WIll occur m two phases. The phase two SIte WIll be sodded untIl thIS portIOn IS
constructed.

Attached please find four sets of the site plan, landscape plan and archItectural plans for the above
referenced proJect. These plans are submitted for the purpose of requestmg a mmor modIficatIOn
to the SIte plan approved by the City CommIssIOn on April 3 2001 The SIte plan approved an
expanSIOn consIstmg of28 000 square feet to the eXlstmg 25 837 square foot bUIldmg.
ThIS request for a mmor modIficatIOn IS to mdIcate a 4,567 5 square foot area of the approved
addition as future expanSIOn. ThIS square footage IS located m the southwest portIOn of the
bUIldmg, IS depIcted as 'Phase Two Ne\v ConstructIOn', and IS proposed to be sodded until
constructIOn IS begun on thIS portIOn. All landscape mdIcated on the approved plan wIll be mstalled
With the constructIOn of the first phase of the additIOn. ThIS applicatIon has been discussed With
LUSIa Galav and falls wIthm the allowable 11l11ltS for reVISIOns allowed under the mmor modIfication
