information for your review and comment.
The information provided with this notice is very brief, but you may contact the
permitting agency directly for further information if needed in order for you to assess the
impacts of the proposed activities on your community.
If the proposed activities are not consistent with your comprehensive plan or other locally
adopted plans or regulations, or if you have any other comments, please submit them to

this office within 30 days of receipt of this notice.
This is also an opportunity for applicable State agencies to concur with or object to the proposed
project under the federal consistency provision of the Coastal Zone Management Act. Review
must be in accordance with the procedures adopted by the Interagency Management Committee
on October 25, 1989. Findings of inconsistency must describe how the project conflicts with your
agency's statutory authorities in the Florida Coastal Management Program and provide
alternative measures, if any, which would make the project consistent. Commenting agencies
must provide a copy of all consistency comments letters to the Florida Coastal Management
Program Director, Department of Community Affairs, 2555 Shumard Oak Boulevard, Tallahassee,
Florida 32399-2100.

Beach, FL There is an existing ground level parking lot that serves patients and staff at the

Dear Mr. and Mrs. Samyn,
Thanl< you for your letter of December 12, 20.03. I have reviewed your concems and can
provide you the following responses:
1. As part of the project design many trees are being relocated from the existing
parkirlg lot to the east side of the existing wall to shield the view of the structure. The
structure is located west of the existing daycare center building, which will remain. The
parking garage's closest location to any residential structure is 215 feet. The parking
garage will be located approximately 645 feet from your specific property. Between the
separation and landscape buffer program I'do not believe you be able to see the
structure from your property.
2. The new plans will supersede and enhance any existing sound site barrier. My office
has closely coordinated the landscape plan with the City Forrester who will be
monitoring both installation and maintenance of the plant materials. He has Code
enfon;ement power to insure that the landscape buffer is properly maintained in the
future. The temporary trailers are scheduled to be removed in early 2004 with the area
being replaced with the new landscape buffer plan.
3. WI! have analyzed the location of the building in relationship to the property lines and
in releltionship to the new enhanced landsca'pe buffer. The structure will have no more
effect on casting shadows on nearby properties than the landscape material, which is
located much closer to the residential area to visually block the structure.

4. According to our Civil Engineer, due. to ground water and drainage constraints, it is
not feasible to build the parking garage two (2) stories below ground level.

5. No formal sound studies have been done with regard to this application; however, the
proposal has no effect on the amount of traffic coming to the property and the fact that it
is enclosed in a garage should provide additional sound buffering from neighboring
properties. Additionally, the garage was sited in the center of the hospital property to
separate it from surrounding residential areas.
6. Thl~ top floor of the garage will always be the last to be filled up and, due to the fact
that the evening shift is the smallest will get minimum. if any, evening use. The top floor
will also have a parapet wall to further reduce any sound spillover.
7. No additional traffic is anticipated. as parking structures themselves do not generate
traffic:. Palm Beach county Traffic Engineering has confirmed a traffic study is not
requil"ed for this structure.
8. The parking plan is the result of detailed studies by the Bethesda Hospttal as
requosted by the City and has been designed as the best method to meet the parking
demond that is generated by the Hospital. The parking garage is a physical
manifestation of the plan. We believe that additional parking at the Hospital will in the
long term help the nearby neighborhoods by providing adequate parking on site with
clear signage.

Dear Mr. Breese:
This letter is in reference to our December 2,2003 meeting with you and Kevin Halahan.
Our office had prepared a response to your letter of November 19, 2003 regarding the
need for additional information, clarification and adjustments to our previously submitted
plans. Those plans accompany this letter. At that meeting, Mr. Halahan expressed his
desire that the trees surrounding the parking structure have additional height at the time of
planting.
We are assessing the implications of that request related to the availability of quality
landscape material (Washingtonian and Florida Royal Palms) at that height (35 to 45 feet).
Furthermore, we are studying the existing standards contained in the City's Environmental
Regulations and any proposed revisions now being prepared related to those standards.
We also continue to research the standards contained within the Palm Beach County
Streetscape Standards Manual for possible planting in the right-of-way of Golf Road.
Therefore, the plans that are being resubmitted on December 4, 2003 do not contain the
specific revisions discussed at that meeting. However, it is our intent to continue to refine
those plans as we assemble and digest the above referenced additional information to
include landscaping material that is over and above that required by code.
It is also our intent that as we assimilate information from various sources, including
meeting and discussing the needs of those residents interested in the project, as identified
by your office, we will continue to amend the plans over time. Additionally, the applicant

