the area.

The leased area will be surrounded by an eIght (8) foot lugh wall for secunty and buffenng
purposes, landscaped and Imgated. ReqUIred SIgns will be posted pursuant to the CIty of
Boynton Beach TelecommurucatIOn Towers and Antennas Code. AllIS illustrated on the
subffiltted drawIngs entItled Quantum Park
The facilIty will also conSIst of electroruc telecommunicatIons eqUIpment wluch will be
housed WItlun one eqUIpment cabInet wluch will be placed on a concrete slab The
electroruc eqUIpment will be connected to the antennas on antenna mounts by way of
appropnate WInng and condUIts.
As stated, the Intended facilIty will be unmanned. A techruclan will viSIt the SIte
apprmamately once every two weeks to check the eqUIpment. The facilIty will not have
any effect on traffic or roads, water use, sarutary sewer; nor will It create any solid waste.
It will have no effect on any public servIces In the area and does not create any negatIve
VIsual or other Impact.
The use requested IS conSIstent WIth RecreatIon (REC) zorung distnct and IS compatible
WIth Pill uses In the surroundIng area. We therefore urge your POSItIve recommendatIOn
of tlus applIcatIon.
