Based on photographs and signed as-built construction documents provided to CBVR
Telecom Design Group by the contractor(UCI Construction Services, LLC) on August 8,
2017, I hereby attest to the best of my knowledge, belief, and professional judgment
that the installation of wireless communications equipment for the aforementioned
project was constructed in general conformance with the approved civil construction
documents by CBVR Telecom Design Group, dated November 16, 2016:
• Removal of(6) antennas
• Installation of(6) new antennas
• Installation of(6) diplexers and (6)triplexers at antenna mounts
• Installation of(6) bias-t's at antenna mounts
• Installation of(6) diplexers and (6)triplexers at equipment pad
• Replacement of T-arm mounts with platform mount
The applicable codes to this installation include:
• Florida Building Code 2015 (5`" edition)
• ASCE 7-10 (ult. wind speed = 170 mph, risk cat. = II, exp. = C)
