David Kelley, Jr., PE/PSM 0/ City Engineer FROM: DATE: January 10, 2003 2nd REQUEST FOR RECORD PLAT REVIEW KENSINGTON PLACE O'BRIEN, SUITER AND O'BRIEN, INC. - LB #353 RE: This Division is in receipt of the (second) revised proposed record plat noted above, received on December 13, 2002. Again, we have reviewed this document in accordance with Chapter 177, F.S. (Land Boundaries) Platting, Chapter 472, F.S. (Land Surveying), and Chapter 61G17-6 F.A.C. (Minimum Technical Standards) and those applicable code sections relating to the Land Development Regulations of the City We have found numerous discrepancies and/or omissions that will require the attention of the preparer of this document that have not been addressed satisfactorily In addition, we have noted additional comments, which will require attention. Once these items have been addressed, a (third) revised proposed record plat shaH be required as a re-submittal for the final review and approval prior to obtaining (any and) all signatories. Only at this point will the record mylar document of the proposed record plat be submitted for City Commission approval and acceptance. Having prefaced the review procedures, we offer the following comments on this submittal. Please note that we will address the responses to my original October 31 st comment review memorandum first. Then we wiH put forth any additional comments observed, second. Finally, we will delineate any procedural needs required in order to accept the proposed record plat by the City ORIGINAL FIRST REVIEW COMMENTS TITLE SHEET, Sheet 1 of2 The following (original) comments are sequenced in a counter-clockwise plan review, starting in the upper left hand corner. 1 DEDICATION AND RESERVATIONS: a) Deleted. b) & c) Addressed. Page 1 of 5 pages 2. ACKNOWLEDGEMENT: Addressed. 3 SURVEYOR'S CERTIFICATE: Addressed. 4 ACCEPTANCE OF RESERVATIONS: Addressed. 5 CITY OF BOYNTON BEACH APPROVAL: a) In the 3'd line, correct the spelling of the word "Regulations" Not Addressed. 6. Impression Seals: (at bottom of the sheet) a) & b) Addressed. PLAT SHEET, Sheet 2 of 2 7 In referring to Comment #1.b. above, a 6-foot ingress-egress easement is shown in the rear of all lots, which does not connect to any public access point. We believe that you meant this to be a utility easement. However, this would also present a problem. Utility easement easements are to be 12-foot wide (per Code), and no way to access them is (shown) available. The 'Landscape Easement' noted on either side of this (these) easement(s) does not permit access through it to whatever the easement is delineated. Please clarify the intent of your easement designation. Assumed to connect to Tract "A" throughout. 8. Addressed. 9 Deleted. 10. Addressed. 11 Addressed. SECOND REVIEW COMMENTS TITLE SHEET, Sheet 1 of2 The following comments are sequenced in a plan review clockwise, left to right. 12. In addition, in numerous other instances within this proposed plat, the city has been cited only, which is the proper way to designate it. I) In regards to the dedication clauses, 6th paragraph, 2nd line, correct the spelling of "HOMEONERS" to read "HOMEOWNERS" m) In regards to the dedication clauses, 6th paragraph, 5th line, delete the word "FLORIDA" Refer to sub-comment "k" above. n) In the authority to dedicate (last) paragraph, change the year from "2002" to "2003" 13 ACKNOWLEDGEMENT: a) In the 1st paragraph, 4th & 5th lines, the corporate officers are listed as the 'President' and 'Secretary' respectively Since they need to be delineated as such in the Dedication Page 3 of 5 pages portion of this proposed plat. they will have to match those officers exactly, whoever they may be. b) In the 1 st paragraph, 5th line, correct the spelling of "REPECTIVEL Y' to read 'RESPECTIVELY" c) In the witness (last) paragraph, change the year from "2002" to "2003" 14 ACCEPTANCE OF RESERVATIONS: In the 1st paragraph, 4th line, change the year from "2002" to "2003" 15 An additional ACKNOWLEDGEMENT is required to support this corporation. (point of Commencement) location to the plan; it simplies the plotting of the plat legal description. 18. Add the surveyor's registration number (or firm's LB number) at each (4) point where the P .R.M. is scheduled to be set. 19 Explain the purpose of the "6' INGRESS - EGRESS EASEMENT" behind the two (2) buildings or delete them from the plat. The landscape and drainage easement serves for the accessibility for maintenance purposes. It is obvious that the sheets (pages) of this proposed record plat need to be thoroughly checked and coordinated by the consulting land surveyor. Quality control is the responsibility of the plat preparer- not the Engineering Division's staff. After these proposed record plat comments have been addressed and re-submitted to this Division for another review, once approved, a field review of the required monumentation will be undertaken. To facilitate this effort, it will be requested of the surveyor-or-record to 'lathe and flag' all monuments Page 4 of 5 pages and P C.P.s for validation. If you have any questions regarding this matter, please contact the undersigned at telephone number 742-6488.
