Site Characteristics: The subject site, consisting of eight (8) platted lots, recently zoned Public Use (PU) with a
Public Private G overnmentall nstitutional (PPGI) I and use designation. Four single-family
detached dwelling units are currently located within the subject property (Lots 1, 2, 3, 6, 7
and 8, Block 1, Seacrest Hills). The remaining two (2) lots (Lots 4 and 5, Block 1, Seacrest
Hills) are vacant. These two lots are heavily wooded with a variety of trees including Mango,
Black Olive and one Live Oak. A four-foot chain link fence combined with a wooden fence
surrounds the subject property currently owned and used by the hospital (Lots 1 -5). The
hospital administrator lives in the house located on Lots 1 and 2.
Proposal:
The developer, Bethesda Memorial Hospital, proposes to clear the entire site and construct
two (2), one-story, covered parking garages containing a total of 85 parking spaces. The
north garage will contain 42 spaces and the south garage will contain 43 spaces. A park
area to be used by hospital employees will separate the two buildings.
The hospital has documented the need for additional parking in a study prepared in August
of 1999 by Kimley-Horn and Associates. However, the applicant indicated in the response to
first review comments for this project that "this is an independent project and is not part of
the hospital master plan" The proposed parking area will be designated for doctor parking
only Access will only be from the existing hospital parking lot. (See Exhibit "B" - Proposed
Site Plan). At the August 27, 2002 Planning and Development Board meeting, the applicant
requested that the review of the site plan be tabled. The land use amendment and rezoning
went fOlWard to the City Commission and were approved on October 15, 2002. This site

plan was revised to respond to neighborhood concerns and staff comments.
The Palm Beach County Traffic Division has determined that the project is not a trip
generator and meets the Traffic Performance Standards.
Conceptual drainage information was provided for the City's review The Engineering
Division has no additional comments regarding drainage. Specific drainage solutions
will be finalized at time of permit review
Access to the parking garages would be through the hospital properly Both proposed
garages have two (2) entrances providing both ingress and egress. Each of the four (4)
entrances measure 25 feet in width. No vehicular access is proposed onto SW 4th Street.
The proposed project consists of two (2), one-story covered parking garages that will consist
of a total of 85 spaces. All spaces will be dimensioned nine feet by eighteen feet (9' x 18')
with wheelstops. This parking facility will serve the doctors only These parking spaces are
considered as part of the total parking provided for the hospital. Handicapped spaces will
not be provided at this site. The required handicapped spaces are in the main parking lot.
Interior lighting in the garages is proposed for security purposes.
The 85 parking spaces proposed are considered as part of the total parking provided for the
hospital. The hospital indicates that there are 1,148 spaces. The parking garage project will
eliminate 12 spaces and add 85 for a revised total of 1,221
The plans for the hospital expansion (NWSP 00-007), approved on July 18, 2000, indicate a
total number of 1,183 parking spaces provided. There are currently 1, 148 spaces available
because 35 spaces are allocated to the area used for the construction trailers and
equipment for the on-going expansion and will be permanently eliminated. The hospital,
including the expansion requires 793 spaces per the Land Development Regulations.
Based on the revised number of spaces currently available, 1,148, the hospital has a
surplus of 355 parking spaces. With the proposed addition of the physicians parking, the
hospital will have a surplus of 428 spaces.
Based on field inspections, staff observed that the area adjacent to the proposed garages is
used for parking and loading. This is the back or service area of the hospital. At a minimum,
appropriate signage and traffic control markings may need to be provided in the existing
parking and/or vehicular circulation area.
The site will be heavily buffered to the west by a variety of trees including Live Oak, Sabal
Palm. Pigeon Plum and Orange Geiger Hedge material consists of Oleanders, Sea Grape
and Dune Sunflower This landscape pattern will continue along the north and south sides
of both buildings. Along the east side of the garages facing the hospital there will be
Alexander Palms and Coco Plum Shrubs planted at the vehicular entrances and at the
pedestrian access openings to the garages.
A park is proposed for the 51-foot wide area in between both parking garages. Currently
Lots 4 and 5 are used as a park/lunch area for the hospital employees. The lots are heavily
vegetated and contain picnic tables. Most of the existing trees will be removed and replaced
as part of the proposed landscaping for the parking garages. There is one live oak tree that
will have to be relocated. The applicant is required to provide a plan showing the location of

