The above referenced project is nearing completion. Prior to the Building Division issuing the
Certificate of Occupancy, I would like to ensure that you do not have any outstanding or pending
conditions concerning this site that must be rectified (excluding issues that are covered entirely
with a City approved surety).
If any outstanding conditions are related to permits issued by the Building Division, please
provide this division a memorandum containing a description of the unresolved conditions. If
the unresolved conditions are not permit related, please notify the owner or the owner's agent to
ensure that the matters are rectified.
Please respond to me, within ten (10) days, in order that we may proceed in a timely manner to
issue the Certificate of Occupancy No response is necessary if your department is satisfied with
the current state of the above referenced site. Thank you for your cooperation.

Dear Mr Sintilien:
On behalf of the administration and staff at Bethesda Hospital, I am providing you with a
copy of the finalized development plans for the Physician Parking Expansion at Bethesda
Hospital. As I indicated at the neighborhood meeting November 4th, 2002, we wanted to
follow up with you regarding the revised plans. For your convenience, we have attached to
this letter, 8 % x 11 copies of the updated plans which include the Site Plan, the Landscape
Plan, Architectural Elevations, a Cross-Section, and a typical photo overlay of the existing
conditions on the site and the proposed structure and landscaping.
As you may recall the initial proposal called for a one-story structure similar in design to a
single-family home. Based on the input we received from City staff and the neighbors, the
structure is still a one-story structure but all openings facing SW 4th Street have been closed
and all sloped concrete panels on the north and south of each building, as well as the slope
and roof on the middle concrete panel of each building have been removed
In addition. I would like to take this opportunity to give you the information regarding the
public hearing schedule with regards to this project. As of January 14, 2003, we have
resubmitted the plans to the city's Technical Review Committee (TRC) for review The
Planning and Development Board meeting is currently scheduled for January 28th at
6:00pm. The City Commission hearing will follow and is scheduled for February 4th, at
6:30pm.
We are notifying the residents of SW 4th Street and SW 5th Street with this mailing. It is our
belief that these property owners will be most affected by the design of this project. If you
should have any questions or concerns regarding any of the documents provided, please

RE. Bethesda Hospital Parking Garage Planning and Development Board Conditions of Approval.
Dear Ms. Gallo;
This letter is in regards to the Planning and Development Board conditions of approval for the project
mentioned above from their meeting of January 28, 2003. The Planning and Development Board approved the
site plan for the parking garages based on staffs recommended conditions of approval and my requested
conditions of approval as modified and supported by the applicant. The following are those conditions;
I That a six-foot high masonry wall will be constructed between the two buildings and setback at least
10' from the west building line (this will replace the vinyl clad chain link fence shown on the
proposed site plan).
2. A six-foot high masonry wall will be constructed from the southeast comer and northeast comer of
the parking garages to the south terminus of the hospital property and connect to the existing wall
north of the parking garages.
3 That no mechanical exhaust equipment be utilized.
4 That a photometric plan be provided to the adjacent residents for their review and consent prior to
submittal for a building permit.
5 That screening such as louvers or Bahama shutters is installed on the garage windows facing the
"park" area between the two buildings only if they meet Building and Fire Code requirements.
6. That a greater variety of tree species is provided.
Please include these Planning and Development Board conditions of approval in your report to the City

