Dear Mr. Champagne:
In response to your request dated September 4, 2003 for the
administrative review and approval of the modifications proposed to
the above-referenced approved site plan, depicting the following:
· Modification of approved landscape plan, substituting Royal
Palms, reducing the gray wood on the Royal Palms from 6 & 12
feet to 5 & 10 feet and reduction of the viburnum hedge on the
perimeter of the parking lot from 36" to 30" (as depicted on the
attached drawing) .
Please be informed that the proposed changes on the revised plan
date stamped 10/15/03 are "minor" as defined within the Land
Development Regulations, Chapter 4- Site Plan Review. This project
may continue to be processed by the Building Division as a permit
application, subject to the following conditions:
1. If the scope of the project demands further changes or removal of
plant material, then such modifications are subject to the approval
of the City Forester.
Be advised that the proposed changes may require a modification to

Dear Mike:
This letter will serve as a request for a Minor Modification to the approved landscape
plans for the Boynton Beach Medical Arts Pavilion. Attached you will find the requisite
filing fee of $100.00 and four (4) sets of signed and sealed landscape plans. The
following will summarizes the changes to the approved plans.
The plant list on the revised landscape plan shows the quantities from the original plan
and the revised quantities to document the changes. The changes in the palm category
include the addition of Royal Palms on the east side of the buildings and at the S.E. 1 st
Place entrance. These Royal Palms replace the two Phoenix reclinatas and two triple
Phoenix roebeleniis shown on the September 4 request for modification. The sizes of
the Royal Palms are reduced from six and twelve feet of gray wood to five and ten feet
of gray wood. Most of the installed palms exceed this size with the exception of three of
the nineteen palms. The size of the viburnum hedge located on the perimeter of the
parking lot was reduced from thirty-six inch (36") to thirty inch (3D").
Overall, the revised plan exceeds the required trees for the site. The plant substitution
is in my view an upgrade to the previously approved plan and we hope you and your
staff will agree. Please call if you need additional information.
