Site Characteristics The subject property is a rectangular-shaped tract of undeveloped land,
composed of four parcels Dense vegetation appears throughout most of the
property A tree management plan was submitted with the application No
recognized environmental conditions were identified within the subject
property
Proposal Mr Gary J Rito with Codina Development Corp, agent for The Watershed Act
II, Inc. is requesting site plan approval to construct a 344 bed substance abuse
rehabilitation hospital on the above referenced lots (see Exhibit "B" - Proposed
Site Plan) On June 4, 2002, the City Commission approved a Use Approval
application for this facility The project is to be constructed in three phases
(see Exhibit "B" Proposed Site Plan)
Concurrency'
a, Traffic - A traffic statement for this project was submitted and sent to the Palm
Beach County Traffic Division for their review and approval Staff has
received confirmation from Palm Beach County's Traffic Division

regarding standards compliance
b. Drainage - Conceptual drainage information was provided for the City's review
The City's concurrency ordinance requires drainage certification at time
of site plan approval The Engineering Division is recommending that
the review of specific drainage solutions be deferred until time of permit
review, when more complete engineering documents are required
Driveways On-site traffic circulation will consist of two (2) proposed two-way driveways
providing access from Beta Drive A stabilized fire access is located in the
northeast corner of the site along Park Ridge Boulevard The north entrance
contains a 12-foot wide entrance lane and a 12-foot wide exit lane separated
by a curbed median four (4) feet in width The southern access will serve as
the major entrance for the project. It consists of 15-foot ingress and egress
drive aisles separated by a 10 foot curbed median Both entrances will be
gated The development design separates the parking into two areas The
majority of the required parking is located in the north portion of the site along
Park Ridge Boulevard
Parking Facility A total of 111 spaces are required for the proposed use based on the ratio of
one (1) space per three (3) beds A total of 143 spaces are provided including
five (5) handicapped spaces All spaces, except handicap spaces will be
dimensioned nine feet by eighteen feet (9' x 18')
The site plan depicts two parking areas One is located at the south of the site
around the main building There are a total of 19 regular spaces and three (3)
handicapped spaces located in this area There is a circular drop-off area at
the front entrance to the main building The loading and dumpster areas are
located in the southwest corner of the site and are accessed by the south or
main entrance
The second parking area is located in the northern portion of the site
containing the balance of the parking spaces The parking lots are connected
by two (2), eight (8) foot sidewalks that traverse the site
Landscaping
The proposed landscape plan exceeds the minimum code requirements The
applicant submitted a tree management plan indicating trees to be saved
relocated or removed from the site The total pervious or "green" area is
107,359 square feet or 38% of the total site The South Florida Water
Management District (SFWMD) easement provides an additional 5% pervious
area
Live Oak trees will be placed off-site in the swale area along Park Ridge
Boulevard The 20 foot north buffer will contain Live Oak trees, a Cocoplum
hedge, Dwarf Schefflera, Pittisporum and Lantana ground cover This
landscape design continues around the corner along the 20 foot buffer on the
west side of the site The two entranceways located on Beta Drive are heavily
landscaped with palms, shrubs, colorful ground cover and Yellow Elder trees
A 40-foot wide landscape buffer (Quantum Park P I D buffer requirement) will
be located along the southern boundary adjacent to the C-16 BOJflton Canal It
will be planted in accordance with the Quantum Park buffer planting plan This

heavy landscaping together with canal right-of-way will provide a significant
buffer between the residential development to the south and the proposed
rehabilitation hospital No fencing is proposed in this area.
A six (6) foot chain link fence exists along the east property line which
separates the proposed hospital from the Canada Dry distribution facility
Pongam trees are proposed for this 15 foot green area along the east property
line of the project.
The interior and terminal landscaped islands in the parking areas will be
planted with Live Oak trees The dumpster enclosure located in the southwest
portion of the site will be landscaped on all three (3) sides with a Youpon
hedge Each of the proposed buildings will have extensive foundation
plantings as well as the interior of the site along the eight (8) bot walkways
Building and Site Building and site regulations will be fully met when staff comments are
incorporated into the permit drawngs
Community Design The project will be constructed in three (3) phases using a campus type site
layout. The first phase includes a four-story main building which will contain
offices, meeting rooms, kitchen dining room and 120 patient rooms Phase
One also includes. a one story, 174 seat auditorium, pavilion, one story fitness
room/cabana and a pool Both phases Two and Three will include one three-
story building These buildings are identical in size Each will house 106
additional patient rooms
The buildings have a Mediterranean design with a textured stucco finish and
tile roofs The roofs will have a two-tone look using Monier Terracotta Antique
and Goldenrod tiles The maximum building height is 45 feet for the four story
main building However, the decorative towers on this building extend past the
maximum PID district height limitation and will require a Height Exception (see
Conditions of Approval)
The building facades will be painted using three base colors in the yellow/gold
palette (Co/orwheel Desert Vista, Sahara Gold and Soft Gold) The accent
color is Colorwheel Honeywind All facades are accented with score lines,
banding and decorative window and door casings The main building will have
a port cochere at the entrance
The project design is compatible with the high development standards
associated with the Quantum Park PID The Quantum Architectural Review
Committee must issue an approval letter for the project (See Conditions of
Approval)
Signage
The applicant is proposing one (1) monument sign to be located at the
northwest corner of the site The monument sign dimensions and style will
conform to the sign program for the Quantum Park P I D Staff recommends
that the sign detail include the propertyaddress at the top of the sign