RESOLUTION
WHEREAS, the City Commission of the City of Boynton Beach, Florida has adopted Ordinance
No. 02-053, in which a Revised zoning Map was adopted for said City; and
WHEREAS, Section 2.e of said ordinance requires the Bethesda Memorial Hospital Board of
Trustees to adopt a Resolution expressing its commitment not to acquire additional property to
the west, north or south of the property, which is the subject of the Ordinance No. 02-053 (see
attached graphic) for further expansion of the Hospital, support facilities or parking areas; and
WHEREAS, the subject property referred within Ordinance No. 02-53 is described as follows:
Lots 1 through 8, inclusive, Block 1, Seacrest Hills, according to Plat thereof as recorded
in Plat Book 25, Page 74, Public Records of Palm Beach County, Florida.
Containing approximately 1 43 acres ofland.
WHEREAS, Bethesda Memorial Hospital agreed to Section 2.e of Ordinance No. 02-053, at the
August 20th, 2002 City Commission meetinglPublic Hearing.
NOW THEREFORE, BE IT ORDAINED BY THE BOARD OF TRUSTEES OF BETHESDA
MEMORIAL HOSPITAL, THAT
The foregoing clauses are true and correct and incorporated herein by this reference.
Bethesda Memorial Hospital shall not acquire additional land to the west, north, or south of the
property, which is the subject ofthe Ordinance No. 02-053, for further expansion of the Hospital,
support facilities or parking areas.
Approved this 26th day of February, 2003 by a vote of 10 to O.

Dear Mrs. Galav'
Attached are 10 copies of revised plans for the Bethesda Physician Parking Land Use
Amendment and Rezoning. Modifications have been made to these plans to address
outstanding TRC comments and issues presented by residents along SW 4th 51. on
November 4, 2002. The list below outlines each of the modifications that have been made
to the individual plans.
Site Plan/Landsca"e Plans:
· The overall building footprint has been shifted One (1) foot, four (4) inches to the
west, towards the Bethesda Hospital main building. This shift allows for eight (8)
inch architectural projections on the east side of the building, while still maintaining
the required thirty (30) foot setback.
· Two existing ficus trees located on the northern portion of the site, along S. W
West 4th Sf. will be preserved, at the behest of several neighbors. The
preservation of the trees will help bolster the proposed landscape buffer, as well
as provide continued noise and sight abatement from the existing hospital.
· The Landscape Buffer has been modified to include more densely planted native
vegetation. The landscaping was modified to essentially blanket the parking
structure from view and deter any security related issues with the proposed use.
Large canopy trees and extensive masses of shrubs will be used along the
western side of the structure.

. The proposed sidewalk along S. W 4th St. has been modified, so that it meanders
into the site, providing an opportunity to landscape areas between the property
line and the sidewalk for additional buffer Along the northern portion of the site
the sidewalk will remain adjacent to the property line due to the location of the
existing retaining wall.
. A fence, with intense landscaping for screening has been proposed to separate
the neighborhood with the proposed park that is located between the two parking
structures on the site plan. The green area will allow a break in the buildings, while
the intense landscape will continue to obscure any noxious noises and sights
emanating from the back of the hospital. In addition to the extensive landscape
buffer, the fence is proposed to further deter any criminal activity from wandering
onto the site.
Architectural Plans
. At the November 4, 2002 meeting with residents, neighbors along S. W 4th St. were
concerned about light pollution emanating from the parking structure. As a result all
window openings and mullions that were facing S. W 4th St. have been removed on
the elevations.
. All sloped concrete panels on the north and south of each building, as well as the
slope and roof on the middle concrete panel of each building has been removed.
. Concrete tilt panel with decorative Candy Anderson molding, score lines and raised
iron fayade features have been added to the elevation plans.
. The front panels have been raised to provide uniformity along the length of the
buildings.
. A pedestrian walkway has been added midway in each of the buildings floor plans
to connect the garage to the parking lot.
. One parking space has been relocated on the north side of the north building to
better conform with existing site conditions.

These plans also resolve the following comments:

1 When design plans are submitted they will show compliance with these
requirements.

3. As required by City Ordinance automatic fire sprinkler systems will be provided in
both structures.

4 As requested a site development plan and survey showing all existing utilities has
been included.
5. The site plan shows and identifies all necessary traffic control devices.
6. The carport will be constructed using the Palm Beach County Building Code
standards; therefore, only English units will be used.
7 A supplemental 8" Solid Y el/ow Striping at the egress of driveways has been added
to the site plan.
8. Stripping has been added as requested.
9. A drainage plan and drainage statement has been provided for this site.