will remain flexible throughout the process to accommodate concerns expressed at both
the Planning and Development Board meeting and the City Council meeting.
We are also preparing the west cross section as requested by your office. In order to be
as accurate as possible, we are taking time to be sure that the graphic representations are
realistic, that the distances are scaled properly and that the graphics produced will be at a
reasonable size for presentation. We plan on using that cross-section in our discussions
with interested neighbors as well as during the scheduled public meetings. We anticipate
contacting those neighboring residents identified by your office during the week of
December 8, 2003. We will schedule an appointment with Mr. Halahan prior to those
meetings to review the cross section, if necessary.
Also, we have also prepared additional graphics to show at the Planning and Development
Board, City Council and in any meeting with individual local residents. These graphics
include an aerial photograph, mounted on a board; with the outline of the hospital property
and the footprint of the parking structure highlighted showing their relationship to the
surrounding residential structures.
Our office appreciates your assistance in these matters and we will keep on working on
these issues up to, during and until the Planning and Development Board Meeting, City
Council and in any meeting with individual local residents. Thank you for your attention to

Dear Mr. Breese:
We have prepared responses to your review of the additional/revised pages submitted for
the above referenced application. These comments were included in your letter of
November 19,2003.
1. The Site & Drainage Plan and Signage and Marking Plan continue to reflect a
deficient backing distance for the parking spaces at the Early Learning Center in
relation to the landscape island in the drop-off area.
Response: All plans have been modified and reflect sufficient backing distance for
the parking spaces at the Early Learning Center. All plans now use the same
dimensions relating to the landscape island in the drop-off area.
2. All of the landscaping can remain in those areas that do not interfere with the H/C
aisles and street crosswalks, either through segmented planting areas, trees with
grates or through a combination of the two. Your response also indicated a conflict
with a covered drop-off, which has not been reviewed by this office.
Response: We have proposed the installation of smaller palms within tree gates
along this area of new sidewalk and along with other segmented planting areas. The
covered drop-off referred to in the previous submittal actually related to the covered

walkway between the proposed parking garage and the main hospital building. It
proved not to be a significant obstacle in achieving our mutual goal of establishing
an accessible landscaped area.
3. The paver/turf block access off of 24th Avenue should be eliminated. This has been
confirmed with the Fire Department. They would however like to retain the gated

Response: We removed the paving material from the access drive as requested by
the Fire Department. A gate, not capable of allowing vehicle headlights to shine
through, is proposed for the opening in the wall to allow fire department access.
Likewise, no trees will be planted in the footprint of the now deleted paver drive. We
also re-arranged the extreme southern edge of the parking area to allow for
additional landscape screening in relation to the access drive and landscaping
screening for the parking garage itself.
4. No surveys are attached to the 10 sets of plans we have in the office. They should
be submitted with the other sheets being revised and to be inserted into the plan

Response: We have included a full set of signed and sealed surveys with this plan

structure in relation to surrounding residential structures, on a mounted board, for
the required hearings.
Response: We have prepared an aerial photograph, mounted on a board, with the
outline of the hospital property and the footprint of the parking structure highlighted
showing their relationship to the surrounding residential structures. We plan to use
this graphic during our presentation before the Planning and Development Board,
City Council and in any meeting with individual local residents.

Staff has reviewed the additional/revised pages submitted for the above-referenced application and
concludes as follows:
A. The Site & Drainage Plan and Signage & Marking Plan continue to reflect a deficient
backing distance for the parking spaces at the Early Learning Center in relation to the
landscape island in the drop-off area.
B. All of the landscaping does not need to be removed from the island along the realigned SW
26th Avenue. Landscaping can remain in those areas that do not interfere with the HIC aisles
and street crosswalks, either through segmented planting areas, trees with grates or through a
combination of the two. Your response also indicated a conflict with a covered drop-off,
which has not been reviewed by this office.
C. The paver brick/turf block access off ofSW 24th Avenue should be eliminated. This has been
confirmed with the Fire Department. They would however like to retain the gated opening in
the wall for emergency purposes. The gates must totally screen any vehicle headlights from
shining into the residential neighborhood. Additionally, no trees should be planted in the
footprint of the now deleted paver drive.
D. No surveys are attached to the 10 sets of plans we have in the office. They should be
submitted with the other sheets being revised and to be inserted into the plan sets.