RECOMMENDATION.
Staff recommends that this site plan request be approved, subject to the comments included in
Exhibit "C" - Conditions of Approval The Technical Review Committee (TRC) recommends that the
deficiencies identified in this exhibit be corrected on the set of plans submitted for building permit.

UTILITIES
1 All utility easements shall be shown on the rectified site plan and landscape drawings (as well as the
Water and Sewer Plan Sheet), so that we may determine which trees may interfere with utilities In
general, palm trees will be the only tree species allowed within utility easements. Canopy trees may
be planted outside of the easement so that roots and branches will not impact those utilities within
the easement in the foreseeable future The LDR, Chapter 7 5 Article I, Section 18 1 gives public
utilities the authority to remove any trees that interfere with utility services, either in utility easements
or public rights-of-way
2. Palm Beach County Health Department permits will be required for the water and sewer systems
serving this project (CODE, Section 26-12)
3 Fire flow calculations will be required demonstrating the City Code requirement of 1,500 g P m as
stated in the LDR, Chapter 6, Article IV, Section 16, or the requirement imposed by insurance

CODE REQUIREMENTS
Hydrants must provide, in addition to domestic requirements and at a
residual pressure of not less than 20 psi, a fire flow of at least 1500 gpm
Design documents where underground water mains and hydrants are to
be provided, must demonstrate that they will be installed, completed, and
in service prior to construction work per the Florida Fire Prevention Code,
(2000) Section 29-2 3.2
Emergency access shall be provided at the start of a project and be
maintained throughout construction per the Florida Fire Prevention Code,
Section 3-5, and NFPA 241, (1996) Safeguarding Construction, Alteration,
and Demolition Operations, Section 5-4 3
Pursuant to City Ordinance 9-3F, the Fire Marshal has developed an
Administrative Order dated May 9, 2001, that provides the minimum
performance for all security gates and emergency access
City Ordinance Section 9-6, 3 /7-11 requires approved automatic fire
sprinkler systems throughout Institutional or educational buildings,
hospitals, nursing homes, homes for the aged, convalescent centers,
rehabilitation facilities, day care centers for more than 12 clients under 1
year of age, adult congregate living facilities, and all occupancies and
uses of similar nature

RE Security Gates and Emergency Access
To Whom It May Concern
Boynton Beach Fire Rescue continues to maintain the positIon that security gates are
potentially detrimental to fire and rescue operations Tile proliferation of security gated
communities has resulted in many problems for responding emergency apparatus trYing to gain
entry
The minimum performance for all security gates shall be as follows
1 All security/entrance gates must have an electronic key number pad
2 The keypad will allow entrance by the simple act of pushing four (4) or five (5) buttons
3 All gates must have a security entry code approved in advance by the Fire Marshal
4 Gates may be operable by telephone from our dispatch office A phone call from our
dispatchers will open the gate and a second call will be required to close the gate
5 In case of power failure, the gate shall open automatically and remain open
6 An exception will be where a 24-hour security guard is stationed at the gate
7 A back-up device such as an authorized security box or key switch is required to operate the
gate in the event the number pad entry does not work
8 A key box shall be installed for such areas or buildings when the Fire Marshal determines
that access is necessary for all life-saving or fire-fighting purposes The type and location of
the key box shall be approved by the Fire Marshal and shall contain
· I<eys to locked points of egress whether in common areas or on the interior or exterior
of such buildings
. I<eys to locked mechanical equipment rooms
. Keys to locked electrical rooms
. Keys to elevator controls
9 No other code numbers operating methods or key systems will be kept on file by the Fire
Rescue Department
10 In the event that our units are unable to gam rapid entry With the above methods It Will
require the use of rapid forcible entry methods to gain entry The City of Boynton Beach
and/or the Fire Rescue Department shall not be responsible for or Incur any costs as a
result of gaining access to a specific area