11 At the time of permit review, a completed and executed City of Boynton Beach Unity

18. The City Commission approved both the small-scale land use amendment and the
rezoning on October 15, 2002.
19 At the time of permit review, a completed and executed City of Boynton Beach Unity
of Title form will be provided.
20 The elevations for all areas of the site have been provided on the drainage plans.
21 A site plan has been provided that is showing the proposed parking structure as it
relates to the existing hospital property
22. To provide better security the parking structure has been sealed off from the
existing neighborhood to the east and a dense landscape buffer will be planted to
deter any criminal activities from entering or exiting the site from the west. In
addition, the parking deck will be well lit from within and openings will be open
enough on the east side of the buildings to allow for high visibility into the structure.
23. The heights for the wood fence have been provided as requested.

To Whom It May Concern.
On behalf of the administration and staff at Bethesda Hospital, I am inviting you to
attend a presentation and workshop regarding the proposed Physician Parking
Expansion at Bethesda Hospital. As I indicated at the recent Planning and
Development Board meeting, the Hospital had previously held two (2) meetings;
including the most affected property owners on Southeast 4th Street.
As the attendance at the Planning and Development Board indicated, interest in this
project extends well beyond the immediate properties on Southeast 4th Street. For
those residents in attendance at the Planning and Development Board meeting, it was
their first opportunity to hear a full presentation in a less than ideal setting.
For that reason, I have been authorized to postpone the August 6, 2002 public hearing
at the City Commission to August 20, 2002 in order to hold another presentation at the
Hospital. At that meeting, we will present the plans which were shown to the Planning
and Development Board the other night. Additionally, we will take input from the
residents as to what, if any, changes could be made to make this project more
acceptable to the neighborhood.
At the Planning and Development Board, a suggestion was made that the land area
along Southeast 4th Street be developed as a pedestrian linear park with amenities such
as lighting, seating and fountains. We will be prepared at the upcoming meeting to
discuss some of these design alternatives.
The meeting will be held at the Clayton Conference Center beginning at 6.30 PM on
August 8, 2002. The best way to get to the auditorium is by entering the hospital at the
Ambulatory Surgery Center located along Seacrest Boulevard. Thank you.

The following traffic statement has been prepared to address the issue of
traffic generation and circulation at Bethesda Memorial Hospital in relation to
the acquisition and proposed rezoning of 1 43 acres on the west side of the
hospital. Currently, the subject property consists of eight separate lots that
are zoned with an R-IAA designation. Four single-family detached dwelling
units are currently located within the subject property It is proposed to
change the zoning designation of the subject property to a PU designation,
consistent with the zoning designation of the contiguous hospital site.
It is also proposed to remove the single-family dwelling units on the subject
property and construct covered surface parking for hospital use. Currently,
access to the single-family dwelling units is provided via driveways on S.W
4'h Street. These structures and access points would be removed, which
would result in an expected decrease in traffic on S.W 4th Street of up to 40
vehicles per day Access to the proposed new parking areas would be
provided from the hospital property to the east. No vehicular access to and
from S.W 4th Street is to be provided. The proposed additional parking is not
expected to generate any additional traffic to the overall hospital property
Instead, the new spaces will provide additional capacity for vehicles already
traveling to the site. A previous parking study performed in 1999 by Kimley-
Horn and Associates, Inc. documented that the existing parking capacity at
the hospital was inadequate to efficiently accommodate parking demand
during peak periods at the hospital. The proposed parking area, which would
be designated for doctor parking, is expected to alleviate some of this existing
parking deficiency

We have been advised by representatives of Kilday and Associates, our Landscape
ArchitectslPlanners, that there is some confusion over the number of parking spaces
available at Bethesda Memorial Hospital (Bethesda).
As we have been informed, information available to you indicates a parking space total of
1,184 Also, as we understand, this information was obtained at the time that Bethesda
was proposing the construction of a remote 148 space employee parking lot on the
northeast comer of Seacrest Boulevard and SE 25th Avenue.
Our Landscape ArchitectsJPlanners have informed you that we currently have 1,148 total
parking spaces. This information is correct and is the result of an inventory of parking
spaces conducted by an outside parking consultant firm in February of this year
The difference between the two totals is the result of state approved major construction,
currently underway at Bethesda. Due to that construction, it was necessary to remove a
total of 36 parking spaces, to allow for the location of construction trailers and other
associated construction material and equipment. At the time that the remote employee
parking lot proposal was before your Division, this construction had not begun.