existing trees and which will be removed, relocated or preserved (see Conditions of
Approval).
It appears from the elevation drawings that the proposed park will be stepped down slightly
in elevation from east to west. The area will be planted with sod and contain a variety of
landscape material. No picnic tables or other seating is shown on the site plan.
Building and Site: Site regulations will be fUlly met when staff comments are incorporated into the permit
drawings.
Community Design: The two garage buildings will be identical in design. They will have painted stucco facades
in Mist Yellow (SW 1681). The base accent color will be Dreamy Peach (SW 1631). An
additional accent color will be Irish Green (SW 1735) used along the roofline. No openings
are proposed along the west or south fa"ade of the buildings facing the residential
neighborhood. As mentioned above, the west side will also be heavily landscaped to
obscure the garages as much as possible. The north and east elevation will have large
window-like openings. The roof is a stepped-up, flat design, with the look of cornice
molding along the top of each parapet. Both buildings will measure 22 feet high to the top of
the highest parapet. This is consistent with the height of the residential dwellings in the
surrounding R-1-AA zoning district.
Regarding security, a six (6) foot vinyl-clad chain link fence is proposed between the front
line of the buildings and the north and south properly line and between the two buildings
themselves. The fence, landscaping, and a solid building fa"ade eliminate any openings
and reduce the opportunities for vandalism.
The proposed design effectively buffers the non-residential use from the neighborhood. All
activity associated with the parking garage is directed inward towards the hospital complex.
Signage:
A "Physicians Only Parking" directional sign will be installed at each entrance to the parking
garage. No other signage is proposed for this site.
RECOMMENDATION:
Staff recommends approval of this revised site plan, subject to the comments, included in Exhibit "C" - Conditions
of Approval. The Technical Review Committee (TRC) recommends that the deficiencies identified in this exhibit
be corrected on the set of plans submitted for building permit.

In reviewing the 2nd Review plans received at TRC for the Physicians Parking project I have a few
additional comments. These issues should be addressed by the applicant at or before the Planning &
Development Board meeting.
1 Add supplemental 8" Solid Yellow Striping (diagonal) at the egress driveways from the parking
structures into the existing parking lot. The intent is to provide a visual barrier between the driveway
aisle and adjacent parking spaces in the existing lot.
2. Add striping for standard stalls at the concrete approach being removed on the east side of the
north building.
3. Previously the drainage plan had been approved due to the applicant's statement that all drainage
would be accommodated under the Master Drainage Plan for the Bethesda Hospital Site. However,
further investigation indicates that three additional lots are being added to accommodate the new
parking structures. These lots are not part of the original Master Drainage Plan. Therefore the
applicant will need to revise the Master Drainage Plan accordingly and show that the drainage
system will handle the proposed development and added run-off

CODE REQUIREMENTS
Design documents shall demonstrate compliance with LDR Chapter 6,
Section 16, which provides requirements for hydrants. Hydrants in
commercial applications shall be no more than 300 ft. apart and the
remotest part of any structure shall be no more than 200 ft. from a
hydrant. Connections shall be to mains no less than 6 inches in diameter
In addition to domestic requirements at a residual pressure of not less
than 20 psi, a fire flow of at least 1500 gpm is required.
Design documents where underground water mains and hydrants are to
be provided, must demonstrate that they will be installed, completed, and
in service prior to construction work per the Florida Fire Prevention Code,
(2000) Section 29-2.3.2.
City Ordinance Section 9-6, 3 17-11 requires approved automatic fire
sprinkler systems throughout all buildings or structures regardless of the
type of construction that are in excess of 12,000 square feet per floor

