Dear Ms Rundquist:
The City of Boynton Beach Development Order for site plan approval for the above
referenced project, consisting of seventeen (17) townhouses on a 1.62 acre parcel, was
approved on August 6, 2002. The necessary public facilities are guaranteed for this
project at the adopted levels of service, provided development commences within
one year of issuance of the Development Order.
. Potable water level of service means the capacity of City potable water
facilities to produce and deliver not less than two hundred (200) gallons of
potable water per capita per day.
. Sanitary sewer level of service means the capacity of City sanitary sewer
facilities to treat and dispose of not less than ninety (90) gallons of sweater
per capita.
. Drainage level of service means that drainage facilities for development
projects shall be designed for a three-year storm for a duration of the time
of concentration for the watershed, for development projects which are
subject to South Florida Water Management District design and orl
permitting requirements and mean that drainage facilities for all other
development projects shall be designed to accommodate the first hour of
a three-year storm on site.
. The Palm Beach County Solid Waste authority has stated in a letter dated
December 18, 2001, that adequate capacity at the adopted 7.2Ibs. of
solid waste per capita per day level of service, exists to accommodate
the County's municipalities throughout the 10 year planning period.
. The Palm Beach County Traffic Division reviewed the traffic study for the
development project and determined that the project meets the Traffic
Performance Standards of Palm Beach County (letter attached).

amendment is adopted prior to forwarding to the Florida Department of Community Affairs and is not reviewed
for compliance with the state and regional plans prior to adoption.
The criteria used to review Comprehensive Plan amendments and rezonings are listed in Article 2, Section 9,
Administration and Enforcement, Item C. Comprehensive Plan Amendments: Rezonings. These criteria are
required to be part of a staff analysis when the proposed change includes an amendment to the Comprehensive
Plan Future Land Use Map.

WHEREAS, the City Commission of the City of Boynton Beach, Florida has

WHEREAS, the procedure for amendment of a Future Land Use Element of a

In going through your recently submitted application for land us amendment and rezoning for
property located on S.E. 23rd Avenue (Golf Road) in the City of Boynton Beach (PIN 08-43-45-
33-25-002-0000), staff has found several items that were not included. The items listed below,
must be submitted to our offices before we can proceed with the processing of the application:
1. Notarized Agent Consent Form (Form VI, page 9 of the application) signed by the
property owners of record and authorizing you to make application for the
proposed land use amendment and rezoning. The contract for sale and purchase,
included with the application, is not sufficient for this purpose.
2. A comparison of the water demand for development under the proposed zoning
with the water demand under the existing zoning. Water demand shall be
estimated using the standards adopted by the Palm Beach county Health
Department for estimating such demand, unless a registered engineer justifies
different standards. (Item (6) page 5 ofthe application.)
3. A comparison of sewage flows that would be generated under the proposed
zoning or development with that which would be generated under the existing
zoning. Sewage flows shall be estimated using the standards adopted by the Palm
Beach county Health Department for estimating such demand, unless a registered
engineer justifies different standards. (Item (7) page 6 of the application.)

I have checked the site plans and there are no water and sewer comparisons included.
Please have your engineer forward them to me as soon as possible. Also, I need a copy of
the deed to the property, showing you as owner. The property owner notification must be
mailed by Saturday, and prior to that the City Attorney must approve the notice.
I would have simply called, but it is best to have written records.

Dear Sir:
In going through your recently submitted application for land us amendment and rezoning for
property located on S.E. 23rd Avenue (Golf Road) in the City of Boynton Beach (PIN 08-43-45-
33-25-002-0000), staff has found several items that were not included. The items listed below,
must be submitted to our offices before we can proceed with the processing of the application:
1. Notarized Agent Consent Form (Form VI, page 9 of the application) signed by the
property owners ofrecord and authorizing you to make application for the
proposed land use amendment and rezoning. The contract for sale and purchase,
included with the application, is not sufficient for this purpose.
2. A comparison of the water demand for development under the proposed zoning
with the water demand under the existing zoning. Water demand shall be
estimated using the standards adopted by the Palm Beach county Health
Department for estimating such demand, unless a registered engineer justifies
different standards. (Item (6) page 5 of the application.)
3. A comparison of sewage flows that would be generated under the proposed
zoning or development with that which would be generated under the existing
zoning. Sewage flows shall be estimated using the standards adopted by the Palm
Beach county Health Department for estimating such demand, unless a registered
engineer justifies different standards. (Item (7) page 6 of the application.)
