Certificate of Occupancy, I would like to ensure that you do not have any outstanding or pending
conditions concerning this site that must be rectified (excluding issues that are covered entirely
with a City approved surety).
If any outstanding conditions are related to permits issued by the Building Division, please
provide this division a memorandum containing a description of the unresolved conditions. If
the unresolved conditions are not permit related, please notify the owner or the owner's agent to
ensure that the matters are rectified.
Please respond to me, within ten (10) days, in order that we may proceed in a timely manner to
issue the Certificate of Occupancy No response is necessary if your department is satisfied with
the current state of the above referenced site. Thank you for your cooperation.

In going through your recently submitted application for land us amendment and rezoning for
property located on S.E. 23rd Avenue (Golf Road) in the City of Boynton Beach (PIN 08-43-45-
33-25-002-0000), staffhas found several items that were not included. The items listed below,
must be submitted to our offices before we can proceed with the processing of the application:
1 Notarized Agent Consent Form (Form VI, page 9 of the application) signed by the
property owners of record and authorizing you to make application for the
proposed land use amendment and rezoning. The contract for sale and purchase,
included with the application, is not sufficient for this purpose.
2. A comparison ofthe water demand for development under the proposed zoning
with the water demand under the existing zoning. Water demand shall be
estimated using the standards adopted by the Palm Beach county Health
Department for estimating such demand, unless a registered engineer justifies
different standards. (Item (6) page 5 of the application.)
3. A comparison of sewage flows that would be generated under the proposed
zoning or development with that which would be generated under the existing
zoning. Sewage flows shall be estimated using the standards adopted by the Palm
Beach county Health Department for estimating such demand, unless a registered
engineer justifies different standards. (Item (7) page 6 of the application.)

This Division is in receipt of the (second) revised proposed record plat noted above, received on
December 13, 2002. Again, we have reviewed this document in accordance with Chapter 177, F.S.
(Land Boundaries) Platting, Chapter 472, F.S. (Land Surveying), and Chapter 61G17-6 F.A.C.
(Minimum Technical Standards) and those applicable code sections relating to the Land Development
Regulations of the City We have found numerous discrepancies and/or omissions that will require the
attention of the preparer of this document that have not been addressed satisfactorily In addition, we
have noted additional comments, which will require attention. Once these items have been addressed,
a (third) revised proposed record plat shaH be required as a re-submittal for the final review and
approval prior to obtaining (any and) all signatories. Only at this point will the record mylar document
of the proposed record plat be submitted for City Commission approval and acceptance.
Having prefaced the review procedures, we offer the following comments on this submittal. Please
note that we will address the responses to my original October 31 st comment review memorandum
first. Then we wiH put forth any additional comments observed, second. Finally, we will delineate
any procedural needs required in order to accept the proposed record plat by the City

portion of this proposed plat. they will have to match those officers exactly, whoever
they may be.
b) In the 1 st paragraph, 5th line, correct the spelling of "REPECTIVEL Y' to read
'RESPECTIVELY"

14 ACCEPTANCE OF RESERVATIONS: In the 1st paragraph, 4th line, change the year from

15 An additional ACKNOWLEDGEMENT is required to support this corporation. Refer to our
Form V -8 for the wording, or duplicate similar wording that was used for the Dedicators above.
16. CITY OF BOYNTON BEACH APPROVAL:
a) In the 1st paragraph, 3'd line, correct the spelling of "REULATIONS" to read
"REGULATIONS"

plat legal description.
18. Add the surveyor's registration number (or firm's LB number) at each (4) point where the
P .R.M. is scheduled to be set.
19 Explain the purpose of the "6' INGRESS - EGRESS EASEMENT" behind the two (2)
buildings or delete them from the plat. The landscape and drainage easement serves for the
accessibility for maintenance purposes.
It is obvious that the sheets (pages) of this proposed record plat need to be thoroughly checked and
coordinated by the consulting land surveyor. Quality control is the responsibility of the plat preparer-
not the Engineering Division's staff.
After these proposed record plat comments have been addressed and re-submitted to this Division for
another review, once approved, a field review of the required monumentation will be undertaken. To
facilitate this effort, it will be requested of the surveyor-or-record to 'lathe and flag' all monuments

and P C.P.s for validation. If you have any questions regarding this matter, please contact the

After these proposed record plat comments have been addresses and re-submitted to this division for

PROJECT DESCRIPTION:
It is proposed to develop the vacant 1.62 acres on the south side of Golf Road, just east of the FEC RR. A
total of 17 Townhomes will be built on the site.
SCOPE OF STUDY
In conjunction with a site plan submission, the City of Boynton Beach Land Development Regulations
requires a Traffic Statement. This traffic study has been prepared in accordance with the standanls and
criteria as set forth in the Palm Beach County Unified Land Development Code. Article 15. Traffic
Performance Standards.
A Traffic Statement is required to determine the total daily trips for the proposed use and the traffic impact
on the nearest major roadway link.
PROJECT TRAFFIC:
Trip generation for this site was analyzed using the Palm Beach County Trip Generation Rates for
Residential, Single family Attached for which the daily trip generation rate is 7 trips per unit.
Trips generated for 17 Units = 17 x 7 = 119 tpd.