Attached are 10 copies of the revised plans that will address the TRC comments received
for the Bethesda Physician Parking Land Use Amendment and Rezoning. These plans
resolve the following comments:
Utilities
1 As requested all existing residential utility services shall be cut and capped or
removed at the construction phase of the project, unless otherwise deemed needed
for the project.

irrigation improvements will be submitted for review by the Parks, and Public Worlcs

38. As requested any viable existing native vegetation on the site will be preserved and
shown during permit review. Sabal Palms have been noted to be 12 foot overall
height.

rezoning.

The following traffic statement has been prepared to address the issue of
traffic generation and circulation at Bethesda Memorial Hospital in relation to
the acquisition and proposed rezoning of 1.43 acres on the west side of the
hospital. Currently, the subject property consists of eight separate lots that
are zoned with an R-IAA designation. Four single-family detached dwelling
units are currently located within the subject property It is proposed to
change the zoning designation of the subject property to a PU designation,
consistent with the zoning designation of the contiguous hospital site.
It is also proposed to remove the single-family dwelling units on the subject
property and construct covered surface parking for hospital use. Currently,
access to the single-family dwelling units is provided via driveways on S.W
4th Street. These structures and access points would be removed, which
would result in an expected decrease in traffic on S.W 4th Street of up to 40
vehicles per day Access to the proposed new parking areas would be
provided from the hospital property to the east. No vehicular access to and
from S.W 4th Street is to be provided. The proposed additional parking is not
expected to generate any additional traffic to the overall hospital property
Instead, the new spaces will provide additional capacity for vehicles already
traveling to the site. A previous parking study performed in 1999 by Kimley-
Horn and Associates, Inc. documented that the existing parking capacity at
the hospital was inadequate to efficiently accommodate parking demand
during peak periods at the hospital. The proposed parking area, which would
be designated for doctor parking, is expected to alleviate some of this existing
parking deficiency

Drainage Statement
Bethesda Memorial Hospital is located on South Seacrest Blvd in Boynton
Beach, Florida. The proposed I 43-acre parking lot expansion project is located
west of the hospital and east ofSW 4th Street. Currently, the site contains several
single-family homes, a small park and a maintenance facility
The additional parking areas will drain to a series of inlets connected by an
ex filtration trench. The exfiltration trench will be designed to meet water quality
criteria set forth in the SFWMD guidelines. The proposed stormwater drainage
system will ultimately discharge to the east through the existing stormwater
system on the west side of the hospital property to the historic outfall on the
north side of the hospital. Post development discharge will meet water quantity
criteria set forth in SFWMD guidelines. A No-Notice Environmental Resource
Permit Application will be developed for submittal to the SFWMD for review
and approval for the site.
The drainage will generally conform with the applicable rules, regulations and
codes including Chapter 6, Article IV, Section 5 of the City of Boynton Beach's

Staff has conducted a preliminary review of your application submittals on behalf of Bethesda Memorial
HospitaL There are several issues relating to the applications:
. Even though a single applicant is making the applications, there are two separate and
distinct properties, Area "A" and Area "B" There should be two separate applications
submitted.
. Area "A" is 1 43 acres and can be processed as a small-scale land use amendment;
however, Area "B" is 10.37 acres and must be processed as a large-scale amendment.
The application deadline for the next large-scale amendment cycle is October I, 2002. If
your client wants the two amendment requests to be processed simultaneously, you
should resubmit the applications closer to the October I date. If the client wishes the
small-scale amendment request to go forward at this time, the application should be
resubmitted for that property only
. Staff continues to oppose the subject request for Area. "A", due to its impact on the
adjacent neighborhood, and continues to recommend that alternatives be considered to
address current parking needs.
. Finally, staff understands the importance of Bethesda Memorial Hospital as an asset to
both the City of Boynton Beach and Palm Beach County and further understands that the
hospital's facilities must be expanded from time to time. For that reason, staff has
requested that the hospital share its long-range development plans with the City so that
we may strive together to achieve mutual objectives in an orderly manner We again
repeat that request.

