Augustine, P.E. 2755 South Federal Hwy., Suite 11 Boynton Beach, Florida 33435 Re: LWDD Proj. #89-986D.Ol, Rouse Building (addition), E-3 1/2-E Dear Mr. Augustine: We have reviewed the drainage plans for the above referenced development and do hereby approve of them for connection to our facilities, subject to the following conditions: At the time of installation a benchmark will be required on the top of the water control structure(s) with the elevation clearly defined. ( ) Developer will reconstruct canal(s) to approved design section along projects limits, including clearing and proper sloping of the maintenance bermes. ( X) Developer will obtain any and all permits required by DER/Army Corps of Engineers, S.F.W.M.D., Palm Beach County and/or any municipality that may be involved prior to the commencing of any construction. ( X) Developer will take all reasonable precautions necessary to prevent turbidity or silting upstream or downstream during construction. ( X) Developer will contact the LWDD Inspection Department to schedule inspections at least 48 hours in advance. Failure to do so may result in rejection of the work until visual inspection can be made. Work must be uncovered for inspection. X Developer must submit "record drawings" within Sixty (60) days of receiving final approval. ( X) The Lake Worth Drainage District will not accept any water from dewatering either on or offsite until written notification of approval from S.F.W.M.D. has been submitted to this office. ( Where improvements are to be erected on lots or parcels contiguous to Lake Worth Drainage District canals, developer shall install gutters and down spouts eliminating surplus water overland flow, assuring the routing of said water into the on-site catchment facility and/or the storm sewer system. Bond will be released upon issuance of final inspection by LWDD and developers submittal of "record drawings" to LWDD. ( X) Developer agrees that significant construction must start within one year of the date of permit issuance or this permit will become void and a new application must be applied for prior to any construction activity on site. In consideration of and by acceptance of the permit issued by the LWDD, the undersigned agrees to perform the above and further agrees to hold the LWDD, its agents and employees harmless for any damage emanating out of or resulting from the permit herein granted.