Please find below the responses to latest comments dated November 13, 2003.
We are also delivering ten (10) sets of revised plans to you along with this
transmittal letter. Also, we have a scheduled meeting with Kevin Hallahan, the
City's Horticulturist on November 17, 2003 to examine issues related to the
relocation of trees on the subject property. This meeting was scheduled prior to
the receipt of your letter.
A. The plans include changes not totally related to staff comments, which
represent removal of landscaping, conflicts with code, and increased
impacts on adjacent properties. These issues are summarized as follows:
1. The landscape island in the center of the drop-off area for the
daycare has been modified such that the parking immediately west
of it no longer has adequate backing distance.
Response: See revised Site Plan.
2. The landscaping originally abutting the handicap parking on the
north side of realigned roadway has been totally removed.
Response: This landscaping was removed to give extra room
for wheelchair accessibility. The proximity of this area to the
hospital's covered drop off area is another factor that impedes
the addition of landscaping in this particular area.
3. A new paver brick access has been added to the plan off of SW
24th Avenue and an opening created in the buffer wall.

Beach's Fire Department. Per that meeting, the new paver
brick has been added to the plan to comply with the City of
Boynton Beach's Fire Department code regulations.
B. Continued deficiency in satisfying recommended landscaping
improvements necessary for the proposed structure:
Response: See revised Site Plan. Planter boxes attached to parking
garages have a long history of being problematic and do not provide
significant aesthetic enhancement to the structure.
C. Lack of Irrigation plan for entire project; and
Response: In the City of Boynton Beach's Land Development
Regulations under Site Plan Review, Section 7, it does not state that
an Irrigation Plan is required. Furthermore, Section 7.C.3. under
Landscape Plan, it states "A notation on method of irrigation" is
required.
D. Lack of irrigation an adequate plan for managing/preserving existing trees
on site.
Response: A sheet has been added to show how the managing and
preserving of existing trees on site will be handled. Specifically,
these can be found on pages LP-4.4 under Preservation Relocation
Notes and Specifications.

2. The "Car County Summary" on Sheet A-1.1 does not accurately reflect the
total of each level of parking when the labeled number for each row is
tallied. The ground floor and roof level counts appear to be off by one
space. On Sheet A-1.1, it would appear that the four (4) spaces at the
eastern jog of the garage may actually be five (5). On sheet A-1.4, it
would appear that the eight (8) spaces along the southern wall may
actually be nine (9).
Response: The revised Sheets A-1.1 and A-1.4 are attached to show
the correct total of each level of parking.
Also attached are the comments from the City Forester that detail issues
surrounding the landscape plans that require addressing by the applicant.
The applicants need to inform us as to how they wish to proceed at this time.

Sheet LP-1.3. LP-2.3
3. The proposed Washington Palm trees should be changed to Florida
Royal Palm trees installed at a gray wood height that will have the
tree canopy at the 35'-45' height level. These Florida Royal Palm
trees can be installed in a single row rather than in cluster in an
effort to break up the large expanse of the garage structure. This
design should visually obstruct the third and fourth levels of the
facility from view of the surrounding neighborhoods. The applicant
should evaluate a combination of these three steps to create a
more immediate visual tree buffer to the facility north, east and west
elevations shown on sheet LP-2.3.
Response: It is the applicant's intent to address the City's
concerns as it relates to appropriate screening of the
proposed parking structure. We believe that the combination
of Washingtonia and Sabal palms provide a higher quality
landscape screen, especially as it relates to how the structure
is or is not seen from adjacent properties. The benefits of this
combination are increased foliage area at multiple heights,
eventual larger palm canopy area for the Washingtonia variety
and a softer - more textured - appearance to the trunks. With
Royal Palms, the canopy is only effective at the upper level of
the parking structure and is ineffective below its canopy. In
fact, the trunks of a Royal Palm closely resemble concrete

pillars. However, we do acknowledge the stately nature of the
Royal Palm and think an array of Royal Palms along the north
side of the structure provide the greatest aesthetic benefit to
the general public. We would like to discuss this idea further
with you to see if we can find the best method to meet our
mutual goal.

