Dear Mr. Rumpf,
This affidavit is being supplied to affirm that the proposed Verizon Wireless antenna
installation at the above address shall be constructed, maintained and operated in
conformance with the performance standards as outlined in the City of Boynton Beach
zoning code.
Please be advised that a typical Verizon Wireless communications facility consisting of a
tower structure, antennas and base station radio cabinets does not produce following:
Noise and Vibrations- The tower, antennas and radio cabinets do not have moving parts,
only electrical switching equipment which does not produce noise or vibrations. A low
level electrical hum may be detected immediately adjacent to the equipment cabinets, but
would not be detected from adjacent properties or rights-of-way.
Smoke, Dust, Dirt or Particulate Matter - No particulate matter is emitted during the
operation of any given cell site.
Odor and Fumes - No fumes or odors are produced under operations of a cell site.
Toxic and Noxious Matter - No toxic or noxious matter is produced during operation of a
cell site.
Fire and Explosion - No risk of fire or explosion is expected under normal operations of
a cell site.
Heat, Humidity and Glare - No heat, humidity or glare is produced during operations of a
cell site.
Liquid and Solid Waste - A cell site by nature does not produce liquid or solid waste.
Only household level electrical service and telephone line connections are utilized in its
operation.

Electromagnetic interference - Verizon Wireless certifies that the proposed installation

Hazardous Materials, Hazardous Waste, Toxic Substances - No hazardous materials,
waste or toxic substances are produced.

Space Plus Storage facility. The city commission is also scheduled to hear this request on March 1, 2005 beginning at 7:00

To Whom It May Concern:
On February 11,2005 SBA Network Services, Inc. (SBA) conducted an aeronautical
study for Verizon Wireless. The purpose of the study was to determine the effect, if any
the proposed 137 foot AGL/150 foot AMSL structure would have on Federal Aviation
Administration (FAA) airspace. This study was conducted in accordance with FAA Part
77 of Federal Aviation Regulations (14 C.F.R. Subchapter E, Part 77) and Federal
Communications Commission (FCC) Part 17 (47 C.F.R. Part 17).
This document and the opinions expressed herein are for the sole use of Verizon Wireless
in making appropriate regulatory decisions and filings concerning this study site and may
not be reproduced in any form or manner.
FAR Part 77 prescribes the slopes and surfaces at and around public use airports and
heliports. Part 77.13 describes the slopes and surfaces, which, if exceeded, would require
the proponent to file a Notice of Proposed Construction (FAA Form, 7460-1) with the
FAA. FCC Rules Part 17, also describe this requirement. Part 77 Subpart C describes
various surfaces and slopes which, if exceeded would require the FAA to provide public
notice inviting comments prior to issuing a determination. Additionally, Part 77 subpart
C also describes slopes and surfaces, which if exceeded, would require the FAA to
identify the proposed structure as a Hazard to navigable airspace.
The proposed construction site is located in proximity to Boynton Beach, Florida. The
study site coordinates are, Latitude 26029' 53.70", Longitude 080005' 13.20", North
American Datum (NAD) 83. The ground elevation at the proposed site is 14 feet above
mean sea level (AMSL). The proposed structure height will be 137 feet above ground
level (AGL), which will give an overall structure height of 150 feet AMSL. The
proposed site is located 33,401 feet or 5.49 nautical miles (NM) South from the approach
end of runway 33 at Palm Beach County Park Airport, the nearest public use facility for
regulatory compliance purposes. The study site is located 34,366 feet or 5.66 NM South
from the ARP of this public use facility.
SBA determined in this study that the proposed 137 foot AGL/150 foot AMSL structure
would not the FAR Part 77.13 (a)(1) or (2) surface or the FCC slope surface. Therefore,
FAA Notice of Proposed Construction or Alteration is not required. SBA is confident the
proposed structure would be approved upon completion of an FAA's study. The
maximum no FAA notice height is 200 feet AGL/214 feet AMSL.
In accordance with Federal Aviation Regulations and Policies, marking and or lighting
would not be required. The maximum no marking or lighting height for this study site is
200 feet AGL/214 feet AMSL.

Jason, is the RF engineering coverage graphic available in PDF for our
insertion into a powerpoint presentation? Also, just a heads-up that a
Commissioner is concerned about it's proximity to the adjacent
neighborhood, particularly given the land available elsewhere farther
from the homes (i.e.
on school property). You should be prepared to explain how you have
arrived at this site given the opportunities (on adjacent properties) to
be farther from the residences but still within a zoning district
permitting towers.
Yes, he was explained that the tower would comply with the minimum code
requried separation standards. The staff report will also state these
facts.

of SW 30th Ave., about a block east of S. Congress Avenue. I notified you of this inquiry when first received several
months ago. Let me know if you have any late concerns or issues, or if you desire to review the final proposal again.
Consistent with the ordinance for the M-1 district, the proposal is for a stealth tower (flag pole), 135 feet high, and to

A pre-application meeting was held today with representatives for Verizon who desire to erect a new cell tower on the M-1
zoned, Space Plus Storage project property located east of S. Congress Avenue just north of S.W. 30th Avenue
(approximately 300 feet south of Golfview Harbour homes). They had some graphics showing RF data and the voids in
coverage. I asked for additional information including a tower map with distances from other towers and residences to
provide the Siting Review Committee.
We stressed the possibility of a stealth tower, and that it be capable of collocation. I think the best may be a flag pole
without the flag. Meaning the pole is more slender but there is not a large flag to attract more attention to it. The committee
can discuss this further.
The last time this vicinity was the subject of tower talks we were considering the possibility of it being located at the
racquet center proposed for expansion.
What is your (or designee) availability for holding a Tower Committee meeting? Days/times to avoid?

Jim, I think I have solved the mystery. I found the pre-app file for the site at 1314
Neptune Drive. I was not an attendee, nor are there any notes regarding parking status. It
left it open that if site requirements can be met than it's a go.
But I recall meeting with someone else prior to that or about the same time about the
possibility of adding a tower to a partially built site with a site plan called "Sterling
Records Management". This site is just north of Neptune Drive, on the north side of 35th
(the entrance to Caloosa Park). Verizon originally came in with some information that was
unclear and partially pointed toward this site and partially to the Television Station
site on the west side Congress.
The Sterling Records site was approved with no surplus parking spaces, which I may have
based my response to on. So at this time I have to maintain that the 1314 site has the
surplus reported to the property owner in conjunction with his inquiry. Of course that is
contingent upon current uses being consistent with site design, and that the site is
modified through our minor site plan modification process.
