Dear Mr. Champagne:
In response to your request dated September 4, 2003 for the
administrative review and approval of the modifications proposed to
the above-referenced approved site plan, depicting the following :
. Modification of approved landscape plan, substituting plant
material consistent with the design theme existing on Seacrest
Boulevard and to avoid conflicts with the existing FPL power
line located on the south side of the entrance drive (as
depicted on the attached drawing) .
Please be informed that the proposed changes on the revised plan
date stamped 09/04/03 are "minor" as defined within the Land
Development Regulations, Chapter 4- Site Plan Review. This project
may continue to be processed by the Building Division as a permit
application, subject to the following conditions:
1. If the scope of the project demands further changes or removal of
plant material, then such modifications are subject to the approval
of the City Forester.
Be advised that the proposed changes may require a modification to

Dear Mike:
This letter will serve as a request for a Minor Modification to the approved landscape
plans for the Boynton Beach Medical Arts Pavilion. Attached you will find the requisite
filing fee of $100.00 and four (4) sets of signed and sealed landscape plans. The
following will summarizes the changes to the approved plans.
The plant list on the revised landscape plan shows the quantities from the original plan
and the revised quantities to document the changes. In the tree category, the Yellow
Elder and Royal Poinciana are deleted. The Yellow Elder, located along Seacrest
Boulevard on the south side of the entrance, was replaced with Royal Palms to match
the north side of the entrance creating a consistent design theme for the Seacrest
frontage. This substitution was made because the existing FPL power line located on
the south side of the entrance drive on Seacrest was removed making it possible to
plant the Royal Palms. Four additional Live Oaks were added in the parking lot
replacing the Yellow Elder.
The changes in the palm category include the deletion of all of the Montgomery palms.
These palms are replaced with two Phoenix reclinatas which is a cluster palm with
multiple stems, two triple Phoenix roebeleniis and five Royal Palms previously mention.
The Royal Palms originally located on the east side of the building have been relocated

In the shrub and groundcover category, three b:nty-eight (328) additional shrubs and
groundcovers are added to the plan mostly due to the reduction in the plant spacing for

the shrubs. Orange Jasmine replaces the podocarpus hedge around the building and
the Schefflera variety "Trinette" replaces the Ixora "Petite Pink" along Seacrest. Three
Bougainvillea Standards were added to meet the Signature plant criteria replacing the
Yellow Elder. Annual beds were also added to the building entrances.
Overall, the revised plan exceeds the required trees for the site. The plant substitution
is in my view an upgrade to the previously approved plan and we hope you and your
staff will agree. Please call if you need additional information.
