above-referenced, approved site plan, please be informed that the proposed changes regarding the parking
lot lighting; bus pad relocation; new Mall Entrance at The Food Court; and upgraded landscaping for the
new entrance and bus pad as shown on the revised plans date stamped 02/05/01 are "minor", as defined
within the Land Development Regulations, Chapter 4- Site Plan Review
This project may continue to be processed by the Building Division as a permit application contingent
upon satisfying the landscaping recommendations outlined in the attached memorandum from Kevin

Are the Dade County hurricane requirements in effect in the City of Boynton Beach?
. Follow SBCCI product approvals. If product cannot be found, then follow Dade County
requirements.
Although not applicable due to no expansion of the building, are radon codes and fees still in
existence?
. Yes!
SITE:
Modifications to site plan are required for any work outside building envelope or enlargement
of existing building. Proposed site changes will be upgrade of the public lighting, relocate bus
pad, upgrade landscaping within the curbline and upgrade of the main mall entrance.
. Submit four (4) sets of signed and sealed sets of plans and specifications, an itemized
list of the upgrades and a check in the amount of $100 to Mike Rumpf for minor

takes about 2 weeks. Building Department will not review a permit application for such

How are exterior signs on the building or parking lot permitted?
. Through a separate sign permit application.
Are there any Palm Beach County countywide amendments enforced in the City of Boynton
Beach? If so, where can we obtain copies?
. No! Handled through Boynton Beach amendments to national codes, which meet or
exceed Palm Beach County countywide amendments.
BUILDING PERMITS.
How many copies of plans and specs are required with the permit application?
. Building permit applications= two signed and sealed sets of plans and specifications.
. Planning and Zoning (site submittals)=four signed and sealed sets of plans and
specifications.
Is there a control number or plan review number for following up on an application?
. Yes! The control number eventually becomes the building permit number

Verify that a raised embossed seal is required on each plan and on the cover sheet of the
specs.
. An embossed raised seal is required on all plans and the cover sheet of the

Are there any drawing size restrictions?
. Yes! The maximum size drawing is 24 X 36 inches.
Does the Building Department red line the drawings?
. No, unless a comment is very minor OthelWise, written comments are prepared by
each reviewing department.
How does the applicant know what the specific comments are and on which specific plan?
. By phone and fax to the applicant. Each reviewing department prepares written
comments for their plan review The applicant receives a phone call and/or a fax of
the review comments. Comments can be directed to someone other than the
applicant if directed on the application.
Are permits issued with conditions or do corrections need to be made before permits are
issued?
. Only if very minor
How is the applicant notified that the plan review is complete?
. By phone and fax to the applicant. Each reviewing department prepares written
comments for their plan review The applicant receives a phone call and/or a fax of
the review comments. Comments can be directed to someone other than the
applicant if directed on the application
Is the permit turn around time still about 4 to 6 weeks?
. Yes, but it must be clear that this is the first review only Additional time is required for
revisions picking up comments and a second review of the corrections. The
resubmission review is routed through all the reviewing departments again, but the
resubmission pile is isolated from the first review pile of applications and takes less
time than the first review
Permit fees = +1.6% cost of project (includes plans exam fees and sub's fees) (Get CODV of

Who pays the sub's permit fees? When?
. The General Contractor pays the sub fees for the "major five", ie, roof, structure,
HVAC, plumbing & electrical. Sprinkler, irrigation and sign contractors pay their own
fees. All contractors must prove competency before picking up their individual permits.
When and where are plan review and permit fees paid?
. Plan review fees are paid at the time of application. Permit fees are paid when the
permits are picked up.
. Additionally, anyone can apply for a permit, however only the general contractor or
authorized representative may pick up permits.
Pay overtime to expedite plan review?
. Yes! This is done on a voluntary basis on weekends, etc. Applications are handled on
a first come, first served basis.
Demo Permits:
. Is it possible to get separate demolition permits? Yes!
. If yes, is this an over the counter permit, or does it require the same period of time as a
normal permit? Same requirements as a normal building permit, but take about two
weeks.
. Is there a separate fee structure. Same fee schedule as a regular building permit.
. Same number of sets of plans and specs as a normal building permit? Yes!
If the renovation is broken down into several permit applications, do we retain one overall
permit number, or will there be separate permit numbers?
. Separate permit numbers and individual CO's.
Are energy calculations required for the renovation? If so, how many copies?
. Not necessarily Depends upon the scope of work, such as replacing HVAC units.
How do we handle drawing revisions after the application is in progress or after the permit is
issued?
. Minor changes can be handled through the inspectors on site using signed and sealed
plans or sketches at the inspector's discretion. Major changes require formal revisions
to the original permit application.

