Dear Pastor Maddox:
In response to your request for the administrative review and approval of the modifications
proposed to the above-referenced, approved site plan, please be informed that the proposed
changes regarding the parking lot as shown on the revised plans date stamped 03/22/01 are
"minor", as defined within the Land Development Regulations, Chapter 4- Site Plan Review.
This project may continue to be processed by the Building Division as a permit application
subject to the following condition(s):
1. A landscape plan must be submitted and approved prior to the issuance of a certificate of
occupancy (C.O.) for the parking lot;
2. Sod parking stalls shall have a base consisting of not less than eight (8) inches of stablized
shell rock, lime rock, or sand, or an equivalent material as approved by the development
director.
Be advised that the proposed change may require a modification to the building permit. Please

RIDER TO SITE PLAN APPLICATION
The undersigned as applicant for Final Site Plan Approval does hereby acknowledge, represent and agree that
all plans, specifications, drawings, engineering, and other data submitted with this application for review by the
City of Boynton Beach shall be reviewed by the various boards, commissions, staff personnel and other parties
designated, appointed or employed by the City of Boynton Beach, and any such party reviewing the same shall