Boynton Beach Fire Rescue Fire and Life Safety DiVision by calling (561) 742-6600 during
normal business hours
Failure to comply will result in the violation of Boynton Beach Code of Ordinances Section 9-3F

Introduction.
The subject property is located at the southeast intersectIon of Park Ridge Boulevard and
Beta Dnve (Figure 1) m the CIty of Boynton Beach, Palm Beach County Flonda. It IS
described as Parcels 48-~ 48-B, 48-C and 48-D of Quantum Park at Boynton Beach
P.I.D., Plat Book 60, PGS 34-36, Palm Beach County
The four (4) parcels described above total approximately 6.5:f: acres more or less. This
property currently is vacant land with extensIve cover by varIOUS exotic plants and trees
and a few natIye tree species.
A biologIst and Certrfied Envlfonmental Professional with Michael J Nichols &
ASSOCIates inspected the property on June 12, 2002 and on June 26, 2002 to determme
the presence/potentIal for occurrence of listed animal speCIes and the eXIsting vegetation
associations on the subject property The purpose of this report IS to describe the findmgs
of those mspections and other research conducted in regard to the SIte.
Materials & Methods:
The property was first analyzed by reVIew of available aenal photography and soil
information from the Palm Beach County Soil Survey, USD~ Soil Conservation Service,
December, 1978 (flIght date 1970). AddItionally, a "Vegetation Map of Southern
Florida", (DaVIS, J.H. Jr, 1943) was consulted. Tins information was reVIewed to obtain
an hIstoncal context. Current aerial photography was also reviewed together with site
observations (random pedestrian transects) m order to derive an accurate representation
of the existmg vegetatIve communities. In addition, a publication by the USD~ Soil
Conservation ServIce, ''26 Ecological Commumties of Florida" was consulted for other
information including plant and animal associatIons for categonzed ecologIcal
communities. ThIS publIcation serves as the basIS for categonzing the type of ecologIcal
commumty found at the subject site.
The Official Lists of "FlOrIda's Endangered Species, Threatened Species and Species Of
Special Concern" published by the Florida Fish and Wildlife Conservation Commission
(FF & WCC) were reViewed for any animal species which appear on "state and federal
official lists of endangered species, threatened species, and other species categonzed in
some way by the respective JUTlsdictional agencies as meritmg special protectIon or
consideratIOn. The state lists of ammals are maintamed by the Flonda Game and Fresh
Water Fish CommiSSIOn and categonzed as endangered, threatened and of speCIal
concern, and constItute Rules 39-27 003, 39-27 004 and 39-27 005, respectively, Flonda
AdmmistratIve Code (F.A.C)"

The Florida Natural Areas Inventory (FNAI) database was consulted to determine
whether any of the bsted specIes (FF&WCC) also occurred on the habItat type selected m
the USDA, SCS "26 EcologIcal Communities of Flonda" Any specIes whIch occurred
on all three ofthese sources was then analyzed and dIscussed to determine the actual
presence or potentIal for occurrence on the SIte.
The resultant discussion for each speCIes was reVIewed m bght ofthe observatIon/lack of
observatIon on the site in combinatIon wIth these other resources. The results are
described as either (1) High (2) MedIUm or (3) Low wIth respect to the likelihood oftheir
occurrence on the SIte.
The map produced for the categonzatIon of land use types was based upon the "Florida
Land Use, Cover and Forms Classification System" by FlOrIda Department of
TransportatIo~ September 1985
Discussion.
VEGETATION -
A map of the vegetatIon associations shows the FLUCCS codes for the several areas on
the site (Figure 1). The dominant vegetation on the site is exotic nuisance specIes.
However, along the western part of the SIte, a number of natIve trees are found. These
are almost exclusIvely all slash pines (Pinus elliottii) They range m SIZe from 3" DBH
to approxrmately 24" DBH. One sand pine (Pinus clausa), an approxrmate 16" DBH tree
was found near the western property lme among a groupmg of slash pine. The other
natIve trees noted were a few cabbage palms (Sabal palmetto) and three (3) pond apples
(Annona glabra) which were found on the shore1me of the canal.
In classifymg the site, it was found to be near a north/south band of wet prairie and scrub
forests as mapped by Davis, 1948 However, because of the scale and general nature of
the mapping, detail IS lacking. The soil survey shows the property to consist of Basinger
fme sand, and Basinger and Myakka sands. These soils are described as nearly level,
poorly dramed, sandy soils in broad, low sloughs and depressIons in the eastern part of
the survey area. The natural vegetatIon IS described as St. Johnswort, slash pme, southern
bayberry and scattered cypress, pineland three-aw~ blue maIdencane, broomsedge
bluestem, and low Pamcum grasses.