1 Add a labeled symbol to the site plan drawing that identifies the location of the
handicap accessible parking spaces. The quantity of the spaces shall be
consistent with the regulations specified in the Florida Accessibility Code for
Building Construction The accessible parking spaces that serve a use shall be
located on the shortest safely accessible route of travel from adjacent parking to an
accessible entrance. The Florida Accessibility Code for Building Construction
states that buildings with multiple accessible entrances shall have accessible
parking spaces dispersed and located closest to the accessible entrance. Florida
Accessibility Code for Building Construction, Section 4 1.2(5), 4 3, 4 6
2. Add a labeled symbol to the site plan drawing that represents and delineates the
path of travel for the accessible route that is required between the accessible
parking spaces and the accessible entrance doors to the structures. The symbol,
required to be installed along the path, shall start at the accessible parking spaces
and terminate at the accessible entrance doors to the structures. The symbol shall
represent the location of the path of travel, not the location of the detectable
warning or other pavement markings. The location of the accessible path shall not
compel the user to travel in a drive/lane area that is located behind parked
vehicles. Identify on the plan the width of the accessible route. (Note: The
minimum width required by the code is forty-four (44) inches). Add text to the
drawing that would indicate that the symbol represents the accessible route and
the route is designed in compliance with Section 43 (Accessible Route) and 46

(Parking and Passenger Loading Zones) of the Florida Accessibility Code for
Building Construction Please note that at time of permit review, the applicant shall
provide detailed documentation on the plans that will verify that the accessible
route is in compliance with the regulations specified in the Florida Accessibility
Code for Building Construction. This documentation shall include, but not be
limited to, providing finish grade elevations along the path of travel.
3 Identify within the site data the finish floor elevation (lowest floor elevation) that is
proposed for the parking structures. Verify that the proposed elevation is in
compliance with regulations of the code by adding specifications to the site data
that address the following issues [Section 3107 1.2, Chapter 31 of the 2001 Florida
Building Code]:
a) The design professional-of-record for the project shall add the following text to
the site data. "The proposed finish floor elevation _ _ NGVD is above the
highest 100-year base flood elevation applicable to the building site, as
determined by the South Florida Water Management District's surface water
management construction development regulations."
b) From the FIRM map, identify in the site data the title of the flood zone that the
building is located within. Where applicable. specify the base flood elevation. If
there is no base flood elevation, indicate that on the plans.
c) Identify the floor elevation that the design professional has established for the
building within the footprint of the building that is shown on the drawings titled
site plan, floor plan and paving/ drainage (civil plans).
6 On the drawing titled site plan, identify and label the symbol that represents the
property line.
7 Place a note on the elevation view drawings indicating that the wall openings and
wall construction comply with Table 600 of the 2001 Florida Building Code.
8 At time of permit review, provide a completed and executed City of Boynton Beach
Unity of Title form. The form shall describe all lots. parcels or tracts combined as
one lot. A copy of the recorded deed with legal descriptions, of each property that
is being unified, is required to be submitted to process the form. The property
owner that is identified on each deed shall match.
9 At time of permit review, submit signed and sealed working drawings of the
proposed construction.
10 Add to the submittal a partial elevation view drawing of the proposed perimeter
wall/fence. Identify the type of the wall material and the type of material that
supports the wall. including the typical distance between supports. Also, provide a
typical section view drawing of the wall that includes the depth that the wall
supports are below finish grade and the height that the wall is above finish grade.
The location and height of the wall shall comply with the wall regulations specified
in the Zoning Code.

11 Add to the drawing the calculations that were used to identify the minimum number
of required handicap accessible parking spaces. Also, state the code section that
is applicable to the computations.
12. Add a labeled symbol to the site plan drawing that represents and delineates the
path of travel for the accessible route that is required between the accessible
parking spaces and the accessible entrance doors to the structures. The symbol
shall start at the accessible parking spaces and terminate at the accessible
entrance door to the structures. The symbol shall represent the location of the path
of travel. not the location of the detectable warning or other pavement markings
required to be installed along the path. The location of the accessible path shall not
compel the user to travel in a drive/lane area that is located behind parked
vehicles. Identify on the plan the width of the accessible route. (Note. The
minimum width required by the Code is 44 inches). Add text that would indicate
that the symbol represents the accessible route and the route is designed in
compliance with regulations specified in the Fair Housing Act. Please note that at
time of permit review, the applicant shall provide detailed documentation on the
plans that will verify that the accessible route is in compliance with the regulations
specified in the Florida Accessibility Code for Building Construction. This
documentation shall include, but not be limited to, providing finish grade elevations
along the path of travel.
13. At time of permit review, submit for review an addressing plan for the project.