Are as built plans required at the end of the job?
· No, but we will furnish them for microfilming of the "final" plans for the renovation.
Is either an architect's or engineer's certification substantial completion letter required?
. No!
GENERAL QUESTIONS/COMMENTS.
Are there any restrictions on building materials, such as dryvit?
. No!
Exit and toilet requirement plans are normally furnished with the permit application
documents. They can be sent for review and comment prior to the application to ensure we
are going the right direction. Verify how the Building Department would like to handle.
· We were directed to furnish the exit requirement and toilet requirement plans with the
first permit application for formal review
Can PVC pipe be used above ceilings in the plenum under any conditions?
. No!
Who will be our and the field's main contact at the Building and Fire Departments?
. The field inspectors.
Need phone and fax numbers and email addresses from our contacts at the city
· Obtained business cards and sign in sheet. See above attendee list.
Review audio/visual alarm requirements in detail.
· The existing fire alarm system does not meet current code and the mall will be cited by
the end of the year at which time, the mall manager will request an extension so that
we can include the upgrades in our first permit application.
Any requirements for an emergency generator?
. No!

Is there anything we are doing that will require further upgrades of the mall beyond the scope
of the budget?
· Unable to get a response until we submit our renovation documents for permit
application.
Additional comments/items discussed:
· Both the mall and food court restrooms must be brought up to code for handicap
accessibility
· Sprinkler shop drawings will be required for any changes to the sprinkler system.
. Field personnel are to follow NFPA #1 if work is being done during mall operating
hours. Advised Building and Fire Departments this should be covered at a pre-
construction meeting.
· Kiosk relocations during renovation were questioned. Again, advised Building and Fire
Departments to cover this at the pre-construction meeting. Special plans may be
required showing the sequence of relocations.
· Existing smoke hatches can remain. No mechanical smoke evacuation is required.
. The existing room number placards must be retained for emergency address
purposes.
. Fire protection shop drawings do not have to be submitted with the building permit
applications.

RE. BOYNTON BEACH MALL - LIGHT POLE INSPECTION REPORT
Engineer's Project No. 00029
Dear Mr. Pusnik:
The inspection of the existing parking lot light poles and the evaluation of their capacity to
support the proposed fixtures has been completed per our agreement dated November 29, 2000.
A visual inspection was performed on each of the light poles which are to be retrofitted with new
light fixtures. The condition of the anchor bolts, sub-bases and concrete pedestals were observed
and deficiencies, ifany, noted. The evaluation of the structural integrity of the existing pole was
made with respect their ability to support the new light fixtures per the specifications provided by
Simon Property Group.
Findings and Recommendation.
The condition of anchor bolts, sub-bases and concrete pedestals are generally good and
will reqnire no maintenance or repairs. The poles that will require some form of repair or
maintenance are identified on the attached full size drawing bearing the title" Boynton
Beach Mall, Site Lighting Renovation Plan", sheet No. E-l, dated 11/6/00.
Pole number 1 has been impacted by a vehicle and has sustained damage to the sub-base
and concrete pedestal (refer to Photo 1 and 2). The existing concrete pole and sub-base
should be removed and the concrete pedestal repaired. The repair will involve removing
the spalled and cracked concrete down to sound concrete and then repair with a concrete
patch applied with an appropriate bonding agent and reinforcement. The type of repair
required will not be completely known until the concrete in removed and the extent to
which the anchor bolt and reinforcement exposure is determined. It is not anticipated that
extensive repair will be required (i.e. complete replacement of pedestal). The interior of
the sub-base at the location of the point of impact should be wire brushed and painted to
prohibit corrosion from starting. Attempting to remove the dent might be desirable from
an aesthetics standpoint but is not necessary to restore structural integrity
Pole numbers 2 through 5 exhibit surface corrosion of anchor bolts between the pole base
plate and the sub-base (refer to Photo 3). The recommended maintenance at these
locations is to use a wire brush to remove surface rust and repaint and re-caulk the bolts
and nuts.
Pole number 6 has an anchor bolt that is corroded to the degree that it should be replaced
(refer to Photo 4). The bolt shall meet the A307 specification and be galvanized. The
bolt dimensions shall match the existing bolt dimensions.
In addition to the specific poles identified above some general maintenance is warranted
to remove debris that was found within the sub-base. It was common to find bird nesting
material inside of the sub-base. The presence of this type of debris will trap moisture that
will encourage corrosion. Also some of the electrical conduits were found to contain ant
nests. We have observed on other projects that some species of ants will remove the
insulation from the electrical wiring causing electrical shorts to occur. It would be
advisable to treat the conduits where ants are present with an appropriate insecticide to
eliminate the ant population. We will defer to a pest control specialist for determination
of an appropriate treatment method.
The structural capacity of the existing poles (KW light poles and Union Metal Corp. light
poles) evaluated using the manufacturer's data and supporting the proposed LSI
luminiare has been determined to be adequate to resist the wind load as specified by the
current building code.
I trust this letter report provides you with the necessary information you require to proceed with
this project. It has been our pleasure to provide professional engineering services to the Simon