Because of the highly disturbed nature of the site from drainage, exotic infestatIons and
other perturbations, the community cannot be referred to as an intact natural commumty
From the soils and other mappmg mformation, It appears to have been a part of a mesic
pme flatwood commumty and perhaps could best be classIfied now as a remnant mesic
pme flatwood commumty using the FNAI system of classification.
The vegetation assemblage proVides five (5) distinct areas on the site. These include 1)
the planted street tree canopy ofpongams and Washingtoman palms, 2) the native tree
canopy of slash pines, 3) the open area dominated by low growmg grasses, grapevme,
love vme, and Caesar weed With occasional Brazilian pepper, earleaf acacia and
Australian pme, 4) the canal bank area dominated by common reed and 5) the SFWMD
C-16 Canal. These communitIes were classified using the "Flonda Land Use, Cover and
Forms ClassIfication System" by Flonda Department of Transportation, September 1985
The review of aerial photography and random pedestrian transects through the property
revealed that the majority of the site is mvaded by exotic vegetation, most notably earleaf
acacia, lead tree and BrazilIan pepper A few Australian pine trees were also found
scattered throughout the site.

During the pedestrian transects, habrtat was analyzed for the potential of occurrence of
listed faunal species. During the transects, no bsted species were observed with the
exceptIon of an indIvidual limpkin (Aramus guarauna). This indivIdual was observed
temporarily roosting in one ofthe pond apple trees along the bank ofthe canal. It IS listed
as a SpecIes of SpecIal Concern (LS) on the State lIst.
Because it IS anticipated that the C-16 Canal will not be a part of the development plans,
the listed specIes assessment is based upon the remainder of the site WhICh is designated
as pine flatwoods.
The publication, 26 EcologIcal CommunItIes of Florida (USDA, SCS), lists the followmg
animals typical of the flatwoods.

Whde gopher tOrtOIseS (SSC) are not listed as occumng m pme flatwoods, they are
included m this analysis because of the presence of a sand pine tree wluch occurs Wltlun
the site. Gopher tOrtOIses' charactenstlc habItat IS long leaf pme-turkey oak hills but they
are known to occur m south Florida where sufficient sandy soils and vegetative cover are
present.
Because of the presence of the sand pme on the site, tlus area was mspected usmg
FF&WCC guIdelines. Sandy areas that appeared as potentIal gopher tortOIse habItat
were mspected. The presence of one burrow was discovered, however, there were no
tortoises encountered. The burrow appeared to be that of an immature indiVIdual as the
hole was only about 6 -8 mches WIde. Because of extremely heavy ramfall over the
preceding tw<rweek penod, It was mdetemunable as to the nature of the burrow It did
not appear to be abandoned but was either actIve or mactIve. Ramfall was occumng
dunng the mspectIon WhICh prevented a determmatIon of recent use (tracks).
Because a single burrow was found, It IS not likely that more than one (1) tortOIse IS
present on the sIte. Further study will need to be conducted to determme whether tlus
burrow IS active.
Any overlap of specIes which occur on the FNAI's SpecIes Summary for Palm Beach
County (http.//www.fua1.org/PALM-SUM.HTM). the FF&WCC Irst and the USDA, SCS
"26 Ecologtcal Communities ofFlonda" (pme flatwoods), or other species wluch are
regarded by the author as potentIal for occurring in the subject habitat, are discussed
below as to the potentIal for occurrence on the site:
Flonda Panther (Felis concolor coryi). the Flonda panther has a large range
requrrement and is prnnarily confmed to the southern Eyerglades region. The
habItat value and food availabilIty of the SIte does not lend Itselfto the
requirements of thIs species. The likelihood of occurrence of the Florida panther
on the site is therefore very low