location and perimeter of the limits of construction proposed with the subject

The above referenced Site Plans, forwarded on May 22, 2002, were reviewed against the
appropriate requirements outlined in the City of Boynton Beach Code of Ordinances (available
on-line at www.cLbovnton-beach.fl.us). Following are the comments with the appropriate Code
and Land Development Regulations (LDR) referenced.
PUBLIC WORKS
No comments.
ENGINEERING
Code Requirements
1 Add a general note to the Site Plan that all plans submitted for specific permits shall meet
the City's Code requirements at time of application. These permits include, but are not
limited to, the following: paving, drainage, curbing. site lighting, landscaping and irrigation.
Permits required from other permitting agencies such as Florida Department of
Transportation (FDOT), South Florida Water Management District (SFWMD), Lake Worth
Drainage District (LWDD), Florida Department of Environmental Protection (FDEP), Palm
Beach County Health Department (PBCHD), Palm Beach County Engineering Department
(PBCED), Palm Beach County Department of Environmental Resource Management
(PBCDERM) and any others, shall be included with the permit request.
2. Show all existing utilities, including water, sanitary and storm sewer, on Site Characteristics
Map (Survey) and Site Development plan (LDR, Chapter 4. Section 7.A.3).
3 Show the proposed site lighting on the Site and Preliminary Site Engineering plans (LDR,
Chapter 4, Section 7.B 4) The lighting design shall provide a minimum average light level of
one foot-candle. On the Lighting Plan, specify that light poles shall withstand a 110 MPH
wind load (LDR, Chapter 23, Article II, Section A.1.a.). Add a note that the fixtures shall be

4 operated by photo-electrical control and are to remain on until 2:00 a.m. (LOR, Chapter 23,
Article II, Section A.1.a.). Artificial lighting used to illuminate any property shall be directed
away from all residential districts. (LOR, Chapter 2, Section 4 N.7 & Chapter 9, Section
10 F 5)
5 A Lighting Plan including photometrics, pole wind loading, and pole details in conformance
with the LOR, Chapter 6, Article IV, Section 11, Chapter 23, Article I, Section 5 B.7 and
Chapter 23, Article II, Section A will be required at the time of permitting. If possible please
provide this information as part of your TRC plan submittals - it is much easier to identify
and correct any deficiencies now than while you are waiting on a permit!
6 It may be necessary to replace or relocate large canopy trees adjacent to light fixtures to
eliminate future shadowing on the parking surface (LOR, Chapter 23, Article II, Section
A.1.b) Unable to determine this as lighting is not shown on the plans.
7 Landscaped areas shall be protected from vehicular encroachment by a continuous, raised
curb (LOR, Chapter 75, Article II, Section 5.A. and Chapter 22, Article II, Section E.).
8. A drainage plan in accordance with the LOR, Chapter 4, Section 7 F.2, including the
engineer's certification, is required as part of the site plan submittal plan. Existing and
proposed elevations, flow arrows, existing and proposed drainage structures, and
retention/detention ponds are to be included in the drainage plan for review and comment.
9 A four (4) foot wide sidewalk shall be placed one (1) foot off the property line along SW 4th
Street (LOR, Chapter 6, Article IV, Section 10 T , and Chapter 22, Article I, Section 5)
10 Show sight triangles on the Site and Landscape plans (LOR, Chapter 75, Article II, Section
5.H ).
11 Add a note to the Landscape Plan that within the sight triangles there shall be an
unobstructed cross-visibility at a level between 2.5' and 6' above the pavement (LOR,
Chapter 75, Article II, Section 5 H.)
12. On the Site and Civil plans, show and identify all necessary traffic control devices such as
stop bars, stop signs, double yellow lane separators striping, directional arrows, and "00 Not
Enter" signage, etc. See City Standard Orawings B-98001 and B-90013 for striping details.
Recommendations/Corrections
1 Indicate to what standard the project is to be constructed; if the FOOT Standard
Specifications for Road & Bridge Construction and Standard Index are to be used - the 2000
Specifications are recommended since they contain both English and Metric units.

