Dear Ms. Elijah:
In response to your request dated November 3, 2006 for the
administrative review and approval of the modifications proposed to
the above-referenced approved site plan, depicting the following:

Please be informed that the proposed changes on the revised
date stamped 11/3/06 are "minor" as defined within the
Development Regulations, Chapter 4 Si te Plan Review.

1. Since the sound data provided is very close to the maximums allowed
in our noise ordinance, any determination that those sound levels
are being exceeded will require further mitigation on the part of
the Hospital.
2. At time of permitting, provide a Utility Plan (at a scale no
smaller than 1 in. = 40 ft.) clearly showing existing and proposed
utilities (water and sanitary sewer main) for the proposed
expansion. Show the locations of both the existing water meter and
sanitary sewer lateral servicing the current utility plant and the
proposed water meter and sanitary lateral. Indicate dimensions of
existing and proposed meters and laterals.
3. Water & Sewer plans will be required to be submitted and approved
by the City's Utilities Dept. Upon receipt and review of the Water
& Sewer plans additional comments may be generated that could
affect the location of the proposed services. Please provide a
draft utility plan with the requested information at your earliest
convenience; complete plans will be accepted during the permitting
process.
4. The CODE, Section 26-34(E) requires that a capacity reservation fee
be paid for this project either upon the request for the
Department's signature on the Health Department application forms

or within seven (7) days of site plan (minor modification)
approval, whichever occurs first. This fee will be determined
based upon final meter size, or expected demand.
5. Provide additional banding to correspond with the existing
structure and paint to match.
Be advised that the proposed changes may require a modification to

Dear Mr. Breese:
This letter is to request a Minor Site Plan Modification for the Bethesda Memorial Hospital
Utility Plan Expansion. The proposed addition to the hospital utility plant provides 2,907
square feet. This is to house new chillers, boilers, and generators to~erve the new
Comprehensive Heart Center currently under construction.
The expansion project poses minimal visual impact to the adjoining neighborhood. This .
has been demonstrated by the existing photos and perspective renderings included with
this submittal.
While the expansion necessitates the removal of ten existing parking spaces, any
accessible parking initially impacted by this expansion project has been r~located,
maintaining the original count of 54 (only 28 are required). The remaining overall parking
count on the site is 1,804, which still exceeds the required number of 801.
We would like to request your consideration on this Minor Site Plan Modification and that
the process can be reviewed in a timely manner to be able to continue through. the
permitting process.
