Additional Requirements for Krispy Kreme application-
(olease note that because of the notification requirements. there will not be a soecial
meetinq of the P & D Board. The meetinq will be November 27,)
1. New property owner notification labels with additional property, as shown on the
attached map, included, postage and envelopes (labels attached). Time is of the
essence since the mailing must be ready to go out Friday.
2, Property owners' consent forms signed by all owners of each property,
According to the tax records of Palm Beach County, Salvador H and Norma T
Sentmanat are the owners of record of one parcel. Anthony Mannino is listed on
the tax records as trustee for the owners of the parcel to the west fronting on
Boynton Beach Boulevard, (08 43 45 29 01 002-0090) and is listed as the owner
of the parcel fronting on NW 1st Ave. (08434529010020101) although we
both know the three parcels to the west are under common ownership.
3, Amended application form that shows the actual applicant, if it is not Dynamic
Doughnuts Florida Realty, Inc. The applications should also be amended to
show the acreage covered by the land use application (now 0.62 acres, as
opposed to the original application for 0.26 acres.)
4. Agent Consent Form from Dynamic Doughnuts Florida Realty, Inc. or legal
documentation showing a clear trail from that firm to the actual applicant named
in the amended application. As discussed last night, the consent form signed by
Dynamic Doughnuts of Boynton Beach, Inc. will not suffice.
5, Fees for re-advertising the public hearings in the newspaper will run
approximately $1,200 and will be the responsibility of the applicant.