2. Show a north arrow for the "Location Map" on Sheet SP-1 since it is oriented differently from
the remainder of the plan drawings.

5 Add a general note to the site plan that all plans submitted for specific permits
shall meet the City's Code requirements at time of application. These permits
include, but are not limited to, the following: paving, drainage, curbing, site
lighting, landscaping and irrigation. Permits required from other permitting
agencies such as Florida Department of Transportation (FDOT), South
Florida Water Management District (SFWMD), Lake Worth Drainage
District (LWDD), Florida Department of Environmental Protection (FDEP),
Palm Beach County Health Department (PBCHD), Palm Beach County
Engineering Department (PBCED), Palm Beach County Department of
Environmental Resource Management (PBCDERM) and any others, shall be
included with the permit request.
6. Show all existing utilities, including water, sanitary and storm sewer, on site
characteristics map (Survey) and site development plan (LDR, Chapter 4,
Section 7.A.3).
7 Show the proposed site lighting on the site and preliminary site engineering
plans (LDR, Chapter 4, Section 7.B.4.) The lighting design shall provide a
minimum average light level of one foot-candle. On the lighting plan, specify
that light poles shall withstand a 110 MPH wind load (LDR. Chapter 23,
Article II, Section A.1.a.). Add a note that the fixtures shall be operated by
photo-electrical control and are to remain on until 2:00 a.m. (LDR, Chapter
23, Article II, Section A1.a.). Artificial lighting used to illuminate any
property shall be directed away from all residential districts. (LDR, Chapter
2, Section 4.N.7 & Chapter 9, Section 10.F.5).
8. A lighting plan including photometrics, pole wind loading, and pole details in
conformance with the LDR, Chapter 6, Article IV, Section II, Chapter 23,
Article I, Section 5.B.7 and Chapter 23, Article II, Section A will be required
at the time of permitting. If possible please provide this infonnation as part
of your TRC plan submittals - it is much easier to identifY and correct any
deficiencies now than while you are waiting on a permit.
9 It may be necessary to replace or relocate large canopy trees adjacent to light
fixtures to eliminate future shadowing on the parking surface (LDR, Chapter
23, Article II, Section A1.b). Unable to determine this as lighting is not
shown on the plans.
10 Landscaped areas shall be protected from vehicular encroachment by a
continuous, raised curb (LDR, Chapter 7.5, Article II, Section 5.A and
Chapter 22, Article II, Section E.).
II A drainage plan in accordance with the LDR, Chapter 4, Section 7.F.2,

the handicap accessible parking spaces. The quantity of the spaces shall be
consistent with the regulations specified in the Florida Accessibility Code for
Building Construction. The accessible parking spaces that serve a use shall be
located on the shortest safely accessible route of travel from adjacent parking
to an accessible entrance. The Florida Accessibility Code for Building
Construction states that buildings with multiple accessible entrances shall
have accessible parking spaces dispersed and located closest to the accessible
entrance. Florida Accessibility Code for Building Construction, Section
4 1.2(5),4.3,4.6.
26. Add a labeled symbol to the site plan drawing that represents and delineates
the path of travel for the accessible route that is required between the
accessible parking spaces and the accessible entrance doors to the structures.
The symbol, required to be installed along the path, shall start at the
accessible parking spaces and terminate at the accessible entrance doors to the
structures. The symbol shall represent the location of the path of travel, not
the location of the detectable warning or other pavement markings. The
location of the accessible path shall not compel the user to travel in a
drivellane area that is located behind parked vehicles. IdentifY on the plan the
width of the accessible route. (Note: The minimum width required by the
code is forty-four (44) inches). Add text to the drawing that would indicate
that the symbol represents the accessible route and the route is designed in
compliance with Section 4.3 (Accessible Route) and 4.6 (parking and
Passenger Loading Zones) of the Florida Accessibility Code for Building
Construction. Please note that at time of permit review, the applicant shall
provide detailed documentation on the plans that will verifY that the
accessible route is in compliance with the regulations specified in the Florida
Accessibility Code for Building Construction. This documentation shall
include, but not be limited to, providing finish grade elevations along the path
of travel.
27 Identify within the site data the finish floor elevation (lowest floor elevation)
that is proposed for the parking structures. VerifY that the proposed elevation
is in compliance with regulations of the code by adding specifications to the
site data that address the following issues [Section 3107 1.2, Chapter 31 of