RESOLUTION
WHEREAS, the City Commission of the City of Boynton Beach, Florida has adopted Ordinance
No. 02-053, in which a Revised zoning Map was adopted for said City; and
WHEREAS, Section 2.e of said ordinance requires the Bethesda Memorial Hospital Board of
Trustees to adopt a Resolution expressing its commitment not to acquire additional property to
the west, north or south of the property, which is the subject of the Ordinance No. 02-053 (see
attached graphic) for further expansion of the Hospital, support facilities or parking areas; and
WHEREAS, the subject property referred within Ordinance No. 02-53 is described as follows:
Lots I through 8, inclusive, Block I, Seacrest Hills, according to Plat thereof as recorded
in Plat Book 25, Page 74, Public Records of Palm Beach County, Florida.
Containing approximately I 43 acres ofland.
WHEREAS, Bethesda Memorial Hospital agreed to Section 2.e of Ordinance No. 02-053, at the
August 20th, 2002 City Commission meetinglPublic Hearing.
NOW THEREFORE, BE IT ORDAINED BY THE BOARD OF TRUSTEES OF BETHESDA
MEMORIAL HOSPITAL, THAT
The foregoing clauses are true and correct and incorporated herein by this reference.
Bethesda Memorial Hospital shall not acquire additional land to the west, north, or south of the
property, which is the subject of the Ordinance No. 02-053, for further expansion of the Hospital,
support facilities or parking areas.

Dear Mr Cronin;
On behalf of the administration and staff at Bethesda Hospital, I am providing you with a
copy of the finalized development plans for the Physician Parking Expansion at Bethesda
Hospital. As I indicated at the neighborhood meeting November 41h, 2002, we wanted to
follow up with you regarding the revised plans. For your convenience. we have attached to
this letter, 8 y;, x'11 copies of the updated plans which include the Site Plan, the Landscape
Plan, Architectural Elevations, a Cross-Section, and a typical photo overlay of the existing
conditions on the site and the proposed structure and landscaping.
As you may recall the initial proposal called for a one-story structure similar in design to a
single-family home. Based on the input we received from City staff and the neighbors, the
structure is still a one-story structure but all openings facing SW 4th Street have been closed
and all sloped concrete panels on the north and south of each building, as well as the slope
and roof on the middle concrete panel of each building have been removed.
In addition, I would liKe to taKe this opportunity to give you the information regarding the
public hearing schedule with regards to this project. As of January 14, 2003, we have
resubmitted the plans to the city's Technical Review Committee (TRC) for review. The
Planning and Development Board meeting is currently sc)1eduled for January 2Slh at
6:00pm. The City Commission hearing will follow and is seheduled ,for' February 4th, at
6;30pm.
We are notifying the residents of SW 4\1l Street and SW 51ft Street with this mailing. It is our
belief that these property owners will be most affected by the design of this project. If you

Dear Mrs. Galav'
Attached are 10 copies of revised plans for the Bethesda Physician Parking Land Use
Amendment and Rezoning. Modifications have been made to these plans to address
outstanding TRC comments and issues presented by residents along SW 4th SI. on
November 4, 2002. The list below outlines each of the modifications that have been made
to the individual plans.
Site PlanILandscape Plans:
. The overall building footprint has been shifted One (1) foot, four (4) inches to the
west, towards the Bethesda Hospital main building. This shift allows for eight (8)
inch architectural projections on the east side of the building, while still maintaining
the required thirty (30) foot setback.
. Two existing ficus trees located on the northern portion of the site, along S. W
West 4th St. will be preserved, at the behest of several neighbors. The
preservation of the trees will help bolster the proposed landscape buffer, as well
as provide continued noise and sight abatement from the existing hospital.
. The Landscape Buffer has been modified to include more densely planted native
vegetation. The landscaping was modified to essentially blanket the parking
structure from view and deter any security related issues with the proposed use.
Large canopy trees and extensive masses of shrubs will be used along the
western side of the structure.