Mangrove (BIg Cypress) fox squirrel (Sciurus niger avicennia) the mangrove fox
squrrrelIs not shown m the FNAI Broward County lIst, presumably due to Its
lImIted range (BIg Cypress Preserve) The likelihood of occurrence on the sIte is
therefore very low
Crested Caracara, (Polyborus plancus) this large uncommon and long-legged
scavenger bIrd is lIsted under pme flatwoods because these habItats can contam
palmetto and wiregrass prairIes as under story with limited canopy However, the
subject sIte does not contain SIgnificant open prame and scrublands or other open
areas wluch are required by the caracara that spends considerable tune on the
ground. The likelihood of occurrence for tlus speCIes on the subject site is
therefore very low
Florida grasshopper sparrow (Ammodramus savannarumfloridanus) as ItS name
Implies it is a bird ofthe prairies wluch can occur in pine flatwoods. Howeyer,
the bird is not shown on the FNAI list for Palm Beach County The likelihood of
occurrence for this speCIes on the subject site IS therefore very low
Southeastern kestrel (Falco sparverius paulus) agam, a small bird, which is
dependent upon open praIries for feeding. The FNAI records do not show this
speCIes as having a confirmed presence in Palm Beach County, although It does
have the potential for occurrmg. Neyertheless, because of the lImIted open praIrie
habitat on the SIte, the likelihood of occurrence for this specIes on the subject site
is consIdered moderate.
Red-cockaded woodpecker (Picoides borealis) the red-cockaded woodpecker IS
more characteristic ofthe long-leaf pine stands and needs large, old (80-120
years) trees to nest m. The subject property does not contam the type of habItat
preferred by this speCIes. However, the species is lIsted in the FNAI summary for
Palm Beach County The likelihood of occurrence of tlus species on the sIte IS
therefore low
Bald eagle (Haliaeetus leucocephalus) the bald eagle is lIsted as a confirmed
speCIes within Palm Beach County by FNAI. However, because ofthe lImIted
size ofthe site, the proximity to major traffic (1-95), It is not expected that the SIte
would be utilIZed by tlus speCIes. The likelihood of occurrence oftlus speCIes on
the site IS therefore low

Eastern indigo snake (Drymarchon cora is couperi) this species requires a large
amount of habitat as smaller isolated tracts have httle potentIal for sustainIng long
term populations (Moler, 1992) They are also assocIated with wetlands that are
normally found m and around pine flatwoods. However, no wetlands were
discovered WIthin or m close proxuruty to the subject sIte with the exception of a
mmor amount of vegetatIon assocIated with the canal bank.. These crrcumstances
combmed WIth the large extent of exotIc vegetatIon gives a low likelihood for the
occurrence of tIns specIes on the subject sIte.
Flonda pine snake (Pituophis melanoleucus mugitus)' tlns species IS reported as a
frequent commensal to the gopher tortoISe, occupying the burrow with a number
of other specIes (Cox, et. al. 1987) Again, because of the highly disturbed nature
of the property and the insignificant burrows found, there IS a low probability of
occurrence for this specIes.
Gopher frog (Rana capito) this species is generally asSOCIated wrth xenc oak
scrub associations but can be found in pine flatwoods. It IS known as a gopher
tOrtOIse commensal due to ItS apparent dependence upon gopher tortoise burrows
although It IS known to inhabIt the burrows of rodents and crayfish as well as
stump holes and other crevIces. Because of the smgle burrow IdentIfied on the
site, the potentIal for occurrence oftlns specIes is low
Conclusions:
The subject 6.5 :t acre site has extensIve coverage by exotic vegetatIon, most prominently
Brazilian pepper, earleaf acacIa and AustralIan pine. A number of natIve trees, mostly
slash pine, are found along the western extent of the property The trees on the property
were mapped as to general locatIon Wlthm the sIte and given an apprmumate size (DBH).
The extent of exotic vegetation, drainage and other perturbences have all contributed to a
Inghly degraded site
The potential for occurrence of lIsted specIes was assessed by pedestrian transects and
research of appropriate scientific literature. The potential for varIous species was listed
as Ingh, moderate or low One indIvidual bird, a limpkm ("Species of Special Concern")
was observed roostmg m a tree along the C-16 Canal on the south end of the sIte. The
only other lIsted species wInch was representatIve of the pine flatwood community was
the gopher tortOIse which was shown to have a Ingh likelihood of occurrence due to the
observation of a smgle burrow Due to heavy ramfall, it could not be determined whether
the burrow was actIve or inactIve.

We recommend that further work be done to make a conclusive determination of the
presence of gopher tortOIses on the site. Because of the close proximity of a gopher
tortOIse preserve to the subject property, it IS recommended that any tortoises found on
the property be evaluated as to the feasibility of placing it (them) withIn the preserve.
Other optIons, including an incidental take may be appropnate If the feasibility of
relocation proves unreasonable.
