Dear Ms. Conover:
In response to your request dated November 4, 2005 for the
administrative review and approval of the modifications proposed to
the above-referenced approved site plan, depicting the following:
. Modification of Site Plan (Sheet SP-l) building footprint on
east side of the Comprehensive Heart Center to match the
approved Architectural and Civil drawings (as depicted on the
attached plans) .
. Addition of a fountain to the Healing Garden along the north and
east side of the Comprehensive Heart Center (as depicted on the
attached plans) .
Please be informed that the proposed changes on the revised
date stamped 11/4/05 are "minor" as defined within the
Development Regulations, Chapter 4 Site Plan Review.

SUBJECT: Proposed Bethesda Memorial Hospital Fountain
Bethesda Memorial Hospital was recently approved for a Minor Modification to their site
plan for a fountain in association with the new Heart Center. Please ensure when the
plans are submitted for permitting that it is noted that a Utility permit is required in
conjunction with the water meter, in case the plan do not depict the installation of the
necessary meter. The fountain will be using potable water and thus requires a separate
meter. The applicant has been verbally advised of this requirement.

Dear Mr. Rumpf:
This letter is to request a Minor Site Plan Modification to the Bethesda Memorial Hospital
Comprehensive Heart Center's recent approved plans by the City Commission on July 19,
2005. The plan has been modified after the City Commission approved the plan and these
changes will be submitted to the Building Department to go through the permitting process.
These all represent minor changes and adjustments, which are clouded on the site plan
and should make for an easier review process.
There are only two (2) minor revisions that our office is requesting to be reviewed.
1. The east facade building footprint has been modified to match the architectural and
civil drawings.
2. A fountain has been added to the Healing Garden on the east side of the proposed
addition.
We would like to request your consideration on this Minor Site Plan Modification and that
the process can be reviewed in a timely manner to be able to continue through the
permitting process.