. The proposed sidewalk along S. W 4th St. has been modified, so that it meanders
into the site, providing an opportunity to landscape areas between the property
line and the sidewalk for additional buffer Along the northern portion of the site
the sidewalk will remain adjacent to the property line due to the location of the
existing retaining wall.
. A fence, with intense landscaping for screening has been proposed to separate
the neighborhood with the proposed park that is located between the two parking
structures on the site plan. The green area will allow a break in the buildings, while
the intense landscape will continue to obscure any noxious noises and sights
emanating from the back of the hospital. In addition to the extensive landscape
buffer, the fence is proposed to further deter any criminal activity from wandering
onto the site.
Architectural Plans
. At the November 4, 2002 meeting with residents, neighbors along S. W 4th St. were
concerned about light pollution emanating from the parking structure. As a result all
window openings and mullions that were facing S. W 4th St. have been removed on
the elevations.
. All sloped concrete panels on the north and south of each building, as well as the
slope and roof on the middle concrete panel of each building has been removed.
. Concrete tilt panel with decorative Candy Anderson molding, score lines and raised
iron fa(:ade features have been added to the elevation plans.
. The front panels have been raised to provide uniformity along the length of the
buildings.
. A pedestrian walkway has been added midway in each of the buildings floor plans
to connect the garage to the parking lot.
. One parking space has been relocated on the north side of the north building to
better conform with existing site conditions.

These plans also resolve the following comments:
Fire
1 When design plans are submitted they will show compliance with these
requirements.
2. When design plans are submitted they will show compliance with these

3. As required by City Ordinance automatic fire sprinkler systems will be provided in
both structures.
Enaineerina
4 As requested a site development plan and survey showing all existing utilities has
been included.
5. The site plan shows and identifies all necessary traffic control devices.
6. The carport will be constructed using the Palm Beach County Building Code
standards; therefore, only English units will be used.
7 A supplemental 8" Solid Yellow Striping at the egress of driveways has been added
to the site plan.
8. Stripping has been added as requested.
9. A drainage plan and drainage statement has been provided for this site.
Buildina
10. As requested the finish floor elevation that is proposed is specified in the Site Data.
11 At the time of permit review, a completed and executed City of Boynton Beach Unity
of Title form will be provided.
12.As requested, signed and sealed working drawings will be provided at permit
review

13. As requested a partial elevation view drawing ot the proposed perimeter wall has
been submitted.
14 At the permit review, the applicant will submit tor review an addressing plan tor the
project.
Parks & Recreation
15.At permit review a detailed irrigation plan tor the right-ot-way landscape, and
irrigation improvements will be submitted tor review by the Parks, and Public Works
Departments.
Forrester/Environmentalist
16. As requested any viable existing native vegetation on the site will be preserved and
shown during permit review
17 As requested trees noted on the plant list will be a minimum at 12 teet in height and
3 inches in diameter
Plannina & Zonina
18. The City Commission approved both the small-scale land use amendment and the
rezoning on October 15, 2002.
19 At the time at permit review, a completed and executed City of Boynton Beach Unity
at Title torm will be provided.
20. The elevations for all areas of the site have been provided on the drainage plans.
21 A site plan has been provided that is showing the proposed parking structure as it
relates to the existing hospital property
22. To provide better security the parking structure has been sealed off from the
existing neighborhood to the east and a dense landscape buffer will be planted to
deter any criminal activities tram entering or exiting the site from the west. In
addition, the parking deck will be well lit from within and openings will be open
enough on the east side of the buildings to allow tor high visibility into the structure.
23. The heights for the wood fence have been provided as requested.

staff and the neighboring residents.
