R98-189RESOLUTION NO. R98-/~'? A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF BOYNTON BEACH, FLORIDA, AUTHORIZING DIANE REESE, FINANCE DIRECTOR OF THE CITY OF BOYNTON BEACH, TO EXECUTE CERTAIN FINANCIAL REPORTS AND DOCUMENTS ON BEHALF OF THE CITY OF BOYNTON BEACH, WHICH ARE REQUIRED UNDER FLORIDA ADMINISTRATIVE CODE RULE 4C-2.032, TO BE SUBMITTED TO THE DEPARTMENT OF TREASURY; AND PROVIDING AN EFFECTIVE DATE. WHEREAS, pursuant to Rule 4C-2.032, Florida Administrative Code, certain documents and reports must be filed and submitted to the Department of the Treasury which require execution by an authorized representative of the City of Boynton Beach; and WHEREAS, the Finance Director is the preparer of said reports and documents, and is responsible for their filing with the proper agencies within prescribed time frames; NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE CITY OF BOYNTON BEACH, FLORIDA, THAT: Section 1. The City Commission of the City of Boynton Beach, Florida hereby authorizes and directs Diane Reese, the Finance Director of the City of Boynton Beach, to execute financial reports and documents, as set out in Florida Administrative Code Annotated, Rule 4C-2.032, which Section is attached hereto and made a part hereof. Section 2. This Resolution shall become effective immediately upon passage. PASSED AND ADOPTED this / day of December, 1998. (1) When the following forms, listed in 4C-2.009, are submitted to the Treasurer, they shall be accompanied by proof that the individuals executing the forms on behalf of the organization are authorized to execute the forms on behalf of the organization: (a) DI4-1000 (rev. 3/97)Contingent Liability Agreement and Affidavit Co) DI4-1001 (rev. 3/97)Public Depository Pledge Agreement (c) DI4-1004 (rev. 3/92)Public Depository Annual Report (d) DI4-1008 (rev. 3/92)Power of Attorney (e) DI4-1009 (rev. 3/92)Public Depositor Report to the Treasurer (f) DI4-1012 (rev. 3/92)Pubhc Deposits Claim Form and Agreement (2) The proof specified in subsection (1) above shall be in the form of a certified copy, or a certified pertinent extract from, one of the following: (a) The minutes of a board meeting evidencing adoption of a resolution by the organization's board of directors authorizing the individual signing the form by name or title to sign such a document on behalf of the organization; (b) 'The organization's charter, constitution, bylaws, or other evidence of the governing body, authorizing the individual signing the form by name or title to sign such a document on behalf of the organization; (c) A written resolution by the organization's board of directors authorizing the individual signing the form by name or title to sign such a document on behalf of the organization; or (d) Evidence of incumbency for constitutional public officers.
