RIDER TO SITE PLAN APPLICATION
The undersigned as applicant for Final Site Plan Approval does hereby acknowledge, represent and agree that
all plans, specifications, drawings. engineering, and other data submitted with this application for review by the
City of Boynton Beach shall be reviewed by the various boards, commissions. staff personnel and other parties
designated. appointed or employed by the City of Boynton Beach, and any such party reviewing the same shall
rely upon the accuracy thereof, and any change in any item submitted shall be deemed material and
substantial.

NOTICE TO APPLICANTS FOR APPROVAL OF LAND DEVELOPMENT ORDERS OR PERMITS
Please be advised that all applications for the following land development orders and permits which are
submitted on or after June 1, 1990 will be subject to the City's Concurrency Management Ordinance, and
cannot be approved unless public facilities (potable water, sanitary sewer, drainage, solid waste, recreation,
park, and road facilities) would be available to serve the project. consistent with the levels of service which are
adopted in the City's Comprehensive Plan:
Building permit applications for the construction of improvements, which, in and by themselves, would
create demand for public facilities.
Applications for site plan approval.
Applications for conditional use approval.
Applications for subdivision master plan approval.
Applications for preliminary plat approval.
Applications for final plat approval.
Applications for rezoning to planned zoning districts.
Applications for revisions to any of the applications listed above, which would increase the demand
for any public facility.
Any other application, which, in and by itself. would establish the density or intensity of use of land, or
a maximum density or intensity of use of land.
Applications for development orders and permits submitted after February 1. 1990 and which
generate more than 500 net vehicle trips per day, must comply with the Palm Beach County Traffic
Performance Standards Ordinance, unless exempt from that ordinance.
Please be advised, however, that the following applications will be exempt from the Concurrency Management
Ordinance, pending final approval of this ordinance by the City Commission:
Applications for the development of property which was platted on or after January 13, 1978 and either
the final plat or the preliminary plat and Palm Beach County Health Department permit applications were
submitted or approved prior to June 1, 1990. and the use of the property is consistent with the general
use which was intended for the property at the time of platting.
Applications for the development of property, which was platted prior to January 13, 1978, the area of
the platted lots does not exceed 2 acres. and the proposed use would not generate more than 500 net
vehicle trips per day.
Applications for building permit, if a site plan or conditional use application was submitted prior to June
1. 1990 and subsequently approved and the site plan or conditional use has not expired.
Applications for the development of property within an approved Development of Regional Impact, and
which are consistent with the approved DRI.
Applications for approval of final plats, if the preliminary plat and application for Palm Beach county

Section 7. Submission Requirements.
Each applicant shall submit to the Planning and Zoning Division the following plans and exhibits in the number
of copies specified by the Planning and Zoning Division, together with a Site Plan Review application and a fee
adopted by resolution by the City Commission.
12 ASSEMBLED COPIES REQUIRED
A. Existinq site characteristics map: A sealed survey. not older than six months, showing all adjacent
streets, alleys and driveways, and also illustrating:
1. Existing natural features. including but not limited to lakes, trees and other vegetation
and soils and topography.
2. Existing buildings, building elevations, other structures, including use, height, dimensions
and setbacks.
3. Existing utility lines and all easements.
4. Existing elevations (corner. street and finished floor)
B. Site development plan:
1. A scaled drawing clearly illustrating proposed buildings and other structures, and any
existing buildings and structures. which are to be retained, including use, height,
dimensions and setbacks.
2. Proposed off-street parking spaces, driveways and sidewalks, including location,
dimensions and setbacks, traffic control markings and signage.
3. Proposed fences and walls, including location. dimensions. setbacks. height and
material.
4. Proposed location of lighting on site.
5. Proposed dumpster location.
C. Landscape plan:
1. A separate scaled drawing (at the same scale as the site development plan) prepared as

Beach.
The Traffic Impal:I AnaJysis lias been pon:formed in accordance with the standards and criteria as set forth
in the Palm Beach CouD1y UIJified Land ~ Code, Article IS, TrafIic PcrformaDI:e Standards.
The scope of the study is to:
i) Delermine the volume of l1lIffic volume to be added to the tborougJJfiu'c system by the
proposed Medical Arts Professional Center.
ii) Assign the pIl!ject trips to the tborougJJfiu'c system witbin the Radius of Development
Inl'lvence. Determine the impal:t of the additionall1llffic on the capacity of the

Drainage Statement
1. INTRODUCTION
1.1 The existing site is 3.917 acres in area and is located north of Golf Road
(SW 23'd Avenue), east of Congress Avenue, within the corporate city
limits of Boynton Beach, Florida, lying in Section 32, Township 45 South,
Range 42 East.
1.2 The existing site is currently undeveloped, relatively flat in contour, and
vegetated by field grasses. No trees existing on-site.
1.3 The existing site currently pools on-site until it runs off to the west into the
existing Congress A venue drainage system.
1.4 The project lies within the C-16 Basin of South Florida Water Management
District, as well as the jurisdiction of the Lake Worth Drainage District.
1.5 Upon completion, the site will accommodate a 42,824 square foot medical
ot1ice building, together with the associated parking and landscaped areas.
The building fronts Congress Avenue with secondary access from Golf
Road.

2. DRAINAGE CRITERIA
2.1 Drainage for the proposed Medical Arts Building will com form with all
rules, regulations, codes, etc. including, but not limited to the City of
Boynton Beach, Florida Land Development Regulations Chapter 6, Article
IV, Section 5.
a. City of Boynton Beach, Florida. The project will detain the
runoff from a 3 year/ I hour storm or the first I" of runoff,
whichever is greater. No discharge from the site will occur from the
25 year/3 day rainfall event except by means of the control structure
(The site perimeter be will be bermed up to the 25 year/3 day storm
for the C-16 basin).
b. Lake Worth Drainage District & South Florida Water
Management District. Both agencies require controlled discharge
based on the 10 year/3 day storm event. The allowable discharge for
the C-16 basin is 62.6 CSM (CFS/square mile). The peak allowable
discharge for this site is less than I CFS. The minimum bleeder
size, as per the Lake Worth Drainage District, is a 6" high x 6" base
inverted triangular bleeder. The water quality criteria for the South
Florida Water Management District is the detention of the runoff for
the 3 year/1 hour storm or the first I" of runoff, whichever is greater.
2.2 Existing basin storage will be maintained post development.

3. DRAINAGE REQUIREMENTS
3.1 The proposed development will be designed to have its runoff collected by
a network of underground pipes and collection basins for detention in an
extiltration system and conveyance through a control structure which will
then monitor the outfall into the existing Congress Avenue drainage
system.
3.2 The perimeter of the site will be mounded to ensure that the 25 year/3 day
storm runoff does not overflow the site.
