Dear Mr. J aroch:
In response to your request for the admimstrative reVIew and approval of the
modIfications proposed to the above-referenced, approved sIte plan, please be informed
that the elimmatIOn of four (4) parking spaces and the truck turnaround as shown on the
revised plans date stamped 05/16/01 are "mmor", as defined withm the Land
Development Regulations, Chapter 4- SIte Plan ReVIew. Also noted on the plans IS a
proposed increase in the bUIldmg square footage of 358 square feet. The SIte plan stIll
meets the parkmg reqUIrement of 38 spaces.
This project may contmue to be processed by the BuIlding DiVIsion as a permIt
applicatIOn subject to the approval of the PublIc Works Department for the new dumpster
location. Be advised that the proposed changes may reqUIre a modIficatIOn to the bUIldmg

EASEMENT
THIS INDENTURE is made thIS 11th day of January, A.D., 2002, by MASCO CONTRACTOR
SERVICES CENTRAL, INC., formerly known as GALE INDUSTRIES, INC, a Flonda
corporation having its prmcipal place of busmess at Daytona Beach, Florida ("Grantor"), to the
City of Boynton Beach, a political subdiVision of the State of Florida ("Grantee"):
(Wherever used herein, the terms "Grantor" and "Grantee" shall include smgular
and plural, hens, legal representatIves and assigns of mdividuals and the
successors and assigns of corporatIOns, wherever the context so admIts or
reqUIres.)
WITNESSETH
WHEREAS, Grantor IS the owner of property (the "Property") situate in Palm Beach County,
Florida, and described as follows.

WHEREAS, Grantee desires an easement for water and sewer utIlities and other appropnate
purposes incidental thereto, on, over and across the Property,

WHEREAS, Grantor is WIlling to grant such an easement on the terms and conditions set forth in
thIS Easement.
NOW, THEREFORE, for and in consideration of the mutual covenants each to the other runnmg
and one dollar and other good and valuable considerations, Grantor and Grantee agree as
follows:
1. Grantor grants to Grantee, its successors and assigns (a) full and free nght and authority
to construct, maintain, repair, install and rebUIld water and sewer facilities on, over and across
the Property, and (b) a perpetual, non-exclusIve easement on, over and across the Property for
said purposes and other appropriate purposes incidental thereto.
2. Grantor shall have the nght to use the surface of the Property for vehicular and pedestrian
ingress and egress and may improve the Property with driveways, SIdewalks and landscaping as
Grantor determines in its sole discretion after obtaining any appropriate permits from Grantee.

3. After construction, maintenance or other actIvity on the Property, Grantee wIll return all
property and improvements to theIr onginal condition, as near as may be practIcable.
4 The terms, conditIOns and obligatIOns of this Easement shall be deemed accepted and
agreed to by Grantee upon the recordmg of this Easement.
IN WITNESS WHEREOF, Grantor has caused these presents to be duly executed in its name
and attested by its duly authorized officers, the day and year first written above.

I am requestmg the CIty of Boynton Beach Planning and Zoning Department to review four (4)
sets of plans (Cover, SIte, UtilIty, & Landscape, signed and sealed) for minor site revisIOns.
The changes encompass a reductIOn m the number of car parkmg spaces, removal of the truck
turn around, and a minor re-alignment of the utilIties per comments from Skip Minor (City of
Boynton Beach UtilIties Dept.) The removal of four (4) car parking spaces IS allowed, as only
38 are reqmred, The truck turn around IS not reqUIred based on mput from the owner; the trucks
that WIll use thIS faCIlIty WIll be "Smgle umt" not "semI-trmler" therefore the turn around IS not
needed. The dumpster was relocated due to the elImmatIOn of the truck turn-around.
