Dear Mr. St. Louis:
In response to your request date-stamped April 7, 2005 for the
administrative review and approval of the modifications proposed to
the above-referenced approved site plan, depicting the following:
· Modification of parking to utilize stabilized parking spaces for
21 of the previously approved 49 asphalt spaces (as depicted on
the attached plan) .
· Elimination of existing window openings on the upper portion of
the east elevation.
· Reduction of proposed window openings to 20" wide X 50" tall
windows.
· Elimination of two overhead door openings and reduction of
opening to 20" wide X 50" tall windows.
Please be informed that the proposed changes on the revised
date stamped 4/7/05 are "minor" as defined within the
Development Regulations, Chapter 4 Si te Plan Review.

Stabilized Parking Spaces
Dear Mr. Breese;
The approved site plan indicated a total of 48 parking spaces, of which two are handicap.
According to the City of Boynton Beach Land Development Code, Chapter 2, Sec. ll-A-6, 50%
of the parking spaces may be stabilized. It is proposed to provide 21 stabilized parking spaces as
shown on the revised site plan, four sets, submitted for your review and approval.
Attached is a check in the amount of $100.00 payable to the City of Boynton Beach.

To Whom It May Concern:
George A. Tibedo, Inc., has prepared the following plan modifications on behalf of the project contractor
relative to renovations and modifications taking place at the referenced address. Our findings and
recommendations are as follows:
Item 1: Small Window Infill
Several existing window openings are being infilled in the upper section of the east exterior 8" cmu wall.
The existing openings measure approximately 54" wide x 24" tall. Infill shall include the use of 8" cmu
with #5 vertical dowels lapped and grouted into existing solid courses at no wider than 48" o.c., or two per
infill (min.). Reinforced cells shall be grouted full. (See Typical CMU Infill Detail)
Item 2: Reduced Window Openings
Several existing window openings are being reduced in size, to 20" wide x 50" tall. The infills shall use 8"
cmu with #5 verticals lapped and grouted into solid coursings, placed at the end cells and jamb cells, and at
spacings not to exceed 48" o.c. Reinforced cells shall be grouted full. (See Typical CMU Infill Detail)
Item 3: Large Opening Infill
Two large overhead door openings are to be infilled to include smaller 20" x 50" window openings. The
infill shall be 8" cmu reinforced with #5 verticals lapped and grouted into solid coursings top & bottom.
Reinforced cells shall occur at the end cells, jamb cells and at spacings not exceeding 32" o.c. Reinforced
cells shall be grouted solid. (See Typical CMU Infill Detail)
Item 4: Grillage Platform
The baptismal dais platform shall be constructed on the existing 5" concrete floor slab using 2x12 lumber
grillage at 12" o.c. in two layers of opposing span direction, sheathed with %" CDX plywood. Provide
continuous 2x12 facia joist/blocking around perimeter of each grillage layer. Finished platform height
shall be approximately 24" above finished floor elevation. Crossing 2x12 joists shall be connected to
joists below using Simpson H2.5 galvanized anchors, nailed per manufacturer's instructions.
Item 5: Accessible Restrooms
We understand that the two restrooms added to the north end of the facility have been adjusted to meet
accessibility dimensional requirements. Specifically, the fixture placements have been installed to the

West Restroom:
Wall to First Fixture = 18"
First fixture to Second Fixture = 18"
Second Fixture to Third Fixture = 38" (accessible stall)
Wall to First Fixture = 20"
First Fixture to Second Fixture = 69" (accessible stall)
East Restroom:
Item 6: HV AC Platform
The owner wishes to build an HV AC access platform above the ceiling of the music storage room to
support two small air handlers. (See attached Framing Plan and Details).
To the best of our professional knowledge and experience the referenced modifications meet applicable
Codes and proper Standards of Care. We request the Building Official accept these modifications as
amendments to the permit documents.
