Please be informed that the proposed changes on the revised plan date stamped 10/31/03 are "minor" as defined within the Land Development Regulations, Chapter 4- Site Plan Review, This project may continue to be processed by the Building Division as a permit application, subject to the following conditions: 1. If the scope of the project demands removal of plant material, then such plant material must be relocated to an area subject to approval of the City Forester; 2, The Spanish Stopper hedge must be planted with branches touching to produce an instant visual barrier and a Live Oak tree comparable in size to the one immediately east of this enclosure shall be planted on the west side of the enclosure, Be advised that the proposed changes may require a modification to the building permit, Please contact me at (561) 742-6260 if you have additional questions. Box 310 Boynton Beach, FI 33425-0310 Re: Medical Arts Professional Center - Open Imaging Tenant 2300 Congress Avenue Request for Minor Site Plan Modification Dear Mr. Rumpf, The above tenant space is currently permitted and under construction. The above tenant specializes in radiology services. The majority of the equipment is self-contained within the space or screened by the existing roof parapet. However, the MRI units do require some equipment to be located outside, which will require screening as discussed at a pre-application site plan meeting on September 22, 2003, The following three pieces of equipment will require screening; an air cool chiller, an emergency generator, and a liquid gas propane tanle The proposed screening will be a combination of a 6'- 6" high dark green vinyl coated chain link fence and native plant material. The capacity of the LOP tank will not exceed 495 gallons. The most economical site location for the equipment is the southeast comer of the building as shown on the modified site plan. Three (3) of the eighteen (18) excess parking spaces currently on the approved site plan will be deleted under this modification request. Please find attached four copies of the proposed modified site plan, equipment cut sheets, and color photographs of the area to be modified.