(Tibochina granulosa) at the ingress/egress area the site. These
trees must meet the minimum size specifications for trees.

possible changes. This extra (3rd review) is not covered in the current process nor application fee,
and will be assessed based on the hourly rate, plus benefits for the respective reviewers.

Dear Mr. Rumpf:
The following statement has been prepared to address the issue of traffic generation
related to the parking garage that is proposed to be built on the Bethesda Memorial
Hospital campus in Boynton Beach, Florida. This parking structure will be located
on an existing surface parking lot that is located south of Golf Road and north of the
existing hospital building.
Although the parking structure will result in an increase in the number of parking
spaces on the overall hospital campus, these additional parking spaces do not
inherently generate traffic. Rather, they are being provided to accommodate existing
parking demand generated by the hospital in addition to demand generated by
currently pending and potential future expansions to the hospital facility. A previous
parking study performed in 1999 by Kimley-Horn and Associates, Inc. documented
that the existing parking capacity at the hospital was inadequate to efficiently
accommodate parking demand during peak periods at the hospital. These additional
parking spaces will accommodate some of this excess demand. No additional access
connections are proposed to be made to extemal roadways such as Golf Road or
Seacrest Boulevard.
Therefore, the proposed parking garage is not expected to impact the traffic
generation potential of the overall hospital site. Should you have any questions

As you are aware, Bethesda Memorial Hospital is in the process of planning a new
multilevel parking structure to support the current and future parking demand
requirements at the hospital. Rich and Associates has been retained to provide
design and engineering services for this project. In planning for the proposed
parking structure Rich and Associates investigated zoning requirements that
pertain to the parking geometry required for this project. After reviewing those
zoning requirements for both the downtown (CRA) and the suburbs of Boynton
Beach we realized that there were two different parking standards. . The hospital is
in the area of the city covered by the standard parking requirements that calls for 9
ft. 6 inch wide parking stalls that are 18 ft. 6 inches long with a 25-100t wide drive
aisle for 90-degree two-way traffic. The parking geometry for the same traffic
condition in the downtown area would require only a 9 ft. wide parking stalls that
are 18 ft. long with a 24 foot wide drive aisle.
It is our desire as a representative of Bethesda Memorial Hospital to request an
administrative exception to the zoning ordinance to allow us to utilize a
compromise between the two different standards that are currently utilized in the
city of Boynton Beach. The attached diagrams illustrate our proposal for parking
geometry of the proposed parking structure.

Our rationale for this request is based on the fact that the upper levels of the
proposed parking structure will be primarily used by employees of the hospital
which is no different than a downtown employee. The traffic patterns of
employees would require a less generous parking space since there is less
movement of vehicles in this area during the course of the day. The ground level
of the proposed parking structure would be utilized for patient and visitor parking.
In this area we would propose striping a more generous space as defined by the
city code since traffic movement in this area would be more frequent and used by
a more elderly population than that of the employees. In this area we would
comply with the requirements of the code for the majority of the floor with some
spaces having a minor intrusion of the column at the front of the space.
As parking consultants with over 40 years of experience at hospitals across the
country we are confident that the parking geometry we have outlined on the
attached diagrams would provide an exceptional level of service for both the
employees and the patients / visitors who would be using the proposed parking
structure. We hope that you will grant the hospital the administrative exception to
the zoning code that we have outlined above.

