Please find attached the Site Plan Approval Application for your review. This submittal is to re.
~rtify the previously approved site plan for a Medical Office located at 2375 West Woolbright
Road. The 6,874 sq. ft. Medical Office is two stories with parking below the building.
As you are aware, the project received site plan approval on November 8, 2001. A one year
extension was requested and approved extending the approval to November 8, 2003. In 2003,
requirements changed to locate an x-ray and associated spaces on the ground floor, reducing the
amount ofparki~g and related square footage. The name of the project changed to Boynton
Beach Cancer Center. The project received a minor modification approval on April 4, 2003.
Following the minor modification approval, working drawings were prepared and submitted for
building permits. Unfortuvately, building permits were not issue~ prior to expiration of the site
plan approval on November 8, 2003.
Prior to submittal for building permit, the zoning code changed modifying the parking stall
dimensions. Due to the restricted size of the site and the design of the building, the increase in
stall size would require a complete redesign ofthe site and building. We submitted a request for
an Administrative Waiver to the City Engineer. On December 10, 2003; the City Engineer
approved this request. (See attachment) .
The project also received minor comments while being reviewed for building permits. The
attached submittal addresses these comments and we have included a response letter. (See
attachment)

Lusia Galav April 4, 2003 letter of approval for minor modifications to the
approved site plan. Response to Comments:
1. 12 foot utility area boundaries were corrected on the Civil Sheet 5.
2. Silver Buttonwood trees were changed to 12 feet height at planting.
3. Redtip Cocoplum hedge has been changed to 30 inches height at planting.
4. Comment is acknowledged.
Karen Main 10/27/03 Plan Review. Response to Comments:
1. The front setback at the stairway is 40 feet. Dimension has been added
to the Site Plan. The setback to the second floor of the building is 50 feet.
The front setbacks were reduced as a result of the added ground floor
X-Ray room, and approved. Please note the West side yard setback
increased with the reduction in overall square footage from 13 feet to
17 feet, and has been corrected in this submittal.
2. The overall building height of30 feet (top of parapet) was approved
by the commission. The tabular data has been revised accordingly.
3. Color chips are included with this submittal. These are the same colors
approved by the commission.
4. Irrigation plans will be provided with the permit application.
5. Cross section drawings are included with this submittal.
6. Note to screen equipment has been added.
7. Cocoplum hedge height is now shown to be 30" at planting.
8. Understood. Previous site plan submittals included signage and have
been left on as a general guide to locations for signage to be submitted for
approval at a later date.

Dear Mr. Jenner:
Nutting Engineers of Florida, Inc. has performed an exfiItration test for the proposed drainage
improvements at the above referenced site. This report presents a brief description of the field
procedures, and provides the results of the ex filtration test.
One ex filtration test was performed at the site as requested by the client. 1bis test was conducted
to a depth of six feet below existing grade in accordance with South Florida Water Management
District (SFWMD) criteria for 'Usual Open-Hole' conditions.
Prior to starting each test, a 6-inch diameter hole was augered to the test depth to detennine the
depth to groundwater and to examine subgrade soils. After establishing the above parameters,
the hole was stabilized by a full-length perforated PVC pipe in accordance with South Florida
Water Management District specifications. Water was then pumped into the hole maintaining a
constant water level at the ground surface. The stabilized flow rates were recorded in one minute
intervals for a total of 10 minutes.

The exfiltration test revealed the hydraulic conductivity CK'-value) of the soil was 3.0 X 10-
cubic feet per second per square foot per foot of head. Soil descriptions and flow rates for the
test are shown on the attached exfiltration summary sheet. We note that the water table was at a
depth of approximately six feet.

Boynton Beach. The proposed project involves the construction of a 5,282 square foot (SF) medical

generated by the proposed project. The proposed project results in 12 AM peak hour trips and 18
PM peak hour trips. Since the total peak hour traffic generation expected from the proposed project
is less than 20 peak hour trips, a traffic study is not required.