a) The design professional-of-record for the proj ect shall add the

the drawings titled site plan, floor plan and paving! drainage (civil
plans).
28. On the drawing titled site plan, identify and label the symbol that represents
the property line.
29 Place a note on the elevation view drawings indicating that the wall openings
and wall construction comply with Table 600 of the 2001 Florida Building

30. At time of permit review, provide a completed and executed City of Boynton
Beach Unity of Title form. The form shall describe all lots, parcels or tracts

31. At time of permit review, submit signed and sealed working drawings of the

32. Add to the submittal a partial elevation view drawing of the proposed
perimeter wall/fence. Identify the type of the wall material and the type of
material that supports the wall, including the typical distance between
supports. Also, provide a typical section view drawing of the wall that
includes the depth that the wall supports are below finish grade and the height
that the wall is above finish grade. The location and height of the wall shall
comply with the wall regulations specified in the Zoning Code.
33 Add to the drawing the calculations that were used to identify the minimum
number of required handicap accessible parking spaces. Also, state the code
section that is applicable to the computations.
34 Add a labeled symbol to the site plan drawing that represents and delineates
the path of travel for the accessible route that is required between the

SITE PLAN REVIEW PROCEDURES

1 ST Review - New Site Plan

Find attached for your review the plans and exhibits for the above-referenced project. Please review the
plans and exhibits and transmit formal written comments. Comments should be made available
via e-mail to Sherie Coale and I no later than 5:00 P.M. on Thursdav June 6, 2002. When preparing
your comments, please separate them into two categories; code deficiencies with code sections
referenced and recommendations that you believe will enhance the project.
Adhering to the following review guidelines will promote a comprehensive review and enable the
applicant to efficiently obtain Technical Review Committee approval:
1 Use the review standards specified in Part IV, Land Development Regulations, Site Plan Review
and the applicable code sections of the Code of Ordinances to review and formulate comments.
2. The documents submitted for the project were determined to be substantially complete with the
exception of traffic data, however, if the data provided to meet the submittal requirements is
insufficient to properly evaluate and process the project based on the review standards or the
documents show code deficiencies, additional data and/or corrections should be requested by the
reviewer by contacting Lusia Galav, or myself
3. Each comment shall reference the section of the code that is incorrectly depicted on the
documents.
4 Technical Review Committee member(s) shall identify in their comments when the plans depict or
when the location and installation of their departmental required improvements may conflict with
other departmental improwments.

5. When a TRC Member finds a code deficiency that is outside of his/her review responsibility, the
comment and the specific code section may be included in their review comments with the name of
the appropriate TRC Member that is responsible for the review specified.
6. If a TRC member finds the plans acceptable, he/she shall forward a memorandum, within the time
frame stated above, to me. The memorandum shall state that the plans are approved and that they
do not have any comments on the plans submitted for review and that they recommend the project
be forwarded through the approval process.
All comments shall be typed, addressed and transmitted or e-mailed to Sherie Coale and I for distribution
to the applicant. Please include the name and phone number of the reviewer on this memorandum or e-
mail. Lusia Galav will be the Planning and Zoning staff member coordinating the review of the project.
First review comments will be transmitted to the applicant along with a list of Technical Review Committee
(TRC) members.
