In response to your request dated July 11, 2005 for the
administrative review and approval of the modifications proposed to
the above-referenced approved site plan, depicting the following:
. Installation of a mansard roof consisting of "Pacific Blue"
standing seam metal panels (as depicted on the attached plan) .
. Removal of door and stoop (as depicted on the attached plan) .
Please be informed that the proposed changes on the revised
date stamped 7/12/05 are "minor" as defined within the
Development Regulations, Chapter 4 Si te Plan Review.

permit. There has been a change of contractor on this permit from S.F. Construction to
Duane S. Cyr, Inc. We have changed the design of the roof from the approved Mansard
to a Hip roof as shown in the attached plans.