Bethesda Memorial Hospital Parking Structure
Major Site Plan Modification
BackQround/Reauest
The applicanVproperty owner Bethesda Memorial Hospital, Inc., is requesting approval
of a Major Site Plan Modification to the existing Hospital site plan. The overall subject
property is located at the southwest quadrant of Seacrest Boulevard and S.W. 23rd
Avenue (Golf Road). The entire site is 24.22 acres in size and specifically, the site plan
modification only pertains to approximately 3.2 acres of land area located on the north
portion of the overall site.
The entire site is zoned Public Use (PU) and possesses' a Land Use designation of
Public-Private GovernmenVlnstitutional (PPGI). On October 15, 2002, the City
Commission approved a Land Use Amendment (Ordinance No. 02-052) from Low
Density Residential (LOR) to Public and Private Governmental/Institutional (PPGI)
together with a rezoning (Ordinance No. 02-053) from Single Family Residential (R-1-
AA) to PU (Public Usage) for a 1.43 acre parcel located on the west perimeter of the
site.
The Hospital is proposing a new parking structure on an existing surface parking lot
located south of S.W. 23rd Avenue (Golf Road) and north of the existing hospital
building. Additionally, the hospital is proposing to realign the access road near the
emergency room, S.W. 26th Avenue. This road will be realigned because of the bend
and swells in the road, which will make it safer for drivers in the future. Furthermore,
once the road is realigned the parking along this road, which is mostly handicapped, will
be relocated to the other side of S.W. 26t11 Avenue. Relocating this handicap parking
will elevate some of the built up congestion.
Parkina Structure
This parking structure will be approximately 38 feet high and will not exceed the City of
Boynton Beach's maximum 45 feet height requirement. This structure will include
ground level and three supported levels for a total of four tiers, which has been
illustrated on the architectural plans provided by Rich & Associates. The ground level
will be reserved for patients only. At this ground level, larger stalls (9'-6" x 18'-6") will be
provided for the patient's convenience, stalls. The upper remaining levels will be

designated employee parking only. The stalls on the upper levels will comply with the
City's parking stall standards of 9'-0" x 18'-0". The parking structure will contain a total
of 880 parking spaces. The following is a parking count summary chart of the different
parking spaces:

PARKING COUNT SUMMARY
With regard to Traffic Impact, a Traffic Statement prepared by Kimley-Horn and
Associates, Inc. dated August 19, 2003 is included with this application. As indicated in
this statement the proposed parking structure and the additional parking spaces are not
anticipated to impact the traffic generation of the overall hospital site. Therefore, this
request complies with the Countywide Traffic Performance Standards.
This proposal is consistent with Chapter 23: Parking Lots: Article II. Required
Improvements of the City of Boynton Beach's Land Development Regulations as
follows:
Illumination: A Photometrics Plan has been provided by Kimley-Horn and Associates,
Inc. to show the lighting requirements.
Traffic Control: The proposal is consistent with all the applicable standards in this
code.
Landscaping: A Landscape Plan has been provided by Kilday & Associates, Inc. to
show the existing and proposed landscaping around the proposed parking structure.
Irrigation: The proposal is consistent with Chapter 7.5 Article II. Landscape Code.
Curbs and car stops: The proposal is consistent with all the applicable standards in
this code.
Drainage: A drainage plan and a drainage statement has been provided by Kimley-
Horn and Associates, Inc. to explain the drainage design. This design is within the

applicable standards of Chapter 6, Article IV, Section 5 of the City's Land Development

Driveway: The proposal is consistent with all the applicable standards in this code.
Parking lot layout: The proposal is consistent with all the applicable standards in this
code.
Fire lanes: The proposal is consistent with all the applicable standards in this code.
Parcel pickup or customer drop off lanes: The proposal does not pertain to this
item.
Sidewalks: The proposal is consistent with all the applicable standards in this code.
Parking lot pavement: The proposal is consistent with all the applicable standards in
this code.
Stacking space at drive-up openings: The proposal does not pertain to this item.
This proposal is consistent with the City of Bovnton Beach's Comprehensive
Plan: The proposed use complies with the goals, objectives, and policies of the
Comprehensive Plan including Objective 1.18.
Based on this Justification Statement and the proposed Site Plan, Kilday & Associates,
Inc. respectfully requests approval of this Major Site Plan Modification to add a parking
structure and to realign S.W. 26th Avenue on site. The Kilday & Associates, Inc. Project

notes on plans to confirm response to general statements/ comments, and bring 10 copies
